1. The Eastern Alliance for Science, Technology, Engineering and
Mathematics (EAST) at the University of Southern Maine is working with
Education Development Center, Inc. (EDC) to assess the impact of EAST
Institutes. EDC is a nonprofit research and development organization
dedicated to improving the quality, effectiveness, and equity of education. We
ask you to complete the survey below to help EAST and EDC understand how
the EAST Institute was implemented and what impact it had on participating
students. By participating you may influence the development and refinement
of the Institutes offered through EAST.
The risks associated with participating in this research are expected to be
minimal. Although a breach of confidentiality is possible, we have made every
effort to minimize this chance. We ask for your name on the survey, but we
will not use your name as we analyze the data. The results of the study, and
therefore excerpts of survey responses, may be presented at scientific
meetings and in published reports for educational, policy, and scientific
purposes but data will be reported in aggregate or quoted anonymously or
using pseudonyms. You will not be identified by name or described in such a
way that it is likely that you can be identified. This information is being
collected only for research purposes and identifiable data will never be shared
with your employer or EAST.
By checking the box below, you indicate that you have read the information
provided above and agree to participate in the research. Your decision to
participate is strictly voluntary. Your decision will not affect benefits or rights
to which you are otherwise entitled. Should you choose to discontinue your
participation in the study, you can withdraw your agreement to participate at
any time without prejudice. However, any information that you have provided
to that point will remain part of the study and may be used in later analysis
and reports. You may also decide not to answer any questions asked during
the study without prejudice.
Questions: If you have any questions or concerns regarding this study please
contact Dr. Babette Moeller (212) 8074205 ([email protected]).
Thank you for your cooperation!
