SAY: Today you will take a reading test. All items include an incomplete sentence and four

words listed below as answer choices. Read the incomplete sentence and choose the best

    word to complete the sentence.

Have students look at the Sample item on page 2.

SAY: It is at night. I need to know which word best completes the sentence. Ill read

the words: red, big, dark, high. Ill try each word in the sentence. It is red at night. It is

big at night. It is dark at night. It is high at night. I know that the sun goes down at night,

and it gets dark. Thats why the answer is C, dark. Do the rest of the items the same

way. Remember to choose only ONE answer for each item. Mark your answers on the

    answer sheet.

When everyone is ready to begin the test,

SAY: Before you begin working on your test, here are a few reminders:

    Read all test directions.

When there is a paragraph with an item, read it to help you choose the best word to

    complete the sentence.

Make sure you are answering on the correct line of the answer sheet.

Check all of your answers after completing the test.

When you are finished with your test, please close it and put it on the corner of

    your desk.

Some items on the test will be harder than others. You may not be able to do all of the

items on the test. Just do your best. If the paragraphs become too difficult for you to

read, you may close your test booklet and sit quietly.

You may now begin taking the test.

How to Use the Student Profile FormStep 1: Create the ProfileOnce you have scored the test, circle the numbers of t he items the st udent answered correctly. Then calculate the percentages.

Example:A student answers 25 of the 45 questions correctly, which converts to a percentage score of 56%. This student will benefit from

the Phonics Review provided in each cluster of Fundamentals. You might also consider using Inside Phonics to augment phonics instruction(see next step).
