2. To establish a master plan which will serve as basis for the whole titling process; 3. To put up a system that would establish a continuous releasing/awarding of titles in

an acceptable time pacing; 4. To ensure that after this project, all beneficiaries (Bambanense) shall have their

individual land titles. All executive orders, rules and regulations that are inconsistent therewith shall be deemed revoked, amended and /or amended accordingly.

OF POPULATION

WHEREAS, Republic Act (R.A.) No. 10625, also known as the Philippines Statistical Act of 2013, which was approved on 12 September 2013, states that the Philippines Statistics Authority (PSA) shall prepare and conduct periodic censuses on population, housing, agriculture, fisheries, business, industry, and other sectors of the economy; WHEREAS, the 2015 Census of Population (POPCEN 2015), which is scheduled to be conducted in August 2015 with 01 August as reference date is designed mainly to take an inventory of the population in the entire Philippines; WHEREAS, to ensure the successful conduct of the census, the citizenry must be called upon to support and cooperate in the POPCEN 2015 by providing complete and correct information to census enumerators; WHEREAS, PSA Board Resolution No. 9 Series of 2014 Approving the composition and terms of reference of the Regional, Provincial, City and Municipal Census Coordinating Board for the 2015 Census of Population;

1. Assess the needs of the PSA in the region in terms of facilities (transportation, communications, training and lodging), security, publicity, and other logistics;

2. Provide transportation facilities for the distribution/delivery of census materials and supplies and transmittal of census returns before, during, and after the census operations;

3. Provide communications facilities for use of census personnel; 4. Provide venues/facilities for the training of census personnel; 5. Provide temporary office/room/space that will serve as headquarter of census

personnel or storage area for census materials and supplies for the duration of the census operations;

6. Provide facilities for the lodging of census personnel assigned in areas outside their official station;

7. Conduct publicity and information campaign on the forthcoming census; 8. Assist the PSA in the census operations to ensure complete coverage of the

municipality; 9. Provide security/guide to census personnel and materials; 10. Refer to the PCCB matters which cannot be resolved in the MCCB; 11. DESIGNATION OF MUNICIPAL BuB FOCAL PERSON

WHEREAS, a strategy to reduce poverty incidence in the country is the implementation of Bottom-Up Budgeting (BuB);

WHEREAS, BuB aims to ensure the inclusion of the funding requirement s for

the development needs of the poorest municipalities and cities in the programs and budgets of National Government Agencies. WHEREAS, as per BuB 2015 Internal guidelines, the Local Government Units should designate a Municipal BuB Focal Person to coordinate and facilitate the implementation of BuB Project(s).

WHEREAS, there is need to create the Municipal 143 First Aid Team;

WHEREAS, the Municipal 143 First Aid Team will composed of at least 15 minimum members coming from the different departments/unit of our Municipal Government;

WHEREAS, the Philippine Red Cross-Tarlac Chapter, an independent, autonomous, non-

governmental organization duly existing by virtue of the laws of the Republic of the Philippines will conduct Safety courses for selected members of the Municipal First Aid Team;

RE-ORGANIZING THE MUNICIPAL DISASTER RISK REDUCTION AND MANAGEMENT COUNCIL

WHEREAS, in order to uphold the people’s constitutional rights to life and property, there is a need to address the root causes of vulnerabilities to disasters, strengthening the country’s institutional capacity for disaster risk reduction and management and building the resilience of local communities to disasters including climate change impacts,

WHEREAS, in order to provide the maximum care, assistance and services to individuals and

families affected by disasters, implement emergency rehabilitation projects to lessen the impact of disaster, and facilitate resumption of normal social and economic activities, the local Government of Bamban under the leadership of Mayor JOSE ANTONIO T. FELICIANO has adopted a disaster risk reduction and management approach that is holistic, comprehensive, integrated and proactive in lessening the socioeconomic and environmental impacts of disasters including climate change, promote the involvement and participation of all sectors and all stakeholders concerned, at all level, especially the local community.

WHEREAS, pursuant to Section 11 of REPUBLIC ACT NO. 10121 otherwise known at the Philippine Disaster Risk Reduction and Management Act of 2010, provides that:

SECTION 11. Organization at the Local Government Level. The existing Provincial, City, and Municipal Disaster Coordinating Council shall henceforth be known as the Provincial, City, and Municipal Disaster Risk Reduction and Management Councils. The Barangay Disaster Coordinating Councils shall cease to exist and its power functions shall henceforth be assumed by the Barangay Development Councils (BDCs) which shall serve as LDRRMCs in every barangay.

NOW, THEREFORE, I, JOSE ANTONIO T. FELICIANO Mayor of the Municipality of Bamban, Province of Tarlac, pursuant to the pertinent provisions of REPUBLIC ACT NO. 10121 otherwise known as the Philippine Disaster Risk Reduction and Management Act. Of 2010 and by virtue of the powers vested in me by the law do hereby order;

SECTION 1. establishment of the Municipal Risk Reduction and Management Council. Henceforth, the Municipal Disaster Coordinating Council of Bamban, Tarlac shall henceforth be known as the Municipal Disaster Risk Reduction and Management Council (MDRRMC-Bamban, Tarlac). The MDRRMC shall coordinate, integrate, supervise, and evaluate all local policies, programs and other activities related to disaster reduction. It shall be responsible in ensuring disaster sensitive municipal and barangay development plans, and in case of emergencies shall convene the different municipal line agencies and concerned institutions and authorities. WHEREAS, the State recognizes the barangay government as a political unit which serves as the planning and implementing arm of government policies, plans, programs, projects and activities. WHEREAS, in line with the government campaign to eradicate corruption, the DILG continues to encourage the involvement of various stakeholders in the community to achieve good local governance. EXECUTIVE ORDER NO. 25 SERIES OF 2015

REORGANIZATION OF THE LOCAL MANILA BAY CLEAN UP REHABILITATION AND PRESERVATION PROJECT INSPECTION, INVENTORY AND MONITORING TEAM

WHEREAS, in compliance with Supreme Court En Banc Decision dated December 18, 2008 and En Banc Resolution dated February 5, 2011, all Provincial Governors and concerned Mayors of Cities and Municipalities within the Manila Bay area are directed to organize the Local Manila Bay Clean Up, Rehabilitation and Preservation Project Inspection, Inventory and Monitoring Team. NOW THEREFORE, I JOSE ANTONIO T. FELICIANO, Mayor of Bamban, by virtue of the power vested in me upon by law do hereby reorganize the Manila Bay Clean Up, Rehabilitation and Preservation Project Local Inspection, Inventory and Monitoring Team. Section 1. Composition. The Manila Bay Clean Up, Rehabilitation and Preservation Project LGU Inspection, Inventory and Monitoring Team shall be composed of the following, to wit; Chairman : Hon. Jose Antonio T. Feliciano Municipal Mayor Members : Engr. Rosanna Calara Municipal Planning Officer Engr. Jose L. Vital Municipal Engineer Dr. Ellaine Lumboy OIC- Health Officer Mr. Julio Silvestre MENRO Ms. Josephine Saclayan MSDO Section 2. Duties and Responsibilities. The Manila Bay Clean Up, Rehabilitation and Preservation Project LGU Inspection, Inventory and Monitoring Team shall have the following duties and responsibilities:

1. Conduct inspection and inventory of all factories, commercial establishments and private homes within the LGU;

2. Monitor the compliance of non-complying factories, commercial establishments and private homes within the LGU;

3. Recommend the imposition of necessary sanctions and penalties to the Local Chief Executives of all non-complying factories, commercial establishment and private homes within the LGU;

4. Prepare and submit the following reports once every quarter. a. Quarterly Key Performance Indicators (KPI) Compliance b. MBCURP Form 1- Inventory and Inspection of Commercial Establishments, Factories and

Private Homes with Septic Tanks and Waste Water Treatment Facilities

c. MBCURP FORM 2.1 – Solid Waste Management (for Cities and Municipalities) d. MBCURP FORM 2.2 – Solid Waste Management (for Barangays) e. MBCURP FORM 3 – Informal Settler Families f. MBCURP FORM 4 – Information Education Campaign g. MBCURP FORM 5 – Associated Reports (Ordinances and Status of Implementation)

Initial Report shall be submitted not later than October 10, 2015 while the succeeding reports shall be submitted every 5th day of ensuing month after the end of the quarter.

5. Perform other task that may be assigned from time to time relative to the Implementation of the Manila Bay Clean Up, Rehabilitation and Preservation Project.

This order shall take effect immediately. Done this 6th of October 2015 at Bamban, Tarlac.

ORGANIZATION OF THE LOCAL VERIFICATION COMMITTEE (LVC)

WHEREAS, the LISTAHANAN or the National Household Targeting System for Poverty Reduction (NHTSPR) project of the Department of Social Welfare and Development (DSWD) is currently doing the validation and finalization phase which intend to assess and verify the preliminary list of poor, near poor and non-poor household and the correctness of the entries in the database; WHEREAS, to attain the objective, there is a need for the organization of the Local Verification Committee (LVC) in our Municipality NOW THEREFORE, I JOSE ANTONIO T. FELICIANO, Mayor of Bamban, by virtue of the power vested in me upon by law do hereby organize the Local Verification Committee (LVC) Section 1. Composition. The Local Verification Committee (LVC)shall be composed of the following, to wit; Chairman Hon. Jose Antonio T. Feliciano Municipal Mayor Vice Chairman Ms. Josephine Saclayan MSWDO Members Engr. Rosanna Calara MPDC Mr. Carlito M. Policarpio Couple’s For Christ Rep. Section 2. Functions:

1. Resolve all complaints and/or appeals brought by individuals and/or groups; 2. Identify households that have not been assessed based on appeals and complaints received; and 3. Certify the completeness or incompleteness of the List of Households living below the poverty
