วสยทศน (Vision)

จะเปนหนวยจายกลางหลกของโรงพยาบาลรามาธบด ในการลาง ตรวจสอบอปกรณ จดชดอปกรณ ท าลายเชอท าใหปราศจากเชอ จดเกบชดอปกรณและแจกจายใหแกหนวยงานตางๆ ในโรงพยาบาลรามาธบด โดยค านงถงคณภาพและความปลอดภยสงสด ดวยมมาตรฐานสากล

วธการเขยนนโยบาย

นโยบาย คอ แนวทางปฏบตเพอบรรลเปาประสงคเปนตวควบคมทศทาง ตองยดหยน ปรบเปลยนได เมอม feed back กลบมา โดยตองรวตถประสงคองคการ /องคกรนนๆกอน และองคประกอบส าคญ - มขอบบเขต(confine, extent)ชดเจน และแนวทางทมงหวง โดยมนยามเสรมเปนระยะๆ เพอความเขาใจทเหมอนกน

- มนยามศพท (Definition) หรอมโนภาพ (concept) เพอไมตความผด

- กรอบเวลา (time frame) แนนอนใหชดวาควรแลวเสรจเมอใดในแตละขน แตละชวง

- มตวชวด หรอ ตวประเมนผล (key performance)

นโยบายของหนวยงานเวชภณฑปลอดเชอ โรงพยาบาลรามาธบด (Policy)

จะใหบรการดวยการด าเนนงานแบบครบวงจรโดยเปนศนยกลาง (CENTRALIZATION) ในการลาง ตรวจสอบอปกรณ จดชดอปกรณ ท าลายเชอท าใหปราศจากเชอ จดเกบชดอปกรณและแจกจายใหแกหนวยงานตางๆ ในโรงพยาบาลรามาธบด

- เพอลดการแพรกระจายเชอโรค และควบคมโรคตดเชอ และการตดเชอของผ ปวยในโรงพยาบาลฯ - เพอลดขนตอนการท างานทซ าซอน และลดคาใชจายดวยมาตรฐานเดยวกนและเปนมาตรฐานสากลทเปนทยอมรบภายในปพ.ศ. 2012 ดวยการบรหารจดการททนสมยและมประสทธภาพและประสทธผล โดยค านวณถงความปลอดภยของผ ปวยเปนหลกส าคญ

พนธกจ (Mission)

1. เพอใหบรการการลาง ตรวจสอบอปกรณ จดชดอปกรณ ท าลายเชอท าใหปราศจากเชอ จดเกบชดอปกรณและแจกจายใหแกหนวยงานตางๆ อยางมคณภาพและทนตอเวลา

2. เพอลดการแพรกระจายเชอโรค และควบคมโรคตดเชอ และการตดเชอของผ ปวยในโรงพยาบาลฯ

เปาหมาย (Target)

1. ใหบรการชดตรวจ/ชดท าหตถการ/เครองมอ/เครองใช/อปกรณ และ/หรอวสดสนเปลองทางการแพทยทปราศจากเชอและไมปราศจากเชอ ดวยความถกตอง ครบถวน อยในสภาพพรอมใชงาน ผ รบบรการพงพอใจ เพอสนบสนนการบรการรกษาพยาบาล การศกษา การวจย และการบรการวชาการ

2. มมาตรฐานการปฏบตงานดวยมาตรฐานสากล คอ ANSI/AAMI ST79 (USA) มาตรฐานหนวยจายกลางของ กระทรวงสาธารณสขและ มาตรฐาน JCI

3. มการน าเทคโนโลยการบรหารจดการสมยใหมเขามาใชเพอรกษามาตรฐานงานและพฒนาประสทธภาพการท างานอยางตอเนอง

4. สามารถใหบรการฯ อยางเพยงพอตอการขยายตวของความตองการบรการของโรงพยาบาลรามาธบดใน 5 ปขางหนา

5. สามารถเปนทศกษาดงานจากหนวยจายกลางทอนๆ ได 6. มการปฏบตงานอยางเปนระบบและมประสทธภาพสงขนอยางตอเนองทก ๆ ป

ขอบเขตบรการ (Scope of work)

1. การท าความสะอาดและการจดเตรยม

1.1 รบชดตรวจ/ชดท าหตถการตางๆ ทงานเวชภณฑปลอดเชอเปนผจดเตรยมใหและผานการใชงานแลวจากหอผ ปวยและหนวยงานตาง ๆ มาท าความสะอาดและจดเขาชด

1.2 บรรจวสดสนเปลองทางการแพทยตามจ านวนและชนดตางๆ ใสซองบรรจเวชภณฑทางการแพทย

1. 3 จดเตรยมวสดสนเปลองทางการแพทยประเภทตาง ๆ แบบไมปราศจากเชอใหงานบรการผาและหนวยงานตางๆ เบก

2. การท าใหปราศจากเชอ

2.1 ท าใหปราศจากเชอชดตรวจ/ชดท าหตถการตางๆ ของหอผปวยและหนวยงานตางๆ ทงาน เวชภณฑปลอดเชอรบมาท าความสะอาดและจดเขาชด 2.2 ท าใหปราศจากเชอชดตรวจ/ชดท าหตถการ/เครองมอ/อปกรณทางการแพทยตางๆ ทหอผปวยและหนวยงานตางๆ น ามาฝากท าใหปราศจากเชอ ทงประเภททจดเตรยม/หอเรยบรอยแลวและทงาน เวชภณฑปลอดเชอตองบรรจใสซองเวชภณฑทางการแพทยให กรณทหอผปวย/หนวยงานไมไดจดเตรยม/หอมา 2.3 ท าใหปราศจากเชอวสดสนเปลองทางการแพทยชนดตางๆ ประเภทใสซองบรรจเวชภณฑทางการแพทย เพอใหหอผปวยและหนวยงานตางๆ เบกไปใช 2.4 รบชดตรวจ/ชดท าหตถการ/เครองมอ/เครองใช/อปกรณทางการแพทยทจดเตรยมเรยบรอยแลวจากงานทนตกรรมและหองคลอด เพอท าใหปราศจากเชอ และน าสงคนหนวยงาน 2. 5 รบท าใหปราศจากเชอเครองมอ/เครองใช/อปกรณทางการแพทยประเภทตางๆ จากหนวยงานภายนอกโรงพยาบาล ตามนโยบายของคณะฯ

3. บรการอนๆ

3.1 สงหอผาประเภทตางๆ และหอผาผาตดทปราศจากเชอแลวใหหองผาตดและหองคลอด 3.2 บรการจดเตรยมและอานผลหอทดสอบประสทธภาพเครองท าใหปราศจากเชอดวยไอน าภายใตความดนใหกบหองผาตดตางๆ ภายในโรงพยาบาล 3.3 บรการใหยมเครองมอ/เครองใชและ/หรออปกรณทางการแพทยทงเปนชนและเปนชดตรวจ/ชดท าหตถการ แบบปราศจากเชอและไมปราศจากเชอ เพอการบรการรกษาพยาบาล การศกษา และการวจย 3.4 เปนสถานทดงานดานการศกษา ส าหรบ - นกศกษาพยาบาลหลกสตรการพยาบาลเฉพาะทางสาขาการพยาบาลควบคมการตดเชอ - นกศกษาแพทยรามาธบดปท 5 หลกสตรเวชศาสตรครอบครว

กลยทธ 1. ตองมแผนภมโครงสรางการปฏบตงานและแผนการด าเนนงานการท า ใหปราศจากเชอส าหรบอปกรณทาง

การแพทย รวมถงการก าหนดบทบาท หนาทความรบผดชอบของบคลากรผปฏบตงานเปนลายลกษณอกษร 2. มวางแผนก าลงการผลตการใหบรการอยางเปนระบบ เพอรองรบปรมาณความตองการการบรการในอนาคต 3. ปรบปรงแผนผงและการไหลของงานทงภายในและภายนอก และวธการปฏบตงาน วธการด าเนนงานใหม

ประสทธภาพและคณภาพเพมขนอยางตอเนองดวยเทคโนโลยการบรหารจดการสมยใหม 4. ตองมคมอการปฏบตงานไดครอบคลมขอบขายความรบผดชอบของหนวยงานททนตอปจจบนและองตามหลก

วชาการและเปนทยอมรบใหบคลากร ในหนวยงานถอปฏบต คอ การลางท าความสะอาด การเตรยมและการจดชดหออปกรณ การน าหออปกรณเขาเครอง การท าใหปราศจากเชอ การเกบชดหออปกรณทปราศจากเชอ และการแจกจายชดหออปกรณปราศจากเชอ

5. ตองมการจดระบบการประสานงานเพอท าการสอสารกบหนวยงานทเกยวของไว อยางชดเจน เปนลายลกษณอกษรในการรายงานขอมลเกยวกบการปฏบตดานการท าให ปราศจากเชอ รวมถงผลการด าเนนงานรวมทงการแกไข ปญหาทเกดขนจากการท าใหปราศจากเชอ เชน การ เรยกคนอปกรณ ในกรณทผลการตรวจสอบทางชวภาพไมผาน

6. ตองมระบบบรหารจดการคณภาพ ตรวจตดตาม ควบคมคณภาพในการใหบรการทนาเชอถอได 7. ตองมการจดการดานงบประมาณ รวมทงการจดสรรอปกรณและเครองมอใหเพยงพอในการใหบรการแก

หนวยงาน จายกลางและหนวยงานตาง ๆ เชน เครองท าใหปราศจากเชอ เครองลาง สารท าความสะอาดและอปกรณทชวยในการลาง วสดทใชในการหออปกรณ งบประมาณในการบ ารงรกษาเครองมอ งบประมาณในการจดสรรเครองปองกนรางกายส าหรบบคลากรในการปฏบตงาน ฯลฯ

Area Requirements : 1. Floors and walls

- Should have no ridges or bumps

- Material will withstand daily or more frequent wet cleaning and the application of chemical agents

- Carpet should not be used in work areas

- Walls should be constructed of material capable of withstanding frequent cleaning

- Wall protectors should be installed at the level of possible cart impacts

- Materials used in floors and walls should not be of a articulate-or fiber-shedding composition

2. Ceiling

- Work area ceiling should be constructed to create a flush surface with recessed, enclosed fixtures

- Pipes and other fixtures above work areas should be enclosed

- Ceiling should be constructed of materials that are not of a particulate-or fiber shedding compositon

3. Doors

- Doors should be made of a durable material that can withstand bumping from back tables and carts and that can be cleaned frequently

- Doors should open easily following the one-way directional workflow and should not have thresholds

4. Ventilation

- The ventiliation system should be designed so that airflow patterns will not allow air contaminants to enter clean areas

- Air should flow from areas of positive pressure to areas of negative pressure

- Air from rooms or areas under negative pressure should be exhausted to the outside via a nonrecirculating system

- The soiled and decontamination area should be designed so that air flows into the area (negative pressure), with a minimum of 10 air exchanges per hour, and so that all air is exhausted to the outside atmosphere

4. Ventilation

- Whenever possible, dedicated local exhaust systems should be used in place of dilution ventilation to reduce exposure to hazardous gases, vapors, fumes, or mists

- Each functional area has its own requirements for air flow, number of air exchanges and exhaust

8. Handwashing facilities

Alcohol-based waterless hand hygiene agents should be used with hand-free equipment and backup power system to ensure its continued operation

9. Special area requirements and restrictions

9.1 Decontamination area

9.2 Preparation area

- Storage of attire for visitors (e.g.Head covers, cover gowns),

supplies for cleaning the preparation area (e.g. detergents, towels),

monitoring and record-keeping supplies(e.g. sterilization process monitoring devices, log books), packaging materials and preparation supplies (e.g. cotton balls, gauze dressing , tip protectors)

9.3 Sterilization area

- The floors, walls, and ceiling surfaces should be constructed of nonporous material that will withstand frequent cleaning and wet conditions

- Sterilizers should be located in a restricted-access area and should not be located in high-traffic areas or near any potential sources of contamination, such as scrub sinks, clinical sinks or hoppers, wash sinks, or containers for the disposal of linen and trash

- Sterilizers to be used for flash sterilization of unwrapped items for immediate use should be located in the restricted areas of the surgical suite or treatment site, where personnel are required to wear full surgical attire, to cover all head and facial hair, including sideburns and necklines and to wear masks in the vicinity of open sterile supplies

- Policies and procedures should be in place to ensure that sterilization cycle parameters are verified before each use

- Air intake or return ducts should not be located in the area designated for cool-down.

- The temperature in the sterilizer access area should not exceed that specified in the manufacturer’s instructions

- If EO sterilizers and other chemical sterilants are used in the same area as steam sterilizers, the area must be designed and engineering controls must be established to comply with OSHA regulations for control of occupational exposure to EO (29CFR1910.1047), formaldehyde (29CFR1910.1048) and other air contaminants (29CFR1910.1000)

- The ventilation system should be designed and balanced to provide controlled, directional air flow from the sterilization area to the access area, both to remove EO and other air contaminants and to minimize microbial contaminants in the area (AIA,2001)

- The air exhausted from EO sterilizers could expose personnel to excessive levels of EO.(ANSI/AAMI ST41)

9.4 Sterile storage

- The storage system (e.g. open wire shelves, open solid shelves or closed cabinets) should be selected based on the environment in which it will be used, the packaging materials and system used, the types of devices packaged, and the handling procedures employed at the health care facility

- The ventilation system should be positive - Maintenance of the sterility of a device to the point of use is

essential. Because most packaging does not provide an absolute microbial barrier, it is important that environmental contamination be minimized to avoid compromising the sterility of devices during storage

10. Emergency eyewash/shower equipment

- Suitable eyewash/shower equipment must be available, with unobstructed access, for immediate emergency use in all locations where potentially damaging chemical (e.g. instrument cleaning solutions and disinfectants, EO) are used

- Minimum of 0.4 gallons per minute continuously for at least 15 minutes, that they be designed to flush both eyes simultaneously, and that they have a “free-hand, stay open” feature once activated

- Emergency eyewash units should be located within 10 seconds travel time of all chemical usage location

- The eyewash facilities should be identified with a highly visible sign, should be maintained in accordance with the manufacturer’s instructions, and should be tested routinely to ensure proper operation

11. Housekeeping procedures

- Floors and horizontal work surfaces should be cleaned at least daily

- Walls, storage shelves and air intake and return ducts should be cleaned on a regularly scheduled basis and more often if needed

- Stained ceiling tiles should be replaced and any leaks causing the stain should be repaired

- Lighting fixtures or covers should be cleaned at least once every 6 months

• น าทใชกบการก าเนดไอน า ควรเปนน าทผานการปรบปรงคณภาพอยางนอยทสดควรเปนน าออน ซงเครองนงบางรนตองใชน าในระดบ Deionization หรอมาตรฐานของน าทใชเครองนงฆาเชอตามาตรฐาน ISO 13683
