W• On October 07, 2011, a 23-year old 3rd shift section

repairman with 3 years experience and six (6) weeks experience at this mine, was fatally injured while attempting to repair a broken conveyor chain on a mobile bridge.

• The victim was lying on the mine floor underneath the mobile bridge making repairs to the bridges conveyor chain.

• The mobile bridge had not been securely blocked against motion . The machine had been supported on the 1st shift by a flat rock , the rock was stood on its end under the bridge conveyor.

• The rock dislodged allowing the bridge to collapse onto the victim resulting in fatal injuries.
