Home Delivered Meal Programs

Need home delivered meals?

Santa Clara County Meals on Wheels Program provides meal service to homebound older

adults 60 years of age and older who are unable to provide meals for themselves or have

difficulty obtaining food because of illness, frailty, or disability. Eligible people receive a weekly

delivery of 7 breakfasts, 5 frozen lunch/dinner entrées, 1 entrée salad, and 1 cold sandwich

supplemented with fresh fruit, juice, milk, bread, and other grocery items. A contribution of $1.80

per day or $54.00 per month is suggested to help offset the cost of the meals.

Are you feeling lonely?

Friends from Meals on Wheels is a friendly visitor program provided for free by The Health

Trust to older adults receiving Meals on Wheels through Santa Clara County’s Senior Nutrition

Program. Older adults receive companionship through regular home visits and phone calls,

in-home exercise programs, free monthly pet food, and referrals to community resources.

Are you a homebound older adult living in San Jose’s Berryessa or Japantown area?

Yu Ai Kai’s Neighborhood Meals on Wheels provides daily (Mon-Fri) hot meals to eligible

seniors in the Japantown and Berryessa Area. A contribution of $3.00 is suggested to help

offset the cost of the meal.

Do you need a hot daily meal delivered to your home?

The Health Trust Meals on Wheels program provides hot meals to eligible adults and older

adults. Hot meals are delivered five days a week along with frozen meals for preparation over

the weekend. Although there is no charge for this service for qualified applicants, Health Trust

requests that those who can afford it contribute to the cost of the meal, which is $12 per day.

Grants and other funding are available to assist low-income older adults.

Need Food?

Second Harvest Food Bank of Santa Clara and San Mateo Counties is dedicated to

ending local hunger. More than half of the food distributed is fresh produce. The Food Bank

provides food through innovative direct-service programs by collaborating with food

distribution and partner sites. Partner agencies include shelters, pantries, soup kitchens,

children's programs, senior meal sites, and residential programs.

Do you make decisions like buying medications over food?

CalFresh is a supplemental food program to help eligible individuals and families get the food

they need to stay healthy. The CalFresh benefit is provided in the form of a debit card (EBT

card) that can be used to purchase groceries and fresh produce at participating grocery stores

and farmers’ markets just like any other debit card. As of October 1, 2017, (ESAP), the annual

recertification interview is now waived for older adults! Recertification applications are only

required every 36 months for eligible participants and can happen over the phone.

Do you want to know how to stretch your CalFresh (Food Stamp) dollars?

Market Match is California’s food incentive program, which matches customers’ federal

nutrition assistance benefits such as CalFresh at local farmers’ markets. Customers who use

their CalFresh EBT card at participating farmers’ markets can get up to $10 for FREE each day

to use towards fruits and vegetables with the Market Match program. Participants can explore

and enjoy delicious “in season” fruits and vegetables!

Department of Aging and Adult Services (DAAS)

DAAS promotes a safe and independent lifestyle for seniors, dependent adults, and the

disabled through the delivery of protective services, quality nutrition, and supportive in-home

services. In addition, DAAS evaluates community needs, develops programs and services,

and advises on matters of policy that concern the welfare of seniors and persons with

disabilities.

Adult Protective Services

Adult Protective Services receives and investigates reports of

abuse or neglect regarding older adults, age 65 or older, or

dependent adults who are unable to protect themselves and

advocate on their own behalf due to mental or physical

disability. APS is available 24 hours a day, 7 days a week, and

is staffed by Master’s level Social Workers who works to

protect and prevent abuse, neglect, or self neglect of elders

and dependent adults.

In Home Supportive Services (IHSS) is a program designed to provide assistance to

disabled older adults, allowing them to remain safely in their own homes. IHSS can include

feeding, bathing, dressing, housekeeping, laundry, shopping, meal preparation and clean up,

respiration, bowel & bladder care, moving in and out of bed, rubbing the skin (to prevent skin

breakdown), accompaniment to medical appointments, paramedical services, and protective

supervision.

Public Administrator/Guardian/Conservator (PAGC)

The Office of the Public Guardian insures the physical and financial safety of persons unable

to do so on their own, and when there are no viable alternatives to a public conservatorship.

The Superior Court determines whether a conservatorship should be established and can

appoint the Public Guardian as a conservator of the person only, estate only (for probate) or

both person and estate.

Have you fallen recently? Say Farewell to Falls!

Farewell to Falls is a FREE, home-based fall prevention program available to seniors, 65

years or older, who live in Santa Clara or San Mateo County. If you have fallen recently, or

are concerned about falling, you may be eligible to participate in a fall prevention program

offered through the Trauma Service and Emergency Department at Stanford University

Medical Center. The program focus includes medication review, home safety assessment

and modification, home exercise program, and other health factors.

Department of Employment & Benefit Services provides low-income individuals and

families with access to health, financial, nutritional, and employment assistance. They

Do you need assistance with food, shelter, counseling, employment, or child care?

United Way Silicon Valley – 211 Santa Clara County 2-1-1 is the dialing code for free,

non-emergency community, health, and disaster information in Santa Clara County. Callers

receive personalized information and referrals from a live, highly-trained Call Specialist who

can answer questions about a variety of nonprofit services and agencies. 2-1-1 is

confidential, available in over 170 languages and available 24 hours a day, 7 days a week.

Are you interested in healthy eating, aging, and living?

The Health Trust provides a variety of direct services aimed at improving the health and

well-being of vulnerable populations in Santa Clara and northern San Benito counties. They

assist older in adults maintaining their health and independence, assist families in obtaining

health insurance, care for children’s teeth, and provide opportunities for people of all ages

to eat well and be active.
