RULES:1. No purchase necessary to participate. Official ballots can be found weekly in the HonoluluStar-Advertiser (Monday - Wednesday) and a printable version is available online at starad-vertiser.com. Photo copies of this ballot will not be accepted. You may also obtain a ballot bymailing a self addressed stamped envelope to Football Fever Ballot or by stopping by in person(see office location above). Only (1) ballot per person, per day allowed in person.

2. To qualify to win, your ballot must have as many or more correct picks than the winning panelist(Football Fever Panelist Picks found in Star-Advertiser every Friday). A game that ends in a tiewill count as a win for either team selected. Weekly winners will be determined by a randomdrawing of all qualified entries received. Winners will be contacted. No phone calls please. Theprevious week’s winner will be announced in the Star-Advertiser on Saturdays.

3. The weekly prize is a Vacations Hawaii trip for two to Las Vegas (value: $1,500) and a DirectTVCHOICE Package (BASIC) + NFL Sunday Ticket for a promotional period of (2) years (Retailvalue: $2,132) - includes service to (1) room in residence. Winner may not be an existing Di-rectTV customer, but service may be transferred to qualified recipient. If winner does notqualify for the DirectTV prize (as a existing customer or due to residence logistics), winnermay opt for a $500 check prize. (1) weekly winner will be selected each week, a total of (18)weekly winners will be selected during the promotional period (August 29 thru December 31).Winners to refer to prize vouchers for other terms and restrictions.

4. Mail in ballots must be postmarked by the Friday before each week’s games are played. Cutand paste to a 4” x 6” postcard. No envelopes accepted. The deadline for dropping off ballotsat any Oahu Hardware Hawaii stores is 4 p.m. on Thursdays. Hardware Hawaii locations: 1.Kailua, 30 Kihapai Street (266 1133), 2. Kaneohe, 46-184 Kahuhipa Street (233 1333), 3. Ma-punapuna – 704 Mapunapuna Street (831 3100). The deadline for dropping off ballots at theStar-Advertiser office (see office location above) is 5 p.m. every Friday. No exceptions.

5. You must be a Hawaii resident 21 years or older to participate.6. Employees (and their immediate families) of Oahu Publications, Inc. and all participating spon-sors are not eligible to participate.

7. Prize(s) are non-transferable and not redeemable for cash. No prize substitutions will be made.Oahu Publications, Inc. and all participating sponsors will not be liable for lost or stolen prizes.

8. Winners must be responsible for all taxes applicable to the total value of the prize(s) received.Winners will provide their Social Security Number and complete/submit an IRS Form W-9 fortax filing. An IRS Form 1099 showing the fair market value of the prize(s) may be issued tothe federal government for tax reporting purposes.

9. Winners authorize the Star-Advertiser to use their name and likeness for promotional ads atno additional compensation.
