Introduction

• Most common gynecological cancer

• Typically: post menopausal women

• 20-25% before menopause, 5% before 40 years

• Young women: good prognosis• Early stage

• High differentiation at diagnosis

• Recommended treatment: total abdominal hysterectomy withstaging depending on risk factors

Patients selection criteria

Page 18: Fertility preserving treatment Endometrial Carcinomafile.trsgo.org/pdf/2018/kongre2018/98.pdf · endometrium and areas of the greatest histological or cytological severity may escape

Selection criteria

• A well-differentiated carcinoma

• Progesterone receptor status?

• The tumor does not deeply invade the myometrium, demonstrated by MRI, TVUS, HSC

• Absence of suspicious pelvic or preaortic nodes

• Obesity remains a significant risk factor of endometrial transformation even after primary treatment (approximately 50% of ≥25 BMI patients recur)

Page 19: Fertility preserving treatment Endometrial Carcinomafile.trsgo.org/pdf/2018/kongre2018/98.pdf · endometrium and areas of the greatest histological or cytological severity may escape

Selection criteria (2)

• Absence of synchronous ovarian tumor : Ca 125, abdomino-pelvic US, pelvic MRI (consider diagnostic laparoscopy + peritoneal cytology). Risk of occult synchronous ovarian tumour in stage I: 1-5%

• No contraindications for medical treatment

• The patient understands and accepts that this is not standard treatment

• The patient should show her desire to complete the follow-up protocol

Positive diagnosis: imaging and surgery

Page 21: Fertility preserving treatment Endometrial Carcinomafile.trsgo.org/pdf/2018/kongre2018/98.pdf · endometrium and areas of the greatest histological or cytological severity may escape

Diagnosis and pre-treatment evaluation: imaging

• Transvaginal sonography when carried out by expert hands shows a comparable accuracy to that of MRI in depicting myometrial infiltration of endometrial carcinoma

• Combination of both transvaginal sonography and MRI could be more accurate in detecting myometrial invasion.

Page 22: Fertility preserving treatment Endometrial Carcinomafile.trsgo.org/pdf/2018/kongre2018/98.pdf · endometrium and areas of the greatest histological or cytological severity may escape

Diagnosis and pre-treatment evaluation: imaging (continued)

• Lymph node greater than 1cm in short axis or with central necrosis is considered to be suspicious. When central necrosis is present, MRI imaging can correctly predict metastasis almost 100% of the time

• MRI : sensitivity of 80% and specificity of 100% in diagnosing myometrial or cervical invasion.

Page 23: Fertility preserving treatment Endometrial Carcinomafile.trsgo.org/pdf/2018/kongre2018/98.pdf · endometrium and areas of the greatest histological or cytological severity may escape

Diagnosis and pre-treatment evaluation: surgery

• Endometrial curettings and biopsies are frequently a source of diagnostic difficulty, due to both the artifacts resulting from the procedure and the changes associated with shedding, metaplasia, or hormone therapy

• A diagnosis of well-differentiated EA is established easily when there is myometrial invasion, but the myometrial invasion is not usually apparent in curettings.

• Interinstitutional discrepancies of surgical pathological diagnoses have been reported

oncology. I. Cancer in endometrial curettings and biopsies, Int. J. Gynecol. Pathol. 17 (1998)

Page 24: Fertility preserving treatment Endometrial Carcinomafile.trsgo.org/pdf/2018/kongre2018/98.pdf · endometrium and areas of the greatest histological or cytological severity may escape

Diagnosis and pre-treatment evaluation: surgery (continued)

• Trimble et al.: prevalence of endometrial carcinoma in patients diagnosed with biopsy proven atypical endometrial hyperplasia is as high as 42.6% → diagnosis of endometrial carcinoma by biopsy alone difficult

• Recent study by Leitao et al: preoperative FIGO grade 1 diagnosis correlates with a final grade diagnosis in 85% of the cases.

• Dilation and curettage > office endometrial sampling

• Dilatation and curettage still incorrectly graded approximately 25% of the patients, with higher grade tumor being missed in about 10%

Diagnosis and pre-treatment evaluation: surgery (continued)

• Biopsy or curettage may not sample the entire endometrium and areas of the greatest histological or cytological severity may escape histological identification

• → need for hysteroscopy with guided biopsies

l

Page 26: Fertility preserving treatment Endometrial Carcinomafile.trsgo.org/pdf/2018/kongre2018/98.pdf · endometrium and areas of the greatest histological or cytological severity may escape

Accurate hysteroscopic evaluation

• Best procedure for excluding cervical involvement and thus second-stage disease (NPV 100%, low Sp). Complementary to MRI.

• Estimate the extent of the cancerous spread within the uterine cavity (>50% prognostic factor)

• Indirectly predictive of tumoral size (independent prognostic factor of lymph node spread and disease-free survival)

Resectoscopic treatment/evaluation (continued)

• Mazzon 2010: three-step technique in which each step is characterized by a pathologic analysis:

• removal of the tumor (step 1)

• removal of the endometrium adjacent to the tumor (step 2)

• removal of the myometrium underlying the tumor (step 3)

• 6 patients. No relapse with 50.5 months mean follow-up

Surgery adverse events

• Hysteroscopic surgery, performed by a well-trained hysteroscopic surgeon, is a safe procedure with an overall complication rate of 3%, with 1% of uterine perforation

• Reports suggest dissemination of malignant cells into the peritoneal cavity during hysteroscopy, but

• prognostic significance unclear (none if disease confined to the uterus (?)), or

• functionally viable cells?

Endometrial Cancer in Fertile Women

• 10 patients, having AEH or EC had total superficial endometrial resection:

-resection at the tumor or thickness site with underlying myometrium

-sampling and superficial resection of the rest of the endometrial cavity

• Allowing evaluation of the whole endometrium and staging of tumor, and

reducing the risk of missing high grade lesions

• Guided with suprapubic ultrasound

• With hysteroscopic biopsies every 3 months during treatment with

progesterone (160mg of ) (BETTOCCHI® without dilation) , until pregnancy

Optimal duration of treatment

• Unclear and no consensus. Individual response time is variable, depending on underlying physiology

• As early as 10 weeks, minimum 3 months, median 9 months

• Anovulatory and obese patients were found to need longer treatments and to require close follow-up to ensure that the endometrium was cycling either spontaneously or from exogenous hormone therapy (oral contraceptives or periodic progestins)

• Individually tailored

LNG-IUD

• However, a small series by Dhar et al. raised concern about the efficacy of single agent progestin releasing IUD

• Much older and co-morbid population

• Very few prospective studies evaluating the sole use of intrauterine progesterone → consider addition of an oral progestin

Other treatment options

• GNRH agonists

• Aromatase inhibitors (reduction of estrogen concentrations. Small studies with letrozole or anastrozole in advanced or recurrent)

• Antiestrogens

• Combined

Wang 2002

• Tamoxifen and GnRH a: extensively studied for treatment of advanced/recurrent endometrial carcinoma

• Theoretically, tamoxifen can inhibit the binding of estradiol to ER, increase PgR in endometrial carcinoma with low initial PgR, and make such tumors more responsive to progestin therapy

• Specific high-affinity receptors for Gn- RHa have been demonstrated in normal and carcinomatous endometrium, providing a rationale for the further exploration of a direct antitumor effect.

• Combination of megestrol acetate +tamoxifen + GnRHa (leuprolide)

• 9 patients. 8 (88.9%) complete remission. 4 recurrences.

• 3 full-term pregnancies. 2 ectopic

• Median follow-up 69 monthsWang CB et al., Cancer 2002;94:2192–8. © 2002 American Cancer Society

Adverse events

• Well tolerated

• Adverse events less than 5%

• Thromboembolic events have been reported as a complication in a number of studies, in addition to weight gain and hypertension

Reproductive outcome

• In a recent literature review that included >400 women whowere managed conservatively for endometrial adenocarcinomaor atypical hyperplasia to preserve fertility, this strategyresulted in pregnancy in one-third of the candidates after amean follow-up of >40 months

• Young women who develop endometrial carcinoma are typically also affected by other factors that affect fertility, such as obesity, polycystic ovarian syndrome, and chronic anovulation.

Reproductive outcome

• No data on optimal time from completion of progesterone therapy to attempting pregnancy

• Lack information on the safety of intrinsic hormonal changes that take place during pregnancy and on the long-term effects of the medications used during ART

• Assisted reproductive techniques do not appear to worsen the prognosis in women with a history of endometrial cancer, increase the chances of successful conception and decrease the interval to conception

Follow-up

L. Chiva et al. / Gynecologic Oncology 111 (2008) S101–S104

• First biopsy after 12 weeks of hormonal treatment (average response time)

• If positive → another 12 weeks treatment

• Second biopsy after 24 weeks of treatment (average length of treatment reported in the literature)

• If positive → standard surgical approach

• If negative → encourage conception

• Advisable to do intermittent biopsies in the preovulatory part of the cycle every 3 or 4 months

Page 63: Fertility preserving treatment Endometrial Carcinomafile.trsgo.org/pdf/2018/kongre2018/98.pdf · endometrium and areas of the greatest histological or cytological severity may escape

Maintenance therapy: suggestions

• If a pregnancy is not desired after completing successful primary therapy, maintenance therapy with ongoing progestagens is necessary until childbearing is wanted.

• Progestin treatment should be followed immediately by downregulatory therapy with GnRHa in order to prevent reinduction of the menstrual cycles and unnecessary prolonged exposure to estrogens

Rationale for post-fertility hysterectomy

• Successful pregnancy does not automatically exempt a coexisting endometrial carcinoma: case report by Mistsuhita et al. of carcinoma remaining after term pregnancy post MPA treatment

• Exploration of the uterine cavity as well as the abdominal/pelvic structures is recommended if cesarean section is to be performed

• Most authors agree that consolidation hysterectomy should be advisable after completion of family planning.

Photodynamic therapy

• 16 patients, mean age 30.7 yo, lesion confined to endometrium

• Surface photoillumination with red laser light at a wavelength of 630 nm was applied to the uterine endometrial cavity and endocervical canal of patients 48 hours after an intravenous injection of 2 mg/kg of photosensitizer

• 75% remission; 36% recurrence

• Second course: final response rate 68%

• 7 successful pregnancies

• No tumor-related deaths or PDT-related severe adverse effects were noted.

Take home messages

• In conclusion, with intensive vigilance to detect disease persistence or recurrence, hormonal management/hysteroscopicresection appears to be a safe and effective option in women diagnosed with endometrial neoplasia who are motivated to retain their reproductive potential.

• There is an increased likelihood of response with progestin treatment of complex atypical hyperplasia compared with treatment of endometrial carcinoma

• Reported pregnancy rates and number of live births suggest that treatment with progestin may allow for reasonable fecundity

Page 70: Fertility preserving treatment Endometrial Carcinomafile.trsgo.org/pdf/2018/kongre2018/98.pdf · endometrium and areas of the greatest histological or cytological severity may escape

Take home messages

• Careful oncologic, psychotherapeutic, genetic and reproductive counseling is advised before offering a non-standard treatment strategy to young endometrial cancer patients.

• Documentation of informed consent should reflect these discussions and patient understanding

• Conservative management can also be useful for inoperable patients

• Lack of prospective, randomized controlled and appropriately powered trials
