SUMMARY

Padma Sweaters Ltd. factory building was constructed in 1994 and started production in 2004. The six (G+11) storied building was constructed as an industrial structure with total floor area of 5948.4sqm and 39.62 meters tall. The building is fully occupied by Padma Sweaters Ltd. from ground floor to roof. There are no separate sheds other than the main building. There were 918 workers in the factory during the inspection.

The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd.

(WEC). The purpose of the survey was to identify significant electrical safety issues and to provide

recommendations for remediation based on applicable standards specified by the Accord. The

scope of this initial electrical safety inspection was limited to the review and identification of major

electrical safety issues. The inspection did not include identification of minor deficiencies, which will

be further addressed as part of follow-up inspections.

Table below summarizes the major electrical safety issues identified during the inspection.

Recommendations have been provided to address each issue.

An implementation schedule shall be developed by the factory to remediate each of the

findings. The specific timing of improvements, including any requested extensions due to

design / installation constraints, shall be submitted to the Accord for approval.

Service line drawn from nearby DESCO feeder pole to substation through wall opening and laid close to generator.

CATEGORY:SERVICE LINE FINDING:

HT service line entering into substation room through wall opening and laid close to the generator without protection. Cable getting heated up due to generator and diesel spillage over the HT cable

RECOMMENDATION:

11kV service UG cable must be properly buried in underground trench 450x600 mm width and height respectively. Fill the trench with dry sand to insulate from generator heat and provide noncombustible metallic trench cover. Built the trench a little higher than the floor level so that oil spills don’t enter into the cable trench.

Breather of 100kVA power transformer showing deteriorated silica gel and empty oil cup

CATEGORY:TRANSFORMER ROOM FINDING:

Silica gel in the transformer breather deteriorated and transformer oil in the oil cup dried up.

RECOMMENDATION:

Change the transformer breather with fresh silica gel and fill the Breather oil cup with transformer oil up to the required level as instructed by the manufacturer. Establish a routine maintenance & inspection program for transformer as well as all other electrical equipment to ensure any future repetition of the occurrence. PRIORITY: P2

Transformer compartment located in the ground floor of the building.

CATEGORY: TRANSFORMER ROOM FINDING:

Transformer compartment, transformer, switchgears in electrical room and generator room filled with lint and dust.

RECOMMENDATION:

Transformer compartment, transformer, switchgears in electrical room must be dust and lint free. Regular cleaning and maintenance must be incorporated strictly. PRIORITY: P3

Transformer, generator and main system earthing looped together and exposed. It is located at the transformer compartment in the ground floor beside generator room.

Main earth bus exposed without proper bus bar chamber. RECOMMENDATION:

All earth looping must be made from proper sized bus installed in metallic bus-bar chamber. The bus must be insulated with noncombustible material after branch loop tapping. Bus-bar chamber or the enclosure shall be located not less than 1ft. above the finished floor level. PRIORITY: P1

Circuit breakers (MCCB, MCB) and motor control devices fixed on wooden plank flushed to wall without protection enclosure. RECOMMENDATION:

All protective and control devices must be fixed and protected in non-combustible enclosures made of metallic enclosure of minimum 20 SWG thickness and the panel body must be connected to main system earthing.

CATEGORY:CABLE & SUPPORTS FINDING:

Surface and exposed wiring in generator room encased in flexible PVC hose without proper support and laid in haphazard arrangements.

RECOMMENDATION:

Permanent wiring encased in flexible PVC conduit must be supported with rigid conduits/cable trays or noncombustible battens attached to vertical walls at orthogonal angles and clamped at regular interval of 600mm. Exposed wirings must be encased in rigid GI or PVC conduits

Bus bars not insulated and multiple cables/wires terminated in single lug. Panel enclosure earthing and door earth bond not provided. (typical in most of the panels)

RECOMMENDATION:

Live bus-bars in the panel must be adequately insulated with noncombustible insulating material. Multiple wire/cable termination in single lug must be removed and cable//wire must be provided with independent lug and connection point at bus. Panel enclosure and door should be connected to main earthing system and ensure zero potential all the time.

Generator batteries placed on a wooden stanchion beside generator room

CATEGORY:GENERATOR ROOM FINDING:

Generator battery placed on a wooden stanchion and the terminal poles not covered.

RECOMMENDATION:

Provide an acid proof battery stand for holding the batteries and ensure sufficient space around it for easy maintenance and periodic inspection and the battery terminal poles must be covered with acid proof insulation. PRIORITY: P2

The cable ducts are filled with excessive lint and dusts.

RECOMMENDATION:

Cable duct/trays must be neat and clean all the time. Establish a routine cleaning program to keep the channels run over the production floor neat and clean. Provide channel covering with noncombustible metallic sheet. The metallic tray must be connected to main earth at one point to ensure zero potential every time.

PFI panel placed temporarily on floor in substation room

CATEGORY:CABLE & SUPPORTS FINDING:

Steam line touching overhead cable duct and the steel cable tray is heated up to 141 degree centigrade.

RECOMMENDATION:

Steam line must be adequately heat insulated and a minimum of 1 ft. air gap between external heat sources and electrical system must be maintained at all times. PRIORITY: P1
