THIS PROJECT CONSISTS OF REPLACING OLD BRIDGE NO. L2818 AND MINIMAL APPROACHWORK ON SAN FRANCISCO TWP ROAD 108 IN CARVER COUNTY TO ACCOMMODATE THENEW BRIDGE.

THIS PROJECT WILL REQUIRE THE DISTURBANCE OF 0.97 ACRES OF SOILS.

THIS PROJECT SWPPP WAS PREPARED BY RON BENSON, PE CERTIFIED IN THE DESIGN OFSWPPP BY THE UNIVERSITY OF MINNESOTA.

LONG TERM OPERATION AND MAINTENANCE

SAN FRANCISCO TOWNSHIP, CARVER COUNTY, MN WILL BE RESPONSIBLE FOR THE LONGTERM OPERATION AND MAINTENANCE OF THE PERMANENT STORMWATER MANAGEMENT.

RECEIVING SURFACE WATERS, DISCHARGE TO IMPAIRED WATERS & SPECIAL WATERS

THE TABLE BELOW IDENTIFIES ALL SURFACE WATERS WITHIN 1 MILE OF THE DISTURBEDSOIL PROJECT BOUNDARIES, WHICH WILL RECEIVE STORMWATER RUNOFF FROM THECONSTRUCTION SITE, DURING OR AFTER CONSTRUCTION.

STORMWATER FROM A DISCHARGE POINT ON THE PROJECT THAT FLOWS TO A SURFACEWATER IDENTIFIED AS IMPAIRED AND/OR SPECIAL MUST INCLUDE THE FOLLOWINGADDITIONAL BMP REQUIREMENTS:

1. ALL EXPOSED SOIL AREAS MUST BE STABILIZED AS SOON AS POSSIBLE TO LIMITSOIL EROSION BUT IN NO CASE LATER THAN SEVEN (7) DAYS AFTER THECONSTRUCTION ACTIVITY IN THAT PORTION OF THE SITE HAS TEMPORARILY ORPERMANENTLY CEASED.

2. TEMPORARY SEDIMENT BASINS MUST BE USED FOR COMMON DRAINAGE LOCATIONSTHAT SERVE AN AREA WITH FIVE (5) OR MORE ACRES DISTURBED AT ONE TIME.THIS PROJECT AS DESIGNED DOES NOT HAVE FIVE (5) DISTURBED ACRES DRAININGTO A COMMON LOCATION AND TEMPORARY SEDIMENT BASINS WILL NOT BEREQUIRED.

DESCRIPTION

PLAN VIEW

QUANTITY TABULATIONCONTRACTOR RESPONSIBILITIES

CONSTRUCTION SHALL BE GOVERNED BY THE MN/DOT SPEC BOOK (2014 EDITION).

THE CONTRACTOR WILL NEED TO IDENTIFY A MN/DOT CERTIFIED EROSION CONTROLSUPERVISOR FOR THIS SITE, THAT IS IN GOOD STANDING WHO WILL BEKNOWLEDGEABLE AND EXPERIENCED IN THE APPLICATION OF EROSION PREVENTIONAND SEDIMENT CONTROL BEST MANAGEMENT PRACTICES.

THE CONTRACTOR IS RESPONSIBLE FOR EROSION AND SEDIMENT CONTROL ON THISPROJECT.

THE CONTRACTOR WILL DEVELOP A CHAIN OF COMMAND WITH ALL OPERATORS ON THESITE TO ENSURE THAT THE EROSION AND SEDIMENT CONTROL WILL BE IMPLEMENTEDAND STAY IN EFFECT UNTIL THE CONSTRUCTION PROJECT IS COMPLETE, THE ENTIRESITE HAS UNDERGONE FINAL STABILIZATION, AND A NOTICE OF TERMINATION (NOT)HAS BEEN SUBMITTED TO THE MPCA.

FINAL STABILIZATION TO BE COMPLETED PRIOR TO SUBMITTING THE NOT INCLUDES:APPROXIMATELY 70% UNIFORM VEGETATIVE COVER OVER ALL PERVIOUS SURFACE, ALLTEMPORARY BMPS CLEANED OUT AND REMOVED, DITCHES STABILIZED, AND FINALMAINTENANCE/CLEANOUT OF PERMANENT STORMWATER SYSTEMS.

THE CONTRACTOR SHALL MAINTAIN THE SEDIMENT AND EROSION CONTROL MEASURESIDENTIFIED ON THIS PLAN UNTIL THE SITE IS STABILIZED. ALL EROSION AND SEDIMENTCONTROL DEVICES SHALL BE INSPECTED AFTER EACH RUNOFF PRODUCING RAIN EVENT.ALL NONFUNCTIONAL DEVICES SHALL BE REPAIRED OR REPLACED WITH NO ADDITIONALCOMPENSATION MADE THEREFOR.

IF SEDIMENT ESCAPES THE CONSTRUCTION SITE, OFF-SITE ACCUMULATION OFSEDIMENT MUST BE REMOVED IN A MANNER AND AT A FREQUENCY SUFFICIENT TOMINIMIZE OFF-SITE IMPACTS. NO FILLING EXCEPT THAT SPECIFICALLY DETAILED IN THEPLAN SHALL OCCUR WITHIN THE AREA CONTAINED BETWEEN THE UPPER STREAM BANKSOR ANY WETLAND.

THE DISTURBED SURFACE AREA AT ANY ONE TIME SHALL MEET THE FOLLOWINGINSPECTION AND MAINTENANCE REQUIREMENTS:

1. THE CONTRACTOR'S INSPECTOR SHALL INSPECT THE CONSTRUCTION SITE ONCEEVERY SEVEN (7) DAYS DURING ACTIVE CONSTRUCTION AND WITHIN 24 HOURSAFTER A STORM EVENT GREATER THAN 0.5 INCHES IN 24 HOURS.

2. THE CONTRACTOR SHALL KEEP A RECORD OF ALL INSPECTIONS AND MAINTENANCECONDUCTED DURING CONSTRUCTION. RECORDS SHALL INCLUDE:

- DATE AND TIME OF INSPECTIONS- NAME OF PERSON(S) CONDUCTING INSPECTIONS- FINDINGS AND RECOMMENDATIONS FOR CORRECTIVE ACTIONS- CORRECTIVE ACTIONS TAKEN (INCLUDE DATE AND TIMES)- DATE AND AMOUNT OF ALL RAINFALL EVENTS GREATER THAN 0.5 INCHES IN 24 HOURS- DOCUMENTATION OF CHANGES MADE TO THE EROSION CONTROL PLANS

3. ALL SILT FENCE AND DITCH CHECKS SHALL BE CLEANED OF SEDIMENT WHEN THESEDIMENT REACHES 1/3 THE HEIGHT OF THE SILT FENCE OR BALE CHECK.

CONSTRUCTION PHASING - EROSION AND SEDIMENT CONTROL SEQUENCING

SILT FENCE, CONSTRUCTION ENTRANCES, AND/OR OTHER SUITABLE PERIMETER BMP'SAS DEPICTED IN THE TEMPORARY EROSION AND SEDIMENT CONTROL PLANS WILL BEINSTALLED PRIOR TO THE START OF ANY LAND DISTURBING ACTIVITY. CONSTRUCTIONWILL BE REQUIRED TO BE PHASED SO THAT ALL DOWN GRADIENT SEDIMENT CONTROLMEASURES ARE INSTALLED PRIOR TO OR IN CONJUNCTION WITH ANY SOIL DISTURBINGACTIVITIES.

WHEN THE EXISTING TOPSOIL IS DISTURBED, THE TOPSOIL WILL BE STRIPPED ANDSTOCKPILED IN SOIL BERMS AT THE TOE OF THE STRIPPED SLOPES ALONG THE PROJECTLIMITS OR OTHER LOCATION DETERMINED BY THE CONTRACTOR. STOCKPILED TOPSOILBERMS WILL NOT BE PLACED IN ANY STORMWATER CONVEYANCES.

SEDIMENT DAMAGE FROM STOCKPILES WILL BE MINIMIZED BY PLACING A ROW OF SILTFENCE 5 FEET FROM THE TOE (INCIDENTAL).

ALL STOCKPILES LEFT FOR A PERIOD OF 7 DAYS SHALL BE TEMPORARILY STABILIZED.TEMPORARY STABILIZATION SHALL CONSIST OF RAPID STABILIZATION METHOD 3(INCIDENTAL).

AFTER STRIPPING THE TOPSOIL THE EXPOSED SOIL INSLOPES WILL BE STABILIZED WITHA RAPID STABILIZATION METHOD TYPE 3 WITHIN 7 DAYS AFTER THE CONSTRUCTIONACTIVITY IN THAT PORTION OF THE SITE HAS TEMPORARILY OR PERMANENTLY CEASED.

THIS ROAD CONSTRUCTION PROJECT AS DESIGNED DOES NOT MEET ANY OF THETEMPORARY SEDIMENT BASIN DISTURBED AREA THRESHOLD REQUIREMENTS ANDTEMPORARY SEDIMENT BASINS WILL NOT BE REQUIRED.

PERMANENT STORMWATER MANAGEMENT SYSTEM

ALL STORMWATER MUST BE DISCHARGED IN A MANNER THAT DOES NOT CAUSENUISANCE CONDITIONS, EROSION IN RECEIVING WATERS OR ON DOWNSLOPEPROPERTIES, OR INUNDATION IN WETLANDS CAUSING A SIGNIFICANT ADVERSE IMPACTTO THE WETLAND.

THIS ROAD CONSTRUCTION PROJECT HAS 0.23 ACRES OF IMPERVIOUS SURFACE ( 0.05ACRES INCREASE).

EROSION PREVENTION PRACTICES

STABILIZATION OF ALL EXPOSED SOIL AREAS MUST BE COMMENCED IMMEDIATELY TOLIMIT SOIL EROSION, AND COMPLETED WITHIN 7 DAYS AFTER THE CONSTRUCTIONACTIVITY IN THAT PORTION OF THE SITE HAS TEMPORARILY OR PERMANENTLY CEASED.FOR AREAS WHERE DISTURBED SOILS DRAIN TO AN IMPAIRED OR SPECIAL WATER THEEXPOSED SOIL MUST BE STABILIZED NO LATER THAN 7 DAYS AFTER THE CONSTRUCTIONACTIVITY IN THAT AREA CEASED. SEE THE IMPAIRED & SPECIAL WATERS SECTION OFTHIS SHEET FOR ADDITIONAL BMP REQUIREMENTS FOR DISTURBED AREAS THAT DRAINTO A SPECIAL OR IMPAIRED WATER.

THE NORMAL WETTED PERIMETER OF ANY TEMPORARY OR PERMANENT DRAINAGE DITCHOR SWALE THAT DRAINS WATER FROM ANY PORTION OF THE CONSTRUCTION SITE, ORDIVERTS WATER AROUND THE SITE, MUST BE STABILIZED WITHIN 200 LINEAL FEETFROM THE POINT OF DISCHARGE INTO ANY SURFACE WATER. STABILIZATION OF THELAST 200 LINEAL FEET MUST BE COMPLETED WITHIN 24 HOURS AFTER CONNECTING TOA SURFACE WATER WITH RAPID STABILIZATION METHOD 4.

PIPE CULVERT OUTLETS MUST BE PROVIDED WITH TEMPORARY OR PERMANENT ENERGYDISSIPATION WITHIN 24 HOURS AFTER CONNECTION TO A SURFACE WATER. THIS WILLINCLUDE DRAINAGE DITCHES THAT DRAIN WATER FROM ANY PORTION OF THECONSTRUCTION SITE.

SEDIMENT CONTROL PRACTICES

TEMPORARY STOCKPILED TOPSOIL BERMS MUST INCLUDE PERIMETER BMP'S ASPROVIDED ON THIS SHEET.

IN ORDER TO MAINTAIN SHEET FLOW AND MINIMIZE RILLS AND/OR GULLIES, THERESHALL BE NO UNBROKEN SLOPE LENGTH OF GREATER THAN 75 FEET FOR SLOPES WITH AGRADE OF 1:3 OR STEEPER.

ALL STORM DRAIN INLETS MUST BE PROTECTED BY APPROPRIATE BMP'S DURINGCONSTRUCTION UNTIL ALL SOURCES WITH POTENTIAL DISCHARGE TO THE INLET HAVEBEEN STABILIZED.

VEHICLE TRACKING OF SEDIMENT FROM THE CONSTRUCTION SITE MUST BE MINIMIZED.STREET SWEEPING MUST BE USED IF SEDIMENT IS BEING TRACKED OFF THECONSTRUCTION SITE ON PAVED SURFACES.

POLLUTION PREVENTION MEASURES

THE CONTRACTOR WILL IMPLEMENT THE POLLUTION PREVENTION MANAGEMENTMEASURES AS DIRECTED IN THE NPDES PERMIT PART IV.F 1-4 AS PERTAINING TO SOLIDWASTE, HAZARDOUS MATERIALS, EXTERNAL TRUCK WASHING, AND CONCRETE WASHOUTONSITE.

THESE MANAGEMENT MEASURES FOR POLLUTION PREVENTION WILL BE STRICTLYENFORCED.

ALL LIQUID AND SOLID WASTE GENERATED BY CONCRETE WASHOUT OPERATIONS MUSTBE CONTAINED IN A LEAK-PROOF CONTAINMENT FACILITY OR IMPERMEABLE LINERONSITE. NO LIQUID OR SOLID WASTE MUST CONTACT THE GROUND AND NO RUNOFF ISALLOWED FROM THE CONCRETE WASHOUT OPERATIONS OR AREA. ALL WASTE MUST BEPROPERLY DISPOSED IN COMPLIANCE WITH MPCA REGULATIONS. A SIGN MUST BEINSTALLED NEAR EACH WASHOUT FACILITY TO INFORM CONCRETE EQUIPMENTOPERATORS TO UTILIZE THE PROPER FACILITIES.

ALL CONSTRUCTION DEBRIS AND OTHER WASTES SHALL BE DISPOSED OF BY THECONTRACTOR IN A MANNER THAT COMPLIES WITH MPCA DISPOSAL REQUIREMENTS.ONSITE BURIAL OF WASTE IS EXPRESSLY PROHIBITED.

BURNING OF TREES, BRUSH, OR OTHER VEGETATED MATERIALS IS NOT ALLOWEDWITHIN PROJECT BOUNDARIES.

ALL BRIDGE DEMOLITION AND REMOVAL OPERATIONS SHALL BE PREFORMED IN AMANNER WHICH CONTAINS ALL DEBRIS FROM ENTERING THE STREAM. IF REMOVALOPERATIONS RESULT IN DEBRIS ENTERING THE STREAM, REMOVAL OPERATIONS SHALLBE SUSPENDED AND METHODS CORRECTED PRIOR TO RESUMING. ANY DEBRIS WHICHDOES ENTER THE STREAM SHALL BE REMOVED.

PAYMENT

NO DIRECT COMPENSATION WILL BE MADE FOR COMPLIANCE TO THE REQUIREMENTS ONTHIS SHEET INCLUDING TEMPORARY STABILIZATION, MAINTENANCE OR REPAIR OFTEMPORARY OR PERMANENT EROSION AND SEDIMENT CONTROL MEASURES, OR CLEANUP OF SEDIMENT OR ERODED MATERIAL. PAYMENT FOR INSTALLATION OF INDIVIDUALCONTROL MEASURES IS PROVIDED FOR AS TABULATED AND NOTED ON OTHER SHEETS.
