    On September 12, 1991 a t 0448 hours in the borough o fBrooklyn, a telephone alarm of f i re was repor ted to th ecen t r a l dispatching of f ice . This telephone alarm wasrece ived from the 911 Pol ice Dispatcher .Uni t s of the F. D. N. Y. were dispatched to a bui ld ingf i r e repor ted to be located on Hegeman Avenue locatedbetween Atkins Avenue and Montauk Avenue. The address o fth e bui lding in which the f i re occurred i s 495 AtkinsAvenue. I t i s a four s tory vacant res iden t ia l bui ld ingapproximately 60x100 fee t in s ize . Although t echnica l lyvacant , the bui lding housed several squat t e rs , and was aknown drug loca t ion .The f i re or ig ina ted in a fourth ( top) f loor apartment,Dl, and was incendiary in or ig in . The f i re rapidly tookpossess ion o f the apartment and extended to the areabetween the ce i l ing and underside of the roof known as thecocklof t .During the extinguishment operat ions of the f i reFi re Fighte r 6 th Grade Kevin C. Kane, operat ing as par t o fthe Forcible Entry Team of the f i r s t ar r iv ing l addercompany was f a t a l ly in jured . While performing search andexaminat ion of the adjacen t apartment (D2) he becamet rapped in a .bedroom when the cei l ing col lapsed and theroom ign i t ed .He was seen in the fourth f loor bedroom by FireDepartment members in the s t r ee t and removed from thewindow by the Tower Ladder bucket of Ladder 107. He wast ransported by EMS and Pol ice Helicopter to New YorkHospi ta l Burn Center where he died the following day.

    FINDINGSOn the morning of Sept. 12, 1991 while performing thedut ies of searching for possible vic t ims and examining for

    f i r e extension a t a major f i re on the top f loor of a fours tory vacant r es iden t i a l bui ld ing , Fire f igh te r 6th gradeKevin Kane rece ived second and th i rd degree burns over 82%of h is body. These in jur ies resu l t ed h is deathapproximately 22 hours l a t e r . Fire f igh te r Kane, under thed i r e c t supervis ion of Lieutenant Charles Daly, had madeh is way in to the bedroom of apartment D2. Whileperforming h is dut ies the ent i re ce i l ing suddenly andwithout warning dropped, cut t ing of f Kane's path ofescape. Unable to leave the room by the entrance he wentto the window as the room became engulfed in f i re . Het r i ed unsuccessfu l ly once more to f ind the doorway throughwhich he had entered the room and returned to the window,now complete ly engulfed in flames.Members in the s t r e e t , a le r ted to his pl igh timplemented rescue procedures and removed him, v ia towerladder ( ae r i a l pla t form). The rescue operat ion tookapproximately one and one ha l f minutes, during which t imehe was almost constant ly exposed to d i rec t flame contact .He was removed to Burn Center, New York Hospital Cornel lMedical Center where he expired on September 13, 1991.Fi re f igh te r Kane"s pro tec t ive clothing was notdesigned to pro tec t him from th i s environment. This i scovered in de t a i l in the sect ion "PROTECTIVE CLOTHING".While there was one hand-held hose l ine in operationa t the t ime of the ce i l ing col lapse , it was heavilyengaged in t ry ing to extinguish the f i re in apartment D1,and could not be read i ly moved to help the vict im. Thesecond and th i rd hand l ines were in the process of being"s t re tched" in to pos i t ion , but had not yet been put in tofu l l operat ion, o r were too fa r away from the t rappedf i r e f igh t e r to be of any value . By the time they wereable to be moved in to pos i t ion , Kane had been fa ta l lyburned and was out of the bui ld ing .

    CAUSE OF DEATH:Kevin Kane died as r esu l t of thermal burns and smokeinh9-la t ion. These in jur ies were caused by di rec t flameimpingement fo r a t l e a s t one and one hal f minutes. Thiscaused 2nd and 3rd degree burns to 82% of body surface .This also caused smoke inhalat ion with resp i ra tory burns.

    CAUSES OF MISHAP1 . DIRECT CAUSE:

    Kevin Kane was opera t ing in a room with heavy f i r e inth e cocklof t , i n which a ce i l ing col lapse occurred,the reby exposing him to d i r e c t flame contac t .2 . INDIRECT CAUSES:A: Use of accelerant in the ign i t ion of the f i r e .B: Fa i lu re to repor t f i r e discovery in early s tages .C: Advanced f i r e condi t ions on a r r iva l of i n i t i a lresponse assignment .D: The fa l len ce i l ing blocked Kane's egress and preventedre scue at tempt by Lieutenant Daly. Fire rapidly tookpossess ion of th e room and without hose l ine read i lyava i l ab le Kane was unable to ex i t the room during the SAFEEGRESS TIME, Safe egress t ime i s defined as the i n t e rva lbetween the t ime hazardous condi t ions preva i l in the roomand th e t ime a victim would suf fe r 2nd degree burns to h isbody. C: The i nab i l i t y to ge t water on the f i re in Apt.D2.Engine 236 had a l ine s t re tched to the top f loor andhad water a t about the t ime of the mishap, bu t was unableto move the l ine in to pos i t ion to rescue Kane.The inves t iga t ive team explored t ha t and found t ha tin Apr i l 1987 Deputy Chief Vincent Dunn, Div.3 hadconducted t e s t s regarding 5 man vs. 4 man engine manning.His r e s u l t s were the subjec t of a repor t dated April 10,1987 s t a t ing in par t tha t when the hose s t re tch team wasreduced by 20 percent (one f i r e f igh te r ) tha t hose s t re tcht ime was increased by 50 to 75 percent .     THE FIRE FIGHT:The f i re f igh t i s described sequent ia l ly but there aremany functions going on simultaneously. All t imes otherthan those taken from the communications tapes are

    approximate.The f i r s t alarm fo r a f i re a t 495 Atkins Ave. wasreceived from a 911 operator a t 0448:35. The informationi s fo r a f i re opposite 1004 Hegeman Ave. in a vacantbuild ing. At 0451:12 box 1918 i s t ransmit ted over voicealarm. (The Computer Assis ted Dispatch System was down a tthe t ime of the alarm.) Engine 290, 2 3 6 ~ Ladder 107, 103& Battal ion 39 are directed to respond.At 0453 hours Ladder 110, Which had been acting asLadder 107 responds to a verbal which had been reported byMr. Mark Lucas, a secur i ty guard in the area, and anindividual known to some of the members of Ladder 107.Realizing the probabi l i ty of a working f i r e , Ladder 110volunteers to respond. At 0454 Engine 332 i s cal led onthe radio and directed to respond to Hegeman Ave. andBradford s t . , a loca t ion some 15 blocks from the f i r e . At0455 hours Engine 290 and Ladder 110 give 10-75 'ssimultaneously. I t i s apparent from the background noiset ha t both apparatus are still responding in to the scenebut ar r iva l i s imminent. Engine 332 asks fo r a 10-7 fo rthe Hegeman Ave. and Bradford s t . locat ion. Engine 283and Ladder 175 are dispatched from Quarters on the 10-75.Bat t . 39 had arr ived and orders and addi t iona l Engine andt ruck.At 0456 Engine 332 i s to ld to go to Hegeman Ave. and

    Berriman s t . Ladder 107 ar r ives and Rescue 2 i sdispatched v ia radio . Engine 290 takes a hydrantdiagonally across from the f i re bui ld ing , on Atkins Ave.and the members s t a r t s t re tch ing . The off ice r , LtVarr ia le goes to the f ron t entrance door and waits whileLadder 110 Forcible Entry team cuts the lock and chainwith a Partner saw. He then ente rs building and proceedsto the courtyard door and looks up in to the shaf t . Hesees f i re out about four windows and he re turns to thef ront door. Concerned with the volume of f i re heconsiders ordering 2 1/2 inch hose but f inds tha t thecompany has 1 3/4 inch l ine already in to the f ront door.He sees members of Engine Co. 236 coming down the blockand he direc ts tha t they are to s t re tch a second l ine andnot help h is company with the f i r s t l i ne . Ladder 107arr ives and begins to se t up fo r tower ladder operat ionplaCing the apparatus on Hegeman Ave. facing west. Bat t39 gives a quick preliminary repor t to Brooklyn radio.

    L t. Varr ia le climbs to the top f loor and ar r ives a t0457. His un i t i s following him up the s ta i r s and i shaving d i f f i c u l ty maneuvering through the debris in thef ron t entrance ha l l and up the re turn type of stairway. Herad ios to the 39 Bat t . t ha t he has f i r e in two apartments .Ladder 103 a r r ives and s t a r t s to ra i s e ae r ia l ladder tothe roo f . The roof man of ladder 110, Fire Fighter Tracyat tempts to use Ladder 107 's bucket to access the roof bu tf inds t ha t the bucket i s already on i t s way to the topf loo r f i r e escape . He and 110 's outs ide vent Fi re Fighterwat t go to the a l ley on Exposure 4 s ide and decide to usethe f i r e escape there t o ge t to the roof . Lt. Daly, L110and the Forc ib le Entry team, Fire Fighter Kane and FireFighter Monaghan arr ive a t the top f loor . Kane uses theext inguisher on the apartment D-1 doorway from which heavyf i r e i s blowing out of in to the publ ic ha l l . Lt. Dalysearches apar tment D-4 and The Forceable Entry F.F.Monaghan searches apartment D-6. B.C. Flynn looks up fromthe f i r s t f loor cour t yard door to check conditions andthen proceeds to the landing between f loors 3 and 4.

    By 0458 Engine 290 i s hooked up and the chauffeur , FFKelly t e l l s the of f i ce r tha t he i s ready to give himwater . Engine 236 s t a r t s s t re tch ing nine lengths of 1 3/4inch l ine from Engine 290' s pumper. Ladder 103' s aer i a lreaches the roof and FF. 6th grade Joyce s ta r t s alone upthe l adder to the roof with a par tner saw. Engine 332ar r ives on the scene and pos i t ions apparatus a t thehydrant on Hegeman Ave. diagonally opposi te from the f i r ebui ld ing . The chauffeur of Ladder 110 with the OutsideVent o f Ladder 103 ra i s e s a portable ladder to the AtkinsAve. f i re escape . The Ladder 110 roof team, F. F. Tracyand F . F . Wa t t take a portable ladder from Ladder 11 0 ' sapparatus and car ry it to the f i re escape on Exposure 4s ide of the bui lding. Lt . Olsen, Ladder 107 and theForc ib le Entry team arr ive on the top f loor of the f i r ebui ld ing . Ladder 103 Outside Vent F.F .Libre t t i , s ta r t s upthe f ron t f i r e escape to vent . Ladder 107 Roof ManF.F.     At 0500 Engine 236 i s s t re tch ing a l ine up s t a i r s andhaving d i f f i c u l ty because the l ine keeps get t ing snagged.It i s not a "well" s t re tch , there i s a l ine already in thes ta i rway and there a re people going up and corning down thestai rway, and they have to work around them. Lt. Varr ia lerad ios Engine 290' s chauffeur to s t a r t water . Severa lseconds l a t e r F.F. Kelly answers "Here comes your water" .Varr ia le radios the 39 Bat t . tha t the re i s f i re in twoapar tments . L t. Henry Gmelin, Eng. 332 radios the 39Bat t . fo r orders . F.F .Joyce, L103 roof s t a r t s saw. He i s in a posi t ion di rec t ly overthe bedroom in the D-2 apartment. Ladder 107 i s workingt he i r way to the top f loor of Exposure 3, which i s 1004Hegeman Ave. Engine 236 i s still s t re tch ing l ine to topf loo r . Engine 290 d i r ec t s t he i r hose stream in to thef ron t door of apar tment D1. Lt. Varr ia le sees Ladder 110on th e f i r e f loor . _L 110 goes in to apartment D3.     At 0502 Engine 290 di rec t s l ine in to the room to ther i gh t of the door . They move in to t h i s kitchen and darkendown t h i s room. They hear a saw running on the roof asF.F. Joyce s t a r t s to cut the roof . Lt . Olsen and the FEteam of Ladder 107 reaches the top f loor of Exposure 3 andbegins examination fo r extens ion in the f ront (HegemanAve. s ide) of apartments .

    At 0503 - Engine 290 had darkened down the f i re in theki tchen and s t a r t to move in to the l e f t room or the l iv ingroom. Lt. Varr ia le sees members of Ladder 11 0 go in toapartment D-2. F.F. Joyce continues to cu t roof . B.C.Flynn radios Engine 283 to s t r e t ch a l ine to the top f looro f Exposure 3. Lt. Joyce takes h is company towards Engine332 's pumper. (Lt. Joyce, Engine 283 i s a bro ther of FFJoyce, Ladder 103 roof man.) Ladder 110 Roof and OutsideVent ar r ive on the roof a t about 0504. Their ascent upthe Exposure 4 f i re escape was hampered by debris andropes and wires strung a t var ious points blocking t he i rrou te . F. F. Tracy was carrying the Par tner .Saw, whichfur ther slowed the i r progress . Tracy t e l l s Watt, theOutside Vent to check the bulkhead and dumbwaiter and thenproceeds to the area of the f i r e . He sees the cutsal ready made by F.F. Joyce and observes f i re through thecuts . He radios Lt. Daly t ha t there i s f i r e in thecocklof t . The hole i s not yet opened and the smokecondi t ion on the roof i s descr ibed as very heavy. I t i sso heavy in fac t , tha t Tracy cannot s t a r t the saw.Captain Quartuccio Ladder 103, hears t h e rad io repor t"Roof opened, f i re in cocklof t" . Engine 290 hears thesame message. Ladder 175 i s ordered by Bat t . 39 to sendroof team (Chauffeur, Roof and Outside Vent) to the roofand FE team, (Officer , Can man and Forcible Entry) torepor t to the f i re f loor . Ladder 107 ascends from the topf loor of Exposure 3 to the roof . Lt. Olsen sees Joycecut t ing the roof . The cu t i s f inished but no t yet pulledand there i s f i re vis ible in the cuts , ( the hole i s about3x6 foot) .

    Lt. Gordon, Ladder 175, arr ives a t the top f loor a tabout the same time as Lt. Gmelin Engine 332 orders the i rl ine charged. Lt. Gmelin then enters window to the D-4apartment from f i re escape. Outside vent F.F. Libre t t i ,L 103 enters apartment with him. The f ron t apartment i sheavi ly charged with smoke. He proceeds toward the publ ichallway. Ladder 175 roof team s t a r t s up Ladder 103 1 sa e r i a l . Ladder 110 Forcible Entry team i s now in l iv ingroom of apartment D-2, F.F. Kane proceeds to the bedroom.The of f ice r and members of Engine 283 are walking East onHegeman Ave. towards Engine 332' s apparatus . There i sheavy smoke but no f i r e showing from top f loor windowsadjoining Exposure 3. Engine 236 ca l l s fo r water in t he i rl ine and the c a l l i s acknowledged. L-103 Roof fee l s theroof move, (bounce or shake). Concerned t ha t th e roof maybe about to col lapse , he s teps over parape t wall toExposure 3 roof .

    At approximately 0506 Lt. Daly hears a loud noisedescribed as a whoosh-crash. He immediately hears Kanec a l l fo r help. He t r i e s to en t e r the bedroom where Kanei s bu t h is way i s blocked by what proves to be the en t i rece i l ing , which had col lapsed in a l ean- to fashion.. In thes t r e e t the Engine 332 Chauffeur FF. Marot ta and L t. JoyceEngine 283 and the three f i r e f igh te r s from Engine 283 a l lhear Kane shout for help . They look up to see him in thewindow, with f i re over h is head. The Eng. 332 Chauffeurgives a message on the radio -"URGENT Fireman Trapped onHegeman Ave s ide ." Lt. Joyce observes no one in thebucket o f Ladder 1 07 and runs to the tu rn tab le of theappara tus . Engine Co. 332 of f i ce r Lieut . Gmelin, i spass ing through the D-4 apar tment when he hears the URGENT(MAY DAY) message. He enters the publ ic ha l l and sees twol ine s in the ha l l . These l ines a re going in to the HegemanAve. s ide apartments. Only one i s .charged. He sees anext inguishe r in the publ ic ha l l , which he takes with him,as he searches the rea r apartments on the top f loor .Rescue Co. 2 has ju s t ar r ived a t the scene. I t s membersare approaching the f i re bui ld ing , walking ea s t on HegemanAve. when the yel l ing a t t r a c t s Capt . Downey's a t ten t ion toFF . Kane in the window. He immediately orders f i r s t a idand burn equipment from the Rescue 's appara tus . Kanel eaves the window. Lt. Daly, still at tempt ing to reachhim from the doorway, sees him run through the room whichi s now fu l ly involved. He descr ibes him as"High-stepping"through the room. He ca l l s to Kane and pushes th e ce i l ingbut Kane runs back to the window. L t. Daly gives"Mayday". Kane reappears a t the window and i s seen by themembers on the s t r ee t as wel l as the members of Ladder 175on the aer i a l o f Ladder 103. Engine 283 members s t re tchl ine from 332's apparatus, and water ca l led for , but thewrong l i ne i s charged. This i s quickly correc ted .

    Fire Fighter Stanley Ladder 107 OV and Fi re FighterJordan Ladder 107 Chauffeur quickly ex i t the apartment tothe f i re escape, Stanley r e - en te r s the bucket as Lt. Joyces t a r t s the bucket moving toward the window. Bat t . 39t ransmits a second alarm a t the t ime of the bucketmovement, a t 0506:35.Lt. Daly succeeds in pushing the ce i l ing ou t of theway but i s unable to get to Kane due to heavy f i re . Hegoes to the f ront window of the l iv ing room o f apartmentD- 2 and sees the bucket moving toward the window whereF.F. Kane i s . \At t h i s t ime Engine Co. 236 was enter ing apar tment D-2with t he i r charged l i ne .

    Lt Daly then at tempts to loca te an Engine with acharged l i ne . He goes to the f ron t of the apartment andf inds Engine 236 and di rec t s them towards the rear bedroom. As they s t r e t ch in to the l iv ing room, the hosegets caught on something. They are jus t short of thesmal l hallway leading in to the bedroom. They direc t t he i rstream down the shor t hallway and in to the f i re room, butit has little e f f ec t on the f i re u n t i l they get more hose.At approximately t h i s t ime Lieut . Gordon of Ladder Co. 175crawls p a s t the nozzle team of Engine Co. 236 and s ta r t sto en te r the smal l hallway leading to the rear bed room.

    Meanwhile on the roof Fire f igh te r Sonnenberg, Ladder107, l eans over the parapet and t r i e s to reach Kane with a6 foo t hook. Stanley c a l l s Lt. Joyce v ia the intercomfrom the bucket and t e l l s him to leave the controls alone,t ha t he w i l l operate the cont rols from the bucket . F. F.Jordan i s running down the f i re escape and t e l l s Joyceverbal ly to l e t Stanley have the cont ro l s . Jordan mountsthe tu rn tab le and takes the pos i t ion from Joyce. Stanleypos i t i ons the moving bucket so tha t he i s approaching thewindow t ha t Kane i s in from below. This i s done becauseof the heavy f i r e emanating from the window. As heapproaches the window Kane dives in to the bucket 1 s opengate on the cont ro l s ide of the bucket . He i s par t i a l lyhanging out of the bucket, and Stanley grabs Kane, thenpu l l s him in to the bucket .

    Engine Co. 283 members who had s t re tched a l ine froms apparatus s t a r t s to water asbucket descends to the ground. They continue to use thel ine on Kane in te rmi t ten t ly to cool him down un t i l theymove him to the ambulance.As Lieut . Gordon L-175 enters the rear bed room, h is

    helmet i s knocked from h is head by the E. 236 1 s hosestream. He f ee l s the rad ian t hea t and sees the glow off i r e above h is head.
