No technical failure.Master believed set point speed would limit speed in which the vessel will come back to set point position. Master believed pressing “manual” on mode selector panel disengaged DP and cleared set point position.DP was in auto position mode all the time from pulling away from the rig until collision took place.DP handover was not in accordance with procedures and good watch keeping practice.

– Prioritizing of call to operators logistics center.– Handing over conn when vessel was in a turn and making sternway.– Master did not confirm that set point position was set at vessel’s present position.– 2nd Officer failed to acquaint himself with the operational status of the vessel when taking over

the conn.

Managing Director, HSE Manager and Fleet Managers holds monthly “safety calls” with the fleet to discuss HSE and quality issues. Main topics are safety campaigns, use of PPE ,watch keeping, voyage planning and safety trends.While some vessels occasionally will be prevented from participating in these calls due to operational reasons, approximately 95% of the fleet has participated on the last couple of calls. It was originally intended to have these calls on a monthly basis, but the calls will be held more frequently if the situation requires it.

The main tasks and responsibilities of the Safety Captain includes:– Display a strong commitment to QHSE

• Contribute to implementation and maintenance of the Company’s QHSE Management system on board the Company’s vessels, promote a first class QHSE culture within the Company and contribute to continuous improvement of the Company’s QHSE Management system.

– Assisting and mentoring vessel Masters and crew in the onboard implementation of the QHSE Management System, including;

• Provision of QHSE training.• Performing vessel safety verifications.• Facilitating the implementation of new safety initiatives.• Ensuring that all existing safety policies and procedures are being followed.

– Identifying best practices on board the vessels and facilitate sharing of best practices between vessels.

– Contribute to development of new, and revision of existing documents in the QHSE Management System.

– Participation at safety meetings, safety conferences etc.– Participate in internal investigations.– Participate in Check-out of new Captains– Evaluate DPO’s maneuvering and DP skills

As a part of our continuous improvement philosophy, we have started a safety verification program . Through this program a series of safety verifications will be performed on our vessels. In contrast to other audits and inspections, these verifications will be carried out while the vessels are in operation and will cover all aspects of safety and vessel operations. They will have a duration of approximately one or two week, and will be carried out by The Safety Captain. We are currently in the process of improving the audit tool for these verifications

The scope of these verifications is:– Verify compliance with company procedures and instructions in vessel

operations.– Assess operational relevance of company procedures and instructions.– Identify best practices.

Captains & Chief Engineers Conference

• A Captains and Chief Engineers conference is held annually with HSE, Operations and technical topics.

• The 2016 conference will be held in Bergen between 29th August and 2nd

September.• This years main topic will be DP operations, were Kongsberg DP experts will

be present. • The conference is carried out for both shifts.

Every two months a new safety topic will be given special focus. The first topic will be hand injuries. The topics will be posted in frames to be put on the inside of everytoilet/bathroom door (or any other suitable position in the toilet/bathroom which is clearly visible) on the vessel. For each toilet/bathroom two frames has been installed. One contains the twelve Tidewater Safe-T-rules, the other a flyer addressing the bimonthly safety topic.Every second month, the vessels receives a flyer with a new safety topic as well as relevant educational/learning materials to be addressed on safety meetings.

• Is it time for the industry to spend more time on operational audits with focus on Human behavior ,and less on verification of procedures and technical equipment (OVID, CMID, DP trials etc) ?

• Should the industry look at implementing mandatory DP simulator re-training ?

• This should be possible to do without increasing the cost, by reducing the frequency requirements for CMIDS, IMCA DP trials etc.
