Introduction and Changes
Background Users of the Lenmed PSQ Application requires new functionality on reports, the following were
discussed:
1. Questions per Section per Department (New Report) The following report is required to indicate a breakdown of each question per section per
department. This will assist PLO’s and Unit Managers in identify areas that need investigating...

Formula: A formula will be formulated; this will be tested and submitted for approval.
Exporting: The PLO must also be able to export reports in a PDF file.
Permissions: All users can view and export the report.

3. Section Reporting to Unit Managers (New Report) The following report is required to submit progress reports on sections to Unit Managers, the PLO
will forward the appropriate email to the respective Manager or Unit Manager to address specified
concerns or issues, these emails will contain a PDF version of the survey captured.
The following is suggested: Under the “Overall Experience” section, should the “NO” option for
question; “Was the standard of service to your satisfaction? If NO, please elaborate,” Is chosen,
under the discretion of the PLO/ capturer, an automatic email to be generated to the PLO with a
copy of the captured PSQ on PDF format. The PLO will then identify the “problem section” of the
PSQ and forward that PSQ to the relevant Unit Manager / Manager.
Permissions: All users capturing surveys.
4. Ratings Per Section Per Department
The formula that was proposed is correct; it does not calculate N/A points. We discovered that the 2
departments “24hr Emergency Unit” and “NICU” had points for the “Catering” department on their
surveys hence it gave a rating.
5. Costing: 32 hours x R250.00
Reports

Questions per Section per Entire Hospital 8 hours R2000.00
Section Reporting to Unit Managers 12 hours R3000.00
Discount -R1000.00

6. Timeframes: Estimated time of completion 10 March 2014, this is dependant:
