As we constantly seek to improve and modify our products, we reserve the right to make changes in designthat vary from catalog descriptions.
Original or Counterfeit
KARL STORZ products are name brand articles renowned around the world and represent the state of the artin important areas of healthcare. A large number of “copy cat” products are currently being offered in manymarkets. These products are designed intentionally to resemble KARL STORZ products and use marketingstrategies that at least point out their compatibility with KARL STORZ products. These products are by nomeans genuine products, since genuine KARL STORZ products are sold worldwide exclusively under thename of KARL STORZ, which appears on the packaging and the product. Artery Retractor for EndoscopicRadial Artery Harvesting
Following the recent revival of the radial artery as thesecond vessel of choice for arterial myocardial revas-cularization, its endoscopic harvesting has been thesubject of great interest in order to achieve the sameclinical benefits as in endoscopic vein harvesting.
The endoscopic technique for harvesting the radialartery offers the following advantages compared tothe open procedure:

Due to the modular c system, the user can individually assemble the desired instrument at any time.

Nothing could be simpler!
The c series continues the development of in-struments which have proven their value for years. The simplicity of handling and ease with which the in-struments can be cleaned has been improved, in parti-cular with respect to assembly and disassembly.When the rear handle is positioned horizontally, the
handle can be separated from the outer sheath and theworking insert at the press of a button. Reassembly isequally reliable and quick. The high frequency connec-tion is mounted in a 45° angle on the upper side of thehandle, thereby ergonomically guiding the high fre-quency cable away from the field of operation.

The ROBI® System
KARL STORZ set itself the task of designing instru-ments that meet the specific demands of minimallyinvasive surgery and developed the rotating, bipolar(“ROBI®”) instruments – CLERMONT-FERRAND model –in co-operation with Prof. Wattiez (Strasbourg, France)for this purpose.
All instruments in this range can be rotated 360° andfeature jaws with a robust hinge for secure grasping.This makes the instruments safe and easy to use.
Another important feature of these instruments is thebipolar coagulation function. The use of bipolar energyoffers significant advantages over unipolar energy as thecarefully applied current flows between the branchesof the utilized HF instrument and must not travel over
long distances through the body of the patient via aneutral electrode. Furthermore, the jaws of the ROBI®
instruments are insulated by an intermediate ceramiclayer. This reduces flashover and, therefore, offersmore safety during HF application.
The new ergonomic handle design with a larger contactarea and the top-mounted 45° HF connector providesenhanced user comfort.
The instruments can be dismantled for easy and safereprocessing and also feature an irrigation connector atthe sheath. In addition to a wide range of scissors orgrasping and dissecting forceps in various lengths andsizes, KARL STORZ offers the ROBI® LUTZ graspingforceps specifically for endoscopic vessel harvesting.
The angled shape of this bipolar grasping forceps en-ables punctual coagulation away from the saphenousvein so that no damage is caused to the main branch.
First the forceps is used to compress the vein wallsalong the entire diameter of the vessel in order tointerrupt the blood flow and thus achieve sufficienthemostasis.
The non-insulated and, therefore, effective surface ofthe forceps is limited. The punctual application ofenergy on this limited surface prevents unnecessaryand inadvertent thermal damage to adjacent structuressuch as the nerves or skin.
In contrast to the use of a bipolar scissors, compres-sion completely interrupts the blood flow. Furthermore,thermal energy is utilized along the entire forceps sur-face for coagulation and does not disperse due to in-sufficient interruption.
The use of this bipolar grasping forceps is generallysufficient to detach the side branches of the greatsaphenous vein. In exceptional circumstances such as,for example, particularly large side branches, a clipapplicator proves useful.

Exact depth perception inside the human body is essen-tial for any endoscopic procedure.
Studies1 have shown that even experienced surgeonsbenefit from three-dimensional display technology withregard to the duration and precision of an intervention.
The IMAGE1 S 3D system provides excellent depth offield to enable precise hand-eye coordination in three-dimensional vision and simplifies particularly complexendoscopic procedures for the surgeon.
This first-class stereoscopic system from KARL STORZleads to significant improvements in surgical efficiencyand patient safety in the OR.
IMAGE1 S 3D consists of 3D video endoscopes with a 0°or 30° direction of view that are 10 mm in diameter as wellas 3D camera modules and 3D monitors with passive-polarized 3D glasses.
The lightweight design makes the 3D video endosco-pes easy to use, even after long hours of surgery.Furthermore, the 3D video endoscopes from KARL STORZare autoclavable.
Two image sensors at the distal end are preciselyaligned mechanically. Complemented by precise elec-tronics in the endoscope and camera control unit, thesystem generates endoscopic 3D images that are trueto life.

To have the direction of view best suited for eachsituation available at all times offers the surgeon ahigher degree of safety. With the ENDOCAMELEON®,visual inspection of the entire surgical field is easilyachieved. Instrument movement can be controlledthroughout the entire procedure and hemorrhages in
previously inaccessible areas can be detected andcontrolled. With a simple turn of the adjusting knob, theENDOCAMELEON® enables the user to easily select thedirection of view between 0° and 120° to suit all ORrequirements.

The innovative ENDOCAMELEON® technology is notdifficult to use and, due to the external moving parts,does not take up extra intracorporal space. Handlingremains straightforward and ergonomic. Image align-ment is the same as rigid telescopes: the direction ofview is selected by simply turning the adjustmentknob, making the system very intuitive to use. As theENDOCAMELEON® is equipped with a standard eye-piece, the variable direction of view benefits all standardcamera systems. Thanks to the HOPKINS® rod lenssystem, ENDOCAMELEON® also offers image qualitythat enables a useful application of three-chip camerasor HD camera systems.
Telescope with variable direction of view
Until now, surgeons had to choose in advance whichtelescope or direction of view to use in a procedure.Moreover, surgeons were restricted to the selected di-rection of view throughout the surgery or had to makean intraoperative telescope change.
To prevent this predicament in the future, we devel-oped the ENDOCAMELEON®: a telescope that allowsyou to adjust the desired direction of view – also duringsurgery – between 0° and 120°.
The ENDOCAMELEON® combines the user comfort ofthe proven HOPKINS® telescope with the advantagesand potential of a telescope featuring a variabledirection of view – offering you the quality you expectfrom KARL STORZ telescopes.

Laparoscopic Aortic Surgery
Patients with abdominal aortic aneurysms are increas-ingly being treated using endovascular techniques.However, due to the long-term problems related toendografts and migrating prostheses, laparoscopicaortic surgery is emerging as a minimally invasive alter-native, especially for younger patients. Thus, inaddition to conventional and endoluminal surgery, athird approach has become established.
Indications for laparoscopic aortic surgery:
Laparoscopic techniques can be offered to patientswith aorto-iliac diseases or abdominal infrarenal aorticaneurysms. A further indication, which is graduallyemerging, is related to patients with failing endograftsor graft migrations following stent implants. In thesecases, laparoscopy is used to occlude the lumbarartery or to fix prostheses.
A distinction is made between total laparoscopic pro-cedures and laparoscopy assisted techniques:

A total laparoscopic approach offers the patient allthe benefits of a video endoscopic procedure butrequires laparoscopic suturing of the aortic anas-tomosis and a certain degree of patient selection.Long-term results are excellent and the side-effectsof a conventional laparotomy can be avoided, i.e.,incisional hernia can thus be prevented.
2. Laparoscopy assisted aortic procedures involve amini-laparotomy between 7 and 10 cm with conven-tional instruments still being utilized to suture theaortic anastomosis. Postoperative pain and the inci-dence of incisional hernias is certainly greater thanthat of a total laparoscopic approach. However, thelaparoscopy assisted technique has the advantagethat it can be offered to most patients.
3. Hand-assisted laparoscopy is now established as athird approach. The advantage of this procedure isthat the surgeon can insert one hand into theabdomen during laparoscopic surgery without losingthe pneumoperitoneum. The so-called handport isinserted through a mini-incision of 7 cm and all theessential steps of the operation are performed underpneumoperitoneum. This procedure can also beoffered to the majority of patients. If the workingincision for the handport is made in the lowerabdomen, post-operative pain and the incidence ofincisional hernia is as low as in a total laparoscopicapproach.
The laparoscopic treatment of complications causedby endoluminal stent implants will play an increasingrole in the future. In this case, total laparoscopy can beused, for example, to open the aneurysmal sac, toremove a thrombus, or to repair a lumbar artery feedingan endoleak. Graft migration can also be prevented byapplying special suturing techniques.
Vascular surgery now avails of the special instrumen-tation required to offer laparoscopic techniques to pa-tients with aortoiliac occlusive disease or infrarenalaneurysms. It can be concluded that, with these mini-mally invasive techniques, postoperative recovery ofpatients is faster as compared to conventional surgery.

A prerequisite for successful, minimally invasive lapa-roscopic procedures is the selection of the correct HFinstruments and units combined with the basicprinciples of surgery.
During a surgical procedure, the operating surgeon mustbe able to grasp various tissue structures, to dissectand work on the relevant organ (incision/dissection),and to control hemorrhage (suturing and/or hemostasis).Instruments that perform multiple functions meansthere is less need for instrument exchange. This savesthe surgeon considerable time and effort.
KARL STORZ set itself the task of designing instrumentsthat meet the specific demands of minimally invasivesurgery and developed the rotating, bipolar (“ROBI®”)instruments – the CLERMONT-FERRAND model – inco-operation with Prof. Wattiez (Strasbourg, France) forthis purpose. Like all other instruments in the ROBI®
series, they are fully rotatable by 360° and can bedisassembled into a handle, sheath and insert. Theinstruments are autoclavable and easy to handle andclean.
ROBI® Bipolar Grasping Forceps
Grasping FunctionThe use of retractors in minimally invasive surgery isnot always possible or useful. The surgeon, therefore,requires an instrument that is capable of graspingtissue precisely and is able to maintain the tissue at asafe distance to the operating site.
To meet various needs and requirements, the jaws ofthe ROBI® bipolar grasping forceps vary in regard tosize and width. Jaws with rougher toothing are suitablefor grasping thicker and heavier tissue whereas jawswith finer toothing are better for grasping more fragiletissue structures. Fenestrated jaws offer the advantagethat even structures which are difficult to grasp can besecurely gripped.
To enable optimum grasping, even from less favorableangles and positions, all instruments can be rotated360°. This function proves to be particularlyadvantageous when using monoarticulate forceps inorder to place the jaws in the ideal position.
Dissection FunctionThe identification of tissue structures via dissection is aprerequisite for minimizing risks in precision surgery.
The delicate KELLY jaws represent a standard dis-section instrument without which the CLERMONT-FERRAND series would not be complete. We have alsoincluded very slender and extra-flat jaws to meet thedemands of all difficult anatomical conditions. By open-ing the handle, the jaws can be moved and the tissuesurface can be spread apart. Conventional bipolarforceps, for example, have no joint between the jaws.Closure of the jaws is effected by sliding an externaltube. As a result, opening is usually passive. Instru-ments with an imprecise grasping function do not allowprecise dissection.

The joint mechanism of the ROBI® instruments providesthe surgeon with a natural tactile feedback. The forceexerted on the tissue can, therefore, be felt at thehandle and thus permits a precise dose for dissection.“Tactile feedback enables optimal tissue dissection,”according to Wattiez.
In addition to biarticulate jaws for easier handling ofdelicate tissue and vessels, the CLERMONT-FERRANDseries also feature instruments with robust,monarticulate jaws which facilitate dissection in criticalregions and reduce trauma to a minimum. Theseinstruments are appropriate for oncological proceduressuch as, for example, lymph node dissection.
Micro instruments are being increasingly used in dis-section due to their enhanced precision and advance-ments in high frequency technology. The delicate in-struments from the CLERMONT-FERRAND series areideal for use with bipolar energy.
The Coagulation FunctionThe use of bipolar energy offers significant advantagesover unipolar energy. The carefully applied currentflows between the branches of the utilized HFinstrument and must not travel over long distancesthrough the body of the patient via a neutral electrode.With the aim of providing maximum user safety, ROBI®
instruments were designed and equipped with bipolarHF technology to ensure minimal patient discomfort.
An essential feature of ROBI® instruments is that theforceps inserts consist of two jaws that are insulated bya ceramic layer. This reduces flashover and, therefore,offers more safety during HF application.
Despite these safety precautions, it is important toobserve basic rules and principles for optimum workwith bipolar HF energy and to keep risks at a minimum.In general, it is important to ensure that no criticalviscera is in contact with the instrument and that onlythe relevant tissue is grasped between the two jaws ofthe forceps when activating or contacting the bipolarinstrument.
Coagulation occurs by means of heating the tissue. Ifthe tissue is heated too severely or the HF applicationtimes are too long, there is a risk that burns will occuraround the operating site. Heat dispersion can berestricted by selecting the appropriate HF powersetting. A 30 – 50 W power setting is generally suffi-cient for laparoscopic interventions. In principle, thehigher the selected HF power setting, the shorter theexposure time and vica versa. The longer the exposuretime, the more extensive the heat dispersion. Caremust be taken that HF energy has adequate pene-tration depth, which is not always easy for the surgeonto see or predict. After selecting the power setting, thesurgeon applies the energy with as short an exposuretime as possible.
The power setting used generally depends on the sizeof the selected jaws as the density of the energytransferred to the tissue is directly relative to the size ofthe jaws. With the same power setting, the narrower thejaws, the greater the energy density.
The thicker the jaws, the higher the power settingrequired. With thicker jaws, the operating surgeon hasto choose a higher power setting (approx. 50 W) butwith a shorter exposure time. With thinner jaws, a lowerpower setting of approximately 35 W is selected.

Tissue impedance is another important aspect thatneeds to be considered in order to avoid unintentionalthermal damage. Through more or less heavy com-pression of the tissue, the surgeon can alter the tissueimpedance and, thus, modify the dispersion of elec-trical energy. The more the tissue is compressed, thegreater the impedance. Altering the electrical HF energyinfluences coagulation time, which then becomesshorter. In contrast to conventional bipolar forceps,grasping forceps from the ROBI® series – thanks totheir tactile feedback – allow the pressure applied tothe tissue to be altered and, as a result, the impedanceto be changed.
During the procedure, blood and tissue may coagulateon the jaws. This may act as an insulation layer andchange the distribution of energy. In extreme cases,there is no current flow and coagulation is no longerpossible. To prevent soiling of the jaws, the appropriatepower setting depending on the size of the jaws mustbe selected. Furthermore, long exposure times andcoagulation in blood should be avoided. Precise,preventative coagulation is the best safety precaution.
ROBI® Bipolar Scissors
Cutting FunctionTo meet the different requirements of various tissuestructures during the incision phase, we also addedcurved METZENBAUM scissors as well as serratedjaws for difficult-to-grasp tissue structures to the existingstraight scissor versions in the CLERMONT-FERRANDseries.
Coagulation FunctionBipolar ROBI® scissors have also been speciallydesigned for use with HF energy and enable bothcoagulation and cutting in the coagulated area, the“tissue that has become white”. As soon as the tissuebecomes pink again, the incision is interrupted andcoagulation can be continued. Of course, the samesafety precautions apply to ROBI® scissors regardingthe use of bipolar HF energy as described above forgrasping forceps.
ConclusionOptimization of the minimally invasive intervention isdependent on the number and selection of functionsthat are possible for the surgeon. ROBI® graspingforceps offer the surgeon an instrument with grasping,dissection and coagulation functions. ROBI® scissorsalso offer the possibility of dissection and coagulationas well as, of course, incision. Thanks to the quick-change ROBI® system, working inserts can be rapidlyexchanged during surgery and according to surgicalrequirements. This multifunctionality leads to anoptimization of the surgical procedure which becomeseasier, faster and safer as a result.
The ROBI® grasping forceps and scissors from theCLERMONT-FERRAND series feature a high degree ofuser-friendliness in the customary KARL STORZtradition.

AUTOCON® II 400 SCBSafety Aspects and Main Features
Integrated voltage stability control or arc controlThe two state-of-the-art generators both guaranteeoptimal surgical cutting and coagulation power thatadapts continuously to the particular indication,especially when there are wide variations in the tissuestructures and tissue impedances. At the press of abutton, the operator can switch between arc control(TOP Cut mode) and voltage stability control (POWER Cut mode).
TOP Cut modeIn this mode, the HF energy required for a cutting effectis automatically reduced to the necessary physicalminimum in each individual case. The electric arcalways remains at a constant level, thus ensuring auniform surgical effect. Such precise work offers a clearsafety advantage and produces a cut that conservestissue and reduces stress for the patient. This increased safety with the AUTOCON®II 400 SCBunit is achieved by using the latest and fastest micro-processor and sensor technology, which enables theunit to capture all the important parameters, such asvariable incision speed, geometry of the activeelectrode, different impedance behavior of biologicaltissue types and fluids, and transition and contactresistances. This data is then used to adjust the HFpower output and HF voltage. This means that theoperator is not restricted in his work or obliged to adaptto the HF unit. On the contrary, the HF unit adaptsoptimally to all the user's application and operatingtechniques.
POWER Cut mode with constant HF voltageand powerThis mode ensures a uniform surgical effect and con-sistent cutting efficiency over a wide impedance rangeand many different tissue types.
RAM system – return electrode applicationmonitoringThis safety system continuously monitors the contactquality between the neutral electrode and the patient’sskin, and additionally indicates it with symbols. If thecontact surface area decreases, the safety systemgives an early visual and acoustic alarm, thus pre-venting a burn under the application site of the patientelectrode. To increase the contact reliability of theneutral electrode, the user can prevent the applicationof single-faced electrodes.
Gastro Cut and Papillo CutThese two new resection modes, which were speciallydeveloped by KARL STORZ for use in flexible endo-scopy, permit a fractionated and controlled cut with nobleeding. The special HF generator technology allowscontrolled output of cutting and coagulation pulsecurrent. Both the pulse sequence and the pulse speedcan be set separately and specifically for each mode.
LF/HF leakage currentsStray currents and the associated risk of burns areminimized by design measures.
SCB- and OR1™-compatibleThe units are designed for integration in the KARL STORZCommunication Bus (SCB). Full integratability of the HFunit in the networked, and even speech-controlled,operating room of the future is already a standardfeature (system requirement: RUI software release2009001-26 or later).
C-Cut® mode and LAP-C-Cut mode – theintermittent coagulation-cutting modewith AUTOCON®II 400 SCBDesigned for blood-free cutting during laparoscopy andwhen using irrigation liquids, the electric current isspecially modulated and offers a reproducible coa-gulation current with high cutting efficiency. This makestime-consuming subsequent coagulation a thing of thepast. Using the C-Cut® mode thus reduces the need forblood transfusions and saves surgery time. The result isan overall reduction in operating costs, and for thepatient it means additional protection.
Bipolar generator with 370 W HF powerThis outstanding performance range allows the unit tobe used in conjunction with the newly developedbipolar special and standard accessories. This high-end unit is even suitable for indications using irrigationliquids, which in the past could only be performed withspecial-purpose HF systems. The safety of the bipolartechnology from KARL STORZ eliminates the need forapplying a neutral electrode, including for interventionswhich used to be standard unipolar procedures.
Unipolar generator with 300 W HF powerWith a peak power of 300 watts, the AUTOCON®II 400SCB is ideally equipped for interventions in all areas ofuse.
