EXAMINATION

A4M Fellowship requirements include:

■ Membership in the American Academy of Anti-Aging Medicine ■ Photocopy of your degree (M.D., M.B.B.S., BDS) ■ Photocopy of your active medical license ■ CV/resume demonstrating a minimum of five years of clinical practice experience (residency training applicable)

■ Signed affidavit of no disciplinary actions ■ Submission of 12 case studies (sample cases provided) ■ Completion of modules 1 to 6 and pass grade at the exam
