New Coverage Effective January 1st, 2012

ELEA and Reliance Standard are pleased to announce the addition of Critical Illness and Accident insurance effective 1/1/12.

Each active full-time Employee working at least 20 or more hours per week will be eligible to take

advantage of these voluntary coverages.

How does Critical Illness insurance work?Critical Illness insurance is designed to pay a lump-sum benefit at the diagnosis of a covered critical illness.

Pays the benefit directly to the insured so you can spend the money as you see fit and stay focused on getting well.

You can choose the level of coverage most appropriate you and your family.

Key Advantage:Covered Critical Illnesses are divided into 3 different categories, This allows for the insured to Collect a benefit multiple times.

Collect multiple benefits from the same categoryYou can collect 50% of your benefit if you suffer from a 2nd diagnosis from the same category. This must occur no sooner than 18 months after the original diagnosis.

Why are there different categories?

Collect a Benefit twice from a different categoryYou can collect 100% of the benefit amount if you suffer a diagnosis from a

Different category. This must occur no sooner the 6 months after the original diagnosis

Each category has a 200% lifetime maximum allowing the coverage to pay up to 6x the chosen benefit amount.

Wellness

Wellness BenefitAn insured is eligible to receive a $50 benefit every 12 months upon the completion of a wellness screening. Some examples of accepted screenings are below

$10,000 of Family Coverage for a 35 year old = $162.00 Minus (3) $50 Wellness Benefits - $150.00 Total Cost per Year = $12.00

Included Features

Portability Allows the insured to continue coverage if he/she ceases to be eligible. Coverage must be in force for at least 12 months prior to exercising portability.

Pre-Existing Limitation (12/12)Conditions diagnosed or treated within 12 months of the policy effective date will be covered after coverage has been in place for 12 months.

Guaranteed Issue Coverage (Special One Time Offer)Employees and Spouses who apply for coverage under $15,000 will not have to answer medical questions or be medically underwritten.
