Il Panitumumab IgG1 MAb, completamente umano, antagonista competitivo dell’EGF che blocca
specificamente le interazioni EGFR-ligando, inibendo la proliferazione e sopravvivenza cellulare, la motilità cellulare, l’invasione e metastatizzazione e l’angiogenesi tumorale.
Razionale di utilizzo Il Cetuximab
Il Cetuximab IgG1 MAb, chimerico, umano-murino, antagonista competitivo dell’EGF che blocca
specificamente le interazioni EGFR-ligando, inibendo la proliferazione e sopravvivenza cellulare, la motilità cellulare, l’invasione e metastatizzazione e l’angiogenesi tumorale.

Cetuximab Studi Preclinici in vitro: l’aggiunta del Cetuximab ad agenti
citotossici o alla radioterapia induce un effetto aggiuntivo all’inibizione della crescita cellulare o all’induzione della morte cellulare nelle linee cellulari di vari tumori umani.
Modelli di xenotrapianti di vari tumori umani hanno mostrato effetti antitumorali incrementati anche in vivo, quando il Cetuximab era associato a chemioterapia o a radioterapia.
L’aggiunta del Cetuximab all’Irinotecan ha incrementato l’inibizione della crescita tumorale e l’efficacia in xenotrapianti di CRC umani, refrattari all’Irinotecan.
Vantaggio nell’inibizione della crescita tumorale con l’aggiunta del Cetuximab all’Oxaliplatino, in xenotrapianti di CRC umani, sia sensibili che resistenti all’Oxaliplatino.

Terapia anti EGFR e KRAS KRAS: proteina normalmente attivata transitoriamente in risposta all’attivazione
dipendente dal ligando dell’EGFR. L’attivazione di KRAS, attraverso la cascata del segnale comprendente la pathway RAF-1/MEK/MAPK, stimola la crescita cellulare e la proliferazione. Mutazioni nel gene KRAS inducono la proteina KRAS in uno stato attivato, che conduce alla proliferazione cellulare incontrollata, alla differenziazione e alla perdita dell’apoptosi, portando alla crescita, invasione e metastatizzazione tumorale.
Il più frequente sito di mutazione KRAS (90%) è nel codone 12 e 13 del gene, che codifica per residui del dominio di legame del nucleotide guaninico della proteina KRAS. Tali mutazioni stabilizzano KRAS in uno stato attivo legato al GTP, impedendo l’intrinseca attività GTPasica della proteina

Non resectable > 70-80% of liver involvement; < 25% of remnant after liver resection; 6 segments involved.
Potentially resectable high risk, with advanced surgery > 1 hepatic vein involved; portal vein embolization; two-stage hepatectomy necessary; intraoperative ablation necessary.
Resectable high risk, with conventional surgery multiple metastases; involvement of < 4 segments.
Resectable low risk, with conventional surgery single metastasis ≤ 5 cm; metastatic disease metachronous.
