Message from CCF Board of Trustees on March 24, 2007

Dear Sponsors, Donors, friends and Participants

On this 15th Rass-Garba, film and folk dance competition we the Chairman, President and Board of Trustee of Charitable Care Foundation (CCF) are happy to welcome you and your family to witness, enjoy and experience the talents of our own tiny tots, youngsters and seniors. Our sincere efforts are to enhance our culture, to preserve our heritage and to encourage our youngsters along with service to humanity. Increasingly popular CCF yearly fundraising event have proved increased community awareness in helping needy and deserving through out the world.

The proceeds from this fund raising event will go towards helping unfortunate seniors of recent Surat flood. The fund will go to Surat Ashktashram Hospital for rebuilding wing for seniors.

This year more then 560 performers, 40 plus choreographers will be on and behind stage showing their talents and competing for winning position. Forty-seven (47) items are categorized as Rass and folk dance, Garba and film dance in groups of children, juniors and seniors. Ten experienced and qualified judges with our coordinating team will judge performance and select winner and runner-up in each category. Special awards are also for Choreography, Costume and Creativity.

During last 15 years CCF raised and distributed more then thirteen million US$ around world. With support of generous donors, fund raising program sponsor and well wishers, CCF successfully distributed money for medical, surgical and emergency services, eye camps, educational and hospital buildings, medical and surgical equipments, housing for senior citizens, food package distribution during flood and cyclone disasters, thanks giving dinners, tsunami and earthquake victims.

We are sure your involvement will make difference in the lives of others as well as in your life. When we give from our heart, it generates long lasting peace, joy and happiness. We pray almighty to give us strength to help the needy so that there will not be poor, sick and hungry around the world. We wish peace on earth in the spirit of “Service of Humanity is Worship of GOD”.

FIFTEEN YEARS OF SERVICE

In the fourteen years since it was formed

in October, 1991, Charitable Care Foundation

(CCF) is proud to have accomplished its mis-

sion to help needy people become healthy,

productive, and self-reliant. Our efforts and re-

sources are focused on local and international

needs – particularly the Bay Area in California

and India. CCF is now an established charitable

organization in the Bay Area. It is popularly

known among the local communities by its

initials CCF or Care Foundation. It is a nonprofit

California corporation with a thirteen member

Board of Trustees. The trustees and volunteers

provide their services at no cost. The founding

spirit "Service of Humanity is Worship of God"

has inspired many young and adult volunteers

and organizations to support CCF activities.

The Articles of Incorporation and Bylaws

of the Foundation state the purpose as fol-

lows:

“The specific purpose of this organization is

to collect, send, and distribute financial aid as

well as food supplies, new and used clothes and

books, medicine, medical equipment and the

like to the poor, ill, destitute and needy. Such

aid may be sent directly to existing nonprofit

charitable organizations, including but not

limited to orphanages, hospitals, disaster relief

foundations, and educational facilities for the

poor.”

PROJECTSCCF continues to fulfill its strong commitment to better the lives of many disadvantaged fam-

ilies by providing the seeds of basic support—food, shelter, clothes, health care, and education—so that they become self-reliant. These individuals have continued to learn new skills and trades, and regained hopes for a bright future.

Every Indian Child should Read and

Write

Pratham USA founded in 1999 with a mission of literacy movement to provide primary education to un-derprivileged children of urban slums and rural areas of India. Pratham has grown with help of Volunteers and financial institution and is marching to-wards achieving the goal. Yet lot more required to be done.

CCF has joined hands and adopted 120 children. CCF board has agreed to adopt more children in future.

Gujarat Flood Relief Efforts 2006

The monsoon rains, beginning during the last week of July 2006 caused wide spread flooding in the state of Gujarat. The worst affected by the flood was Surat which at one point of time remained submerged in water up to 20 feet high. The flood left thou-sands without food, water and shelter and millions lost their livelihood and suffered heavy losses.

CCF provided fund to flood af-fected area school “Surti Modh Vanik Sarvajanik Trust” for emergency sup-

ply and reconstruction of building CCF Trustee members recently vis-

ited flood affected area in Surat. One of the highly damaged Hospital Structure and equipments was at Ashaktashram Hospital, Rampura Road, Surat. On basis of the report and photographs by visiting trustees, CCF Board decidedto donate Rs. 15 lacs (US$ 35,000) for re-construction of one entire floor of El-

derly Care Home. The proceeds of 15th Rass-Garba competition will go towards this project.

CCF board also decided to assign supervising and project follow-up responsibility to Dr. Ashit Desai well known Neuro Surgeon of Surat.

Navsari (Gujarat) D.N. Sarvajanik Trust

Hospital

CCF joined hands with Gujarat Samaj of Dayton (Ohio) and funded D.N. Sarvajanik Hospital for their ex-pansion project. Check for the amount $ 25,000 was delivered to Dr. Harshad Desai and Dr. Satyan Desai in a function held in Sunnyvale Restaurant.

Sunnyvale Hindu Temple and Sankara Eye

Foundation

CCF staged Gujarati Drama by artist from India and raised fund for Sunnyvale Hindu Temple and Sankara Eye Founda-tion.

$ 5001 to Sunnyvale Hindu Temple will go to their Community Service fund. Sankara Eye Foundation 20.20 motto has appealed CCF committee and has promised to fund substantial amount to their New Hospital Building at Anand (Gujarat). CCF will release fund in parts after construction of building begins.

Dist Surat

CCF and donors Thakorbhai Patel (Ca), Vinod Patel (Ca), Balvantbhai Patel (Ga) completed major school project.

The school in rural area will provide Primary section for 545 students, Sec-ondary section for 777 students, higher secondary for 340 students and Ashram Sala for 150 resident students. Facility will also provide Computer lab, sports ground, water purification plant, audito-rium with audiovisual equipments and school bus.

Hurricane Katrina Disaster Fund

Hurricane Katrina was declared as National Disaster for year 2005. Sig-nificant damage was done to homes and several hundreds lost their lives in New Orleans and surrounding area.

CCF Committee members vis-ited local Red Cross office and had discussion with Chief Development Officer Brian Aird and Director of Development Kathy Miller. Red Cross Officers were impressed when they learn about CCF volunteering with no administration fee and 100% money goes for donation. Red Cross staff request CCF board to join hands for future projects. Vinod Patel Secretary CCF presented check of $ 15,000 and details about projects on hand.

Tsunami Earth Quake Relief Fund

In the year 2004 India, south East Asia and Shri Lanka was devastated by

Tsunami Earth Quake. Hundreds of Thousands lost their lives and millions lost everything.

CCF inspired by Shri Muneeji and Sadhvi Bhagwati during Sant Shree Rameshbhai Katha in Sacramento decided to donate for the deserving in South East part of India.

CCF take lead in two projects Vocational Training center and con-struction of new houses in costal vil-lage Panithittu about 15 kms south of Pondicherry Union Territory.

We are happy to inform that on 13th of December 2005 the Training Center to Village and new homes were officially given to the 80 families by the hands of the Honorable Lt. Gover-nor of Pondicherry and the Honorable Governor of Uttranchal.

CCF along with Sunnyvale Hindu Temple, India American Society of Bay Area and other 10 Bay Area Organiza-

tions organized Fund raising musical evening on 8th January 2005. This event raised more then $45,000. The amount was disbursed successfully to deserving families in South India.

Gujarat Flood Relief Fund

In year 2005 heavy rain and floods affected Central Gujarat, South Gujarat and part of Saurastra. About 200,000 become homeless and infrastructure was seriously disturbed. Immediate help was appealed by Government of India. Vishwa Gujarati Samaj of Ahmedabad Gujarat request CCF for donation. CCF released $11,000 for food, shelter, clothing and emergency supply.

Thanksgiving Dinner for deserving in San Francisco and Santa Clara County

CCF very actively joined hands with various Organizations of Bay Area in providing Thanksgiving din-ners. Executive Committee of CCF gathered at City Team Ministries Fam-ily Service Center at 13th Street in San Jose, at Martha’s kitchen located at 311 Willow Street and Glide Foundation at 330 Ellis Street in San Francisco and distributed food for 3500 families.

Jeff Davis of City Team Ministry, Edith Cruz of Martha Kitchen and Janice Mirikitani of Glide Foundation expressed their thanks to CCF for their kind and generous donation.

Shri Agasi Mata Charitable Trust Project

CCF through target donation and from CCF reserve fund, funded $27,500 to Shri Agasi Mata Charitable Trust. The fund with be used in procuring X-Ray unit, Sonography Unit, Ambulance Van, Laboratory Test Equipments and Mobile clinic in rural areas of Surat District.

Elderly and Battered Woman assistance Program

CCF donated funds for Anupam Mission in Mogri dist Anand Gujarat

for helping elderly and battered wom-an in rural area.

Donation was also made to Ashaktashram Hospital Surat Gujarat for maintaing homes for elderly wom-an care and nursing school.

Vishva Gujarati Samaj High School

Year 2001 earthquake of Katch-Bhuj, Gujarat was devastating. Most affected are schools and children of the area. CCF imme-diately released funds for emergency relief and promised further funds for school.

Swami Sacchidanand Sewa Samaj Trust (SST) funded five Primary Schools in Bhuj area and CCF contributed against construc-tion of each schools. These schools are now opened and in operation.

Ribon cutting ceremony at Vishva Gujarati Samaj High School

CCF further donated $ 60,000 for construction of Secondary School at Har-ipara, Bhuj-Katch. This school in association with Vishwa Gujarati Samaj and Rotary club was inaugurated by Textile Minister of India Shankersingh Waghela on Christmas day 2004. CCF Chairman Hira Patel was the Guest of Honor at the inauguration. Board of Trust-ees Ratilal Patel was also present to witness the inauguration.

The concept of Vatsalyagram or Home of maternal love was explained by didi Maa Sadhvi Ritambhara during her visit in August 2004 and had discussions with CCF Board of Trustee.

Vatsalyagram Donation Project

CCF with other organizations arranged two days fund raising for the project. Didi Maa explained How to secure a future for children who has no future.

Vatsalyagram is a home for orphaned children, but not an orphanage. The home

is comprising of mother, an aunt (masi), a grandmother (nani) and seven children in each vatsalya family. Each vatsalyagram will have 100 homes. One of such model vatsaly-agram is now in operation at Vrindavan UP. Two more are coming up in Omkareshwar MP and Solan HP.

Diabetic Retinopathy Equipment Project

RNC Eye Hospital at Valsad Gujarat is well known in the area for more then half a century. The hospital is run by the trust and is treating eye and carrying out eye treatment and surgery free of charge to each and every patient.

Late Savitaben B.Desai and family trust in San Jose California donated $ 40,000 for purchasing latest laser equipment for Dia-

betic Retinopathy treatment and surgery. This unit will help preventing blindness to diabetic patients. This treatment is available to any one at free of charge.

NY-WTC Disaster Relief Fund

The September 11, 2001 known as 911 was a day of disaster that triggered the USA economy as well as the global economy to a lower level. Thousands lost their lives including the white collar workers, the maintenance staff, the fire fighter, the police officers, and the rescuers. They lost their lives on duty serving and rescuing hundreds of individuals trapped in the burning inferno. CCF donated $25,000 through the local chap-ter of the United Way.

Bidada Hospital, Mandvi, Gujarat

Shree Bidada Sarvodaya Trust is a nonprofit charitable organization at Bidada, Mandvi, Gujarat started in 1974. Two and half million patients are treated during the 30 years period. During their last 30th Medical and surgical camp thousands of villagers from Gujarat, Rajasthan and Maharastra was treated by professionals from USA and mem-bers of AAPI USA.

CCF made a donation from funds raised during 12th Annual Ras Garba folk and film dance competition.

School for Handicapped in Navasari (Gujarat)

“Manava Kalyan Trust” for Handicap, deaf, and mentally retarded is operating since 32 years. The Trust is expanding their activi-ties to help handicapped to earn their living and to become self-sufficient. CCF has agreed to fund for building for mentally retarded and handicapped youngsters. CCF trustee has visited the institution and construction of the additional building.

Educational Project by SMV Sarvajanic Trust, Surat

The SMV Trust has ambitious project of building modern methods and technological School and Library. CCF is funding to build Computer Laboratory and provide Technical instrument.

Gujarat Drought Relief Project

Due to the drought in Gujarat thousand of animals died and hundreds of residents had to fetch water from miles. Gayatri Sewashram Trust (GST) and Sacchidanand Sevasamaj Trust (SST) very actively coor-dinated the efforts of bringing water via tank-ers and diggings underground wells in the

area near Ambaji, Gujarat. CCF funded the project and saved human lives and provided feed for animals.

BP Kelavani Mandal, Bardoli

Assisted by Arvind Patel of Castro Valley, CCF funded $27,200 for the establishment of a modern learning resource center at Gopal Vidyanager Sankul.

Shree Sarvajanic Vidya Mandal, Astan, Bardoli

CCF funded $4,500 for building Ladies Collage at Astan, Bardoli. President and Trustee of Ladies Collage visited USA and attended CCF meeting explaining need of such funds.

Gujarat Cyclone Relief Fund

CCF activated this fund for the dev-astated families in Saurashtra and Gujarat during the flood of July–August. CCF sent im-mediate relief in the form of $7,500 through ‘Shri Saurashtra Leuva Patel Sewa Samaj’ under the supervision of Savjibhai Korat. Ad-ditional funds were raised through a musical evening in August. CCF sent funds to needy families.

Orissa Cyclone Project: During the 1997 CCF funded to the following organizations to help Orissa Cyclone affected families:

• Sanskrit Foundation of Mumbai for adopting small village in super cyclone areas in Orissa, in cooperation with Friends of Tribal Society (FTS) who will further assist local people to improve their life style.

• Utkal Bipanna Sahayata Smiti to im-prove social and economic condition and quality of life of costal communities of district of Orissa

• Latter-Day Saint Charities to provide a 40 feet container full of clothing and milk.

The total contribution from CCF was $85,000

Kargil Project

During the April 2000 CCF contributed through Poorva Sainik Seva Parishad, New Delhi, India for the welfare of families of defence personnel killed or wounded in action at Kargil. The check was presented on April 18 by the trustee, Suresh Patel and Haren Pathak. Hon’ble Minister of State for the defence Production and Supplies India at the Red Fort function to celebrate Azad Hind Fauji Diwas.

Cancer Hospital Building Project

In the December 1999, CCF contributed through the Muni Seva Ashram, Gujarat, district Vadodra, Gujarat for the cancer treat-ment. This hospital will serve surrounding villages of Vadodra district. Miss Anuben, the recipient of the State and Central Humani-tarian awards, was very instrumental in this project. She is known as a Mother Teresa of Gujarat.

Modern Blood and Urine Analyzer Project

Modern Blood and Urine Analyzer was purchased and supplied from CCF fund. It is also maintained by the donations from CCF.

Emergency Ambulance Project

CCF raised $ 41,868 through their Fund raising program Twelfth Annual Ras Garba, folk and film dance competition. As promised $ 15,001 was donated to Shri Sardar Smarak Hospital, Bardoli, Surat District for pur-chasing Ambulance equipped with modern facilities. This Ambulance is now considered as a greatest help to poor and needy for first aid services.

Gujarat Drought Project

In the June 2000, CCF contributed total $7,500 through Sacchidanand Seva Samaj Trust, Dantali, Petlad and through Gayatri Sevashram Trust, Bopal, dist. Ahmedabad. The fund will be utilized to support the drought victims of Gujarat.

Navsari Lions Sarvajanic Charitable Trust Hospital

CCF, in association with Maganbhai and Taraben Patel and Naresh and Rama Patel of

Northridge, CA donated $105,000 for Gen-eral Clinic, Dialysis Center and Maximum Facility Operation Theater.

Maharashtra Earthquake Rehabilitation

“Aple Ghar” (Our Home) Hostel Con-struction. During the 1993 earthquake in Latur district, the Rashtra Seva Dal (RSD) and Charitable Care Foundation (CCF) helped the villagers with food, clothes, and medicine. The Government of Maharashtra donated ten acres of land in Naldurg near Sholapur for the “Aple Ghar” project to build a resi-dential school for 225 children who lost their parents in the earthquake. These orphaned boy and girls study during the day and sleep at night in the school building constructed by RSD and CCF. The ground breaking cer-emony took place in Naldurg on September 28th, 1995. The ground floor structure of the 7,000 square foot hostel was completed in October, 1997 by CCF.

CCF has donated a total of $65,000 for the complete project. The hostel building is now ready and occupied by 250 students. During early January, 1998, several members of CCF Board of Trustees visited at the open-ing ceremony.

Fangani Kelavani Mandal computer center

Chamoli Construction Project

Chamoli is located in North India, mountain area. Due to frequent earthquakes, the houses need to be reconstructed every time. CCF Secretary Praful Kothari visited the site and presented the check for constructing earthquake-resistant homes for the area residents.

Fangani Kelavani Mandal Computer Center

Fangani Kelavani Madal, Fangani is a registered trust which aims to educate the poor and needy children of the rural area in Petlad Taluka, district Kheda. The Principal of the Fangani Kelavani Mandal, Ramjibhai B. Patel, has a vision for their students and started computer classes in this rural area. In his words, ‘in the modern age of computer, e-mail, internet and information technology computer education is the most important and beneficiary for the students’. They submitted a proposal for “Computer Center” project to educate thirty students per hour in each batch for seven hours a day. Charitable Care Foundation Board approved the project in January 1999 and provide funds to the Mandal.

Jivan Jyot Trust, Amoli

A small town has been awarded a Certificate of Merit by State and Federal Government of India as “Ideal Village”. Amoli operates its own Arts and Commerce College, Cooperative Bank, High School, child care unit and Jivan Jyot Hospital. The Hospital was badly damaged during the flood of 1999. The General Ward and Operation Theater where completely damaged. CCF Board Trustee Naranji Patel and Suresh Patel personally vis-ited the site. Based on their detailed report, the CCF board approved fund for recon-structing the damaged buildings. First stage of funding is already disbursed.

In addition to continued support for blindness prevention and check dams in Gujarat, CCF has started new projects for assisting the handicapped and the tribal com-munities.

• A target donation was sent to Charitable Public Dispensary in the village of Puni in Surat district. This clinic treats about 12,000 patients a year from the sur-rounding tribal areas for paralysis.

• CCF donated $1,000 to Gujarat Navsar-jan Trust in Vadodara for educating young handicapped children suffering from Polio and Leprosy.

Becharbhai Patel of Umrakh (presently residing in Foster City, California) made a donation to construct an engineering college for diploma in civil, computer, and electronic engineering. CCF also donated $3,000 for this educational program.

• Akhil Bhartiya Vanvasi Kalyan Ashram, Ukai, Surat: CCF donated $1,500 for educational services to this rural adivasi area of Gujarat.

• Shri Sarvodaya Kelavni Mandel, Karch-alya, Mahuva, Dist. Surat: CCF donated $3,500 for the construction of the Library and Science Laboratory building at this rural area education facility.
