Driver Agreement Driver Ineligibility Defined › An employee will be ineligible to drive if within the past five (5) years, the
employee has any of the following on his or her driving record:
– DWI/DUI conviction for driving under the influence of alcohol or drugs (or related impaired driving offenses)
– Reckless driving offense(s) – Suspension or revocation of license – Vehicular felony conviction – Driving without a valid license
› An employee may be ineligible to drive if within a consecutive three (3) year
period, the employee has an accumulation of three (3) moving violations or a combination of moving violations and at-fault accidents that total three (3).
› An employee may be ineligible to drive if, within a consecutive twelve (12) month period, the employee has an accumulation of two (2) moving violations or a combination of moving violations and at-fault accidents that total two (2).
