Informació per al/per a la pacient

1. En aquest full hi consten totes les prescripcions vigents introduïdes al sistema de recepta electrònica. Les prescripcions marcades en color gris ja han estat dispensades però encara no han superat la data de fi de tractament.

2. Per a poder recollir els productes a la farmàcia és obligatori presentar aquest full i la targeta sanitària individual, com a claus per a accedir a la vostra medicació. Aquest full no és una recepta.

3. Únicament és vàlid el darrer Pla de medicació. 4. Retireu de la farmàcia els medicaments a mesura que els necessiteu. Els tindreu disponibles fins a la data de fi de vigència

de la prescripció i els podreu anar recollint, d’acord amb la dosi i freqüència d’administració, a qualsevol farmàcia de Catalunya. 5. Si perdeu el Pla de medicació, us en donaran una còpia al centre de salut. (*) El càlcul d’aquest import s’ha determinat d’acord amb les dades disponibles en el moment de la prescripció.

D’acord amb la Llei orgànica 15/1999, de 13 de desembre, les dades consignades en aquest full constaran en el Registre de prestació farmacèutica. Les finalitats d’aquest

fitxer són el registre de l’activitat medicofarmacèutica i el control de la prestació farmacèutica. Aquestes dades es podran cedir als organismes que estableixi la legislació

aplicable. L’òrgan responsable d’aquest fitxer és l’Àrea d’Atenció Sanitària del Servei Català de la Salut (CatSalut). En virtut de la Resolució SLT/1202/2016, de 2 de maig,

podeu exercir els drets d’accés, rectificació, cancel·lació i oposició d’aquestes dades davant la Gerència d’Atenció Ciutadana del CatSalut (trav. de les Corts, 131-159; Edifici

Olímpia; 08028 Barcelona).
