E-Wellness Self-Check Screening Procedure (Begins Monday, August 3rd)

1. Open the e-Wellness self-check website (bookmark on your computer or smartphone) 2. Click forward to the next screen (green box)

3. Review the disclaimer, click to agree (red box), and proceed to next screen (green box)

4. Please fill out: a. First and last name b. EID or PID c. Cell phone contact (if you do not pass screening and UEOHC/OHS needs to

contact you) d. Location (Faculty Physicians enter “Emergency Medicine”; Medical Center enter

“other” and “Emergency Department”) e. Manager’s last name (individual who UEOHC/OHS needs to notify if you do not

pass screening) f. Proceed to next screen (green box)

5. Select if you have any of the following symptoms or feel like you have a temperature greater than 1000F.

6. Select if you have a known community exposure or are awaiting results of COVID-19

testing.

7. If no symptoms, fever, community exposure or pending test you have passed screening!

8. If you do not pass screening, please contact your supervisor as well as Employee Health
