Series of events from pre-accident communication to start of work

1. Used a copy of the Safety Check Slips for the previous job.

3. The planned work location turned out to be already completed, so

        they decided to work on a location that had not yet been completed.

4. They were going to change their assigned work after having a discussion with JR.

However, they were unable to discuss with the worker of that day, so,

they faxed the slip for the assigned work without making changes.

5. They were going to talk to the worker later, to change the assigned work.

6. They did not test if power was shut-off, nor did they provide proper grounding prior to starting work.

2. Because it was a copy, there were power shut-off marks in locations where power had not been shut-off.

→ Visually check that power is shut-off

2. Visually clearer support slips  

→ Installation of color fax machines

3. Prohibition of unplanned or unauthorized work by conducting stringent safety meetings.

→ In principle, once signed-off, work assignments may not be changed.

4. Increase safety patrol

→ Increase safety patrol on managers with an accident history.
