• Distant metastasis (eg. Liver, lung)• Local spread to the surrounding organs

– Aorta– Bronchial tree (oesophago-tracheal fistula)

• Poor general condititon• Severe concomitant disease

– Cirrhosis hepatic– Ischemic heart disease– poor lung function
