Event

On April 21st 2015

Fraser driller was pulling the last steel.

As the operator lifted the boom of the rig, the drill rig tilted forward.

While the drill rig tilted forward the left track came down and pinched the drillers right foot between the grousers.

Corrective Actions

Worker proximities to the rig training was done with all workers who work around the drill rigs. Example, sampler’s, driller’s, drill helper’s

Also, all people wanting to enter the work area of the drill rig, have to contact the driller and ask for permission to enter his work area.

Risk areas are areas surrounding the drill perimeter that are designated to show the potential risk area.

Drillers’-before starting to use the drill, make sure there is nobody inside the risk area of the drill rig.

Specific Working Distances in the Risk Areas of the Rig

Operating remotely, setting up functions at the drill location, or tramming, the drill operator is to be no closer than 1 meter to the rig. Operating remotely, any other worker is not to be within 3 meters of the drill when it is working, and only after the driller has been contacted and permission given to enter the risk area. 11

Specific Working Distances in the Risk Areas of the Rig

For in cab drills and when the drill is drilling in the hole, the drill operator can be closer than 1 meter to the drill, providing that a risk assessment of the area has been done by the drill operator before exiting the cab.

For in cab drills, any other worker is not to be within 3 meters of the drill when it is working, and only after the driller has been contacted and permission given to enter the risk area.
