LETTER FROM THE MEDICAL DIRECTOR Few people live a full lifespan without seeing an oph- thalmologist. According to the American Academy of Ophthalmology (AAO), in the US alone, six out of every ten adults need some type of corrective eye- wear. Thankfully for most of the population, a good pair of glasses or contact lenses, or a Laser vision procedure corrects the shape of the cornea or loss of flexibility in the lens that causes the loss of vision. For many people, the threats to visual health are more severe. Our comprehensive diagnostic and treatment protocols to assist such patients are all evidence based with significantly higher success rates than the industry standards. For instance, our SupraLASIK re-treatment rates are as low as 0.05% as compared to industry norms of up to 20% for reg- ular LASIK procedures - this really sets us apart and has led to word of mouth referrals coming for treat- ment from all corners of the world. Undoubtedly, the success of a medical establishment is measured usually by the number of satisfied pa- tients who praise the treatment and services offered by the organization. For us, it is the feedback and sup- port of thousands of happy patients we have treated in the last 30 years in the Middle East that keeps us on our toes to constantly innovate and remain at the forefront of what we do. The peripheral part of the retina is divided into four quadrants where is the severi-ty of the disease depends on how many quadrants are involved and at what level.

At early stage of the disease the blood vessels show a slight change of calliper and microscopic haemorrhages into the retina can be seen.

ADVANCED DIABETIC RETINOPATHY Untreated eyes can end up with Retinal detachment and complete vision loss which is in some cases can be permanent.Earlier stages of Diabetic Retinopathy can be treated with Laser photocoagulation with or without intra ocular medication like steroids or antigrowth fibrovascual agents such as Lucentis.

Surgical treatment can be a sight saving where conventional methods have failed to stop the progression of disease.

Most Retinal eye surgery nowadays can be done under local anaesthesia and no hospitalization is required.

Having a systematic method of screening, proper referral and follow up of Diabetic patients by the specialists will hugely re-duce the incidence of vision loss caused by diabetes, maintain the quality of life and lower the financial impact on individ-uals and governments.

EARLY DIABETIC EYE DISEASEModerate stage can be more severe with almost all quadrants involved and in-creased tortuosity of the blood vessels. At advance stage abnormal blood vessels can start to develop at the optic disc or elsewhere, if not treated can start to bleed into the eye cavity causing significant loss of vision.

Diabetes Mellitus is one of the most chal-lenging medical conditions affecting many people worldwide especially in our region. According to the recent report the Gulf Re-gion has almost the highest incidence of Diabetes comparing to other regions which is almost affecting 25% of the adult popu-lation.

One of the major side effects with Diabetes is the ocular involvement in the form of Dia-betic Retinopathy.

Less problematic with Diabetes is the early development of Cataract comparing to nor-mal individuals which is usually successful-ly treated with no adverse events.

NORMAL EYE FUNDUS

Regular eye test and screening by a special-ized eye doctor is very important to detect early stages of the disease which will pre-vent permanent vision loss if treated early.Poorly controlled Diabetic patients are at higher risk of developing Diabetic Retinop-athy and can be slower to respond to treat-ment.

Diabetic Retinopathy occurs when the Reti-na blood vessels start to leak and swell up which can lead to loss of vision.

There are different levels of Diabetic Retin-opathy according to extent of disease and areas involved.

Normally the Central part of the Retina that is called the Macula, which is the most sensitive part of the vision, appears dark-er than the rest of the retina and it has no blood vessels running across.

The major effect on the central part of the Retina is usually with the leakage of fluids which can blur the central part of vision. This usually leads to increase thickness of this part of the retina which is now days can be measured with special equipment that is very useful in terms of diagnosis and mon-itoring recovery during the course of man-agement.

WHAT IS IT?WHAT EXACTLY IS ANINTRAVITREAL INJECTION (INJECTION)?

This is a minor surgery, which involves the injection into the eye (vitreous) of a sub-stance as the underlying condition may be either cortisone or a new class of drugs, the anti-angiogenic drugs.The injections are given by specialist in diseases of the retina and macular oph-thalmologists in specially designed areas. In their application, strict rules of sterility are followed and specially designed tools are used, that make the whole process very fast (takes about 6 minutes) and painless.

WHAT ARE THE SIGNS AND HOW DO INTRAVITREAL IN-JECTIONS WORK?

Diseases such as Age related macular de-generation (AMD), diabetic retinopathy (DR) and, to a lesser extent, other vascular and inflammatory eye diseases are the most frequent indication for the application of in-travitreal injections.These patients have lesions in the blood vessels that supply nutrients to the eye. Often weak and abnormal vessels also grow, that tend to bleed and leak fluid in the macular region. With intravitreal injections of appropriate drugs, inhibition of growth of abnormal blood vessels is caused and bleeding and leakage are reduced.

WHAT ARE THE RESULTS OF TREATMENT WITH INTRA-VITREAL INJECTIONS?

Traditional forms of treatment such as la-ser aim at the stabilization and not at the improving of the vision. This means that, in patients where some of the current treat-ments were successfully implemented, the vision, at best, remains stable or deterio-rates rapidly.With the intravitreal injections of new drugs, improvement of the vision is obtained in about half of the patients while further de-terioration of vision is prevented at a rate exceeding 90%. These results depend on the size and the age of the original lesion, and in many cases, repeated injections are required to maintain a successful result.
