This summer, the building expansion kicked off with three new floors over the East Wing. The goal is to expand without interruption to patient care. “This is an exciting time for Hospital for Special Surgery with changes in many different areas to better accommodate patients and their families,” said Louis A. Shapiro, President and CEO. “The construction project is vital to our ability to continue to provide the first rate patient care for which HSS is so well known,” he continued. A new ninth floor will serve as the home for HSS’s renowned pediatric rehabilitation department. Features will include a large physical therapy gymnasium that will overlook the East River and facilities for occupational, fine motor skills, sensory integration, and speech therapies. Another feature of the ninth floor will be a relocated and enlarged Hospital pharmacy. Plans for the Hospital’s new tenth and eleventh floors include the addi- tion of an inpatient unit, which will include private rooms as well as rooms designed to accommodate two patients. These units will provide patients with the best possible envi- ronment by locating the patient rooms around a central staff/support core and providing each room with river-oriented views. Approximately 80% of Americans aged 65 and older suffer from a chronic condition, such as arthritis, that imposes limitations on their day- to-day lives. This segment of the population is rapidly growing, and will represent nearly 20% of the total U.S. population by 2030. “The additional space will allow HSS to stay ahead of the curve and will ensure that the patient experi- ence is an even better one,” said Lisa Goldstein, Chief Operating Officer. Dr. Pernis has made novel obser- vations regarding the regulation of inflammation and autoimmunity, which plays a critical role in diseases such as lupus and arthritis. the next – and most significant –phase of our expansion, as steel wasraised to expand the East Wing ofthe Hospital. That wing was con-structed in 1995 and was built as aseven story building. In 2007, weadded a new eighth floor, and now,with all of our city, state, and federalapprovals behind us, we will turnthis into an 11 story building withthe addition of three new floors. Surgeons from Hospital forSpecial Surgery have identi-fied a drilling technique thatimproves the outcome of

surgery to reconstruct the anteriorcruciate ligament (ACL). The newswas presented during the annualmeeting of the American OrthopedicSociety for Sports Medicine, in July.

In recent years, an improvedunderstanding of the anatomy of the ACL has allowed surgeons torefine techniques to reconstruct it.Investigators at Hospital for SpecialSurgery (HSS) set out to compare the outcomes of surgeries using twocommon techniques. “The goal inrepairing the ACL is to recreate thenormal anatomy, and there are avariety of different techniques to prepare tunnels for ACL reconstruc-tion that have evolved over time,”

said Dr. Asheesh Bedi,a fellow in sportsmedicine and shouldersurgery at Hospitalfor Special Surgerywho was involvedwith the study. “Thefocus of the study wasto compare two verycommon techniques interms of their abilityto reproduce thenative ligament anato-my and restore thestability of the kneeafter reconstruction ina cadaveric model.”

The investigatorsused 10 matchedcadaveric knees todirectly compare thetwo techniques. The

researchers found that the first tech-nique could not reproduce the natu-ral position of the ACL, whereas thesecond technique could better restorethe native anatomy.

“This study clearly demonstratesthat restoring the anatomy of theACL and the stability of the knee isfar superior when the femoral socketis reamed through the anteromedialportal rather than the tibial tunnel,as has been traditionally done bymost surgeons,” said David W.Altchek, MD, attending orthopedicsurgeon and co-chief of the SportsMedicine and Shoulder Service atHSS. “HSS is an international leaderin this innovative solution toward

further improving patient outcomesin ACL surgery.”

According to Dr. Bedi, the workhas translated into modified tech-niques in the operating room at HSS,where more than 800 ACL surgeriesare performed each year. Tears of theACL are quite common, withbetween 70,000 and 80,000 reportedeach year in the United States.

Going to BatAgainst TendonInjuries

For the past few years HSSphysicians Stephen Fealy, MD,Assistant Attending OrthopedicSurgeon, and Ronald S.

Adler, MD, PhD, Chief, Division ofUltrasound, Department of Radiologyand Imaging, have been hard at workresearching injuries that often plagueprofessional and amateur athletes:tendon injuries of the shoulder, elbowand knee.

Their research attracted the atten-tion of Major League Baseball in2007, when the organization awardedthem a three-year $150,000 grant toevaluate common tendon injuries.Since then, the doctors and their colleagues have made great stridestoward understanding tendon prob-lems, which they hope will eventual-ly lead to injury prevention. Theirfirst study used enhanced ultrasoundto show that blood supply to tendons,specifically the rotator cuff, decreas-es as people age.

“Use of ultrasound contrast agentsprovides an exquisitely sensitivemethod to assess vascularity at tendon repair sites,” Dr. Adler says.“This has implications in assessingthe efficacy of various physical therapy protocols in increasing bloodflow to the tendon.”

Earlier this year, Drs. Adler andFealy published an article in theJournal of Shoulder and ElbowSurgery documenting the healingprocess and patterns of vascular tendon changes after surgery. Usingenhanced ultrasound, they and others determined that tendons, par-ticularly those with a defect threemonths after surgery, lacked bloodflow. They hypothesize that improv-ing blood flow in the region may helpincrease the rate of tendon healingafter rotator cuff surgery.

“We have been able to demonstratethat an age-related alteration in bloodflow to the damaged tendon is onevariable linked to tendon failure,” Dr. Fealy says. “We are workingtoward reversing this process andkeeping athletes in their sport longer.”

Next, the physicians aim to deter-mine if this diminished vascularity isa cause or effect of a failed repair. ●

Dr. Pernis comes to HSS fromColumbia University where sheserved as the Advisor Dean and anAssociate Professor of Medicine. Shewas also a member of the HerbertIrving Comprehensive Cancer Centerat Columbia. Dr. Pernis received her

doctor of medicine from ColumbiaUniversity’s College of Physiciansand Surgeons in 1986. She did herinternship/residency in internal med-icine at Columbia University MedicalCenter from 1986–1989. Dr. Perniscompleted a fellowship in allergy/immunology at Columbia UniversityMedical Center from 1991–1994.

Dr. Pernis has received numerousgrants, and is currently the principalinvestigator on a National Institutesof Health (NIH) grant on SignalingCascades in Humoral Immunity andAutoimmunity. A new chair inautoimmunity and inflammation isbeing established as part of herrecruitment to HSS.

We want to usethis information and identify thepredictors for people doing wellover the long haul.” The patientsin the registry will be followed forat least five years.

Registry staff members areworking to contact 100 percent ofHSS patients having hip, knee, or shoulder total joint replacementfor consent to enter the registryand to fill out their baseline questionnaire. Patients are thensent a follow-up questionnaire at six months and two years after surgery.

“The registry really depends on the patients,” says Dr. Mandl.“The data the registry provides isenriched by each patient who consents and continues to fill outand send in the questionnaire each year.”

The results will provide infor-mation on outcomes, variationsand the economic impacts of totaljoint surgeries. To date, over13,000 patients have been enrolled,with an expected enrollment of20,000 patients by 2012. ●

With more people selecting totaljoint replacement surgery toimprove their mobility and main-tain active lifestyles, Hospital forSpecial Surgery is working todetermine how the implants aregoing to hold up to long term use.The joint replacement registry currently underway at HSS is aninitiative included in the Centersfor Education and Research onTherapeutics (CERT) programfunded by the Agency forHealthcare Research and Quality.The overall goal of the program isto conduct research and provideeducation that advances the opti-mal use of therapeutics such asdrugs and medical devices. Thespecific goal for HSS’s CERT initiative is to evaluate the use and success of the devices usedfor total hip, knee, and shoulderreplacements. HSS had previouslyestablished registries for hip andknee, which the new registrybuilds upon. It now includes infor-mation for shoulder replacement.This collaborative effort betweenHSS and Weill Cornell MedicalCollege will be the most compre-hensive registry in the UnitedStates focusing exclusively ontotal joint replacement. SpecialSurgery’s CERT program isdirected by Thomas P. Sculco,MD; Robert G. Marx, MD; Lisa A.Mandl, MD, MPH; and Michael M.Alexiades, MD.

Dr. Ross comes to HSS fromWashington University in St. LouisSchool of Medicine. He was a researchprofessor in the department of pathol-ogy and immunology at WashingtonUniversity in St. Louis School ofMedicine. He received his master ofscience in physiology from theUniversity of the Witwatersrand inJohannesburg, South Africa. Hereceived his PhD in molecular sciencesfrom University of Warwick, Coventry,UK in 1976.

< Special Surgery and the NYC Marathon In a new partnership with New YorkRoad Runners (NYRR), Hospital forSpecial Surgery is now an OrthopedicConsultant to New York Road Runnersfor the 2009 ING New York CityMarathon, Sunday, November 1.

Leading up to the Marathon, HSS willbe offering a series of educational ses-sions on injury prevention and will post aseries of articles on the NYRR website.The purpose of both is to teach marathonrunners what to expect, from a muscu-loskeletal perspective, as they train, andhow to prevent injury leading up to andafter the marathon. The sessions andarticles are targeted to both first-time andexperienced marathoners. HSS will alsobe offering educational sessions on“Marathon Monday,” the day after therace, on post-Marathon recovery.

Live-tweeting a New KneeHospital for Special Surgery opened theOR doors to a digital audience onWednesday, June 17, allowing unprece-dented access to a total knee replacementvia social media outlets online. The sur-gery, performed by Steven Haas, MD,chief of the Knee Service, was broadcastlive over the web. David Mayman, MD,narrated the surgery, but he wasn’t theonly one giving up-to-the-minute detailsto the surgery’s audience – medicalreporter Dr. Max Gomez, of CBS2, waslive-tweeting the surgery from thepatient’s side. Gomez posted updates toCBS2’s Twitter feed, which goes out tomore than 4,000 followers, and the

tweets were then shared on HSS’sown Twitter feed, HSpecialSurgery.

The surgery was successful andthe patient, Mary Kiefer, 55, ofMiddletown, NJ, was up and walking inrehab therapy the next day. Kiefer hadbeen postponing the surgery for eightyears, believing incorrectly that she wastoo young for the procedure.

At the start of a session, patients areeducated about theirdisease and the infusiondrug they will receive.Each station isequipped with automat-ic blood pressure and IV pumps andcurtains that provide privacy whenneeded. To help make treatment ascomfortable as possible, each stationalso features a lounge-style chair, pri-vate telephone, free Internet access,and a personal television. Soft drinksand a cold lunch or a snack are avail-able, depending on the treatment

When the Hospital forSpecial SurgeryInfusion TherapyUnit opened its

doors in 1994, it was the first ambu-latory infusion unit dedicated to thecare of patients with rheumatoidarthritis.

The center developed out of agrowing need for treatment servicesin the rheumatoid community, saysStephen Paget, MD, FACP, FACR,Physician-in-Chief and Chairman ofthe Division of Rheumatology.Typically reserved for patients whohaven’t found adequate relief withoral medications, infusion therapyhelps people with a variety ofrheumatoid conditions – disordersthat affect the joints, muscles, ten-dons, ligaments and bones – main-tain a normal lifestyle by providingmedication intravenously on an out-patient basis. Often, these conditionscause inflammation, swelling, andpain that can make everyday activi-ties difficult. Treatment can reducejoint inflammation, relieve pain, pre-vent or slow joint damage and loss offunction which helps patients main-tain productive, active lives.

Individual treatment times varywidely, ranging from an hour to 12hours, depending on the patient’s con-dition and medication. The center is

open weekdays from 8 a.m. to 8 p.m.to accommodate patients’ busy lives.As a result, wait time for an appoint-ment is minimal and the unit enjoys ahigh level of patient satisfaction. Dr. Paget can see why: “The InfusionTherapy Unit is a safe haven. Peopleare made to feel better both physical-ly and emotionally, so what could oth-erwise be a very anxiety-provokingexperience in actuality becomes acomfortable one. It’s a safe, soothingand supportive environment.”

Focused on Patients’ NeedsManaged by Linda Leff, RN-BC, a 20-year Special Surgery veteran, the Infusion Therapy Unit is one ofthe largest in the country and cantreat up to 25 patients a day, seeingabout 3,000 patients a year. The unit,recently featured in Infusion CentersAcross America: Advancing Optionsfor the Care of Rheumatoid Arthritisfor its advances in rheumatoid arthri-tis care, is staffed entirely by nurseswho coordinate with patients’ doc-tors, social workers, nutritionists,and physical therapists to provide apatient-centered approach and fullcontinuity of care. “At the beginningstages, patients are often frightenedand trying to find a treatment regi-men that works. Assistant attending orthopedicsurgeon Michael L. Parks, MD,was honored at the 2009 AmericanAcademy of Orthopaedic Surgeons(AAOS) annual meeting by beingnamed to the organization’s Boardof Directors. Dr. Parks will serveas a Member-at-Large for the“under 45 year old” Board posi-tion. The Directors play a pivotalrole in AAOS’ future, managingits affairs and maintaining admin-istrative authority of its activitiesand policies.

This spring, Hospital forSpecial Surgery orthopedicsurgeon David M. Dines,MD, performed two live

shoulder surgeries during profession-al conferences: the Live SurgeryInternational Shoulder AdvancedCourse in Annecy, France and theCurrent Concepts in Joint Replacement(CCJR) meeting in Las Vegas.

Dr. Dines, a respected shouldersurgeon and past president of theAmerican Shoulder and Elbow

Society, was one of only nineAmerican surgeons selected to par-ticipate in the Annecy Live SurgeryInternational Shoulder AdvancedCourse. Attracting surgeons from 47different countries and five conti-nents, the program features 33 livesurgeries over the course of fourdays, with the goal of demonstratingthe most up-to-date surgical tech-niques. According to LaurentLaFosse, course chairman, the pro-gram allows participants “to learnhow to face different situations anddifficulties managed by the bestworldwide experts.” Since it began in2005, the conference has beendeemed the “Woodstock of ShoulderSurgery” because of its ability tobring together over 1,000 surgeons

from all over the world. Dr. Dinesperformed a total shoulder arthro-plasty which entails resurfacing boththe humeral head and the glenoid(socket). The structure of the confer-ence allowed Dr. Dines to answerquestions from the audience as hewas performing the procedure.

For the CCJR meeting, Dr. Dinesperformed a reverse shoulder recon-struction. For patients suffering from debilitating rotator tear cuffarthropathy, reverse shoulder recon-struction provides pain relief, whilealso restoring stability and function.This specialized procedure hasgained popularity around the world,particularly in treating difficultcases, and is performed by relativelyfew shoulder surgeons, nine of whomare at HSS. The CCJR meeting is the

largest of its kind focusing on arthro-plasty, attended by over 2,000 ortho-pedic surgeons annually. The surgerywas telecast live from Hospital forSpecial Surgery’s operating rooms,with an orthopedic surgeon fromTexas serving as moderator.

According to Dr. Dines, HSSphysicians have always been at theforefront of orthopedics, whether pio-neering a new joint replacement ordeveloping innovative surgical tech-niques. Thanks to improved technol-ogy, live surgery telecasts give HSSsurgeons one more way to make significant contributions to the fieldof international orthopedic surgery.“There’s a beacon here,” he says. “We share it with the world.” ●

• A Children’s Pavilion, HSS’s newmusculoskeletal pediatric “hospitalwithin a hospital,” will span the Eastand West Wings of the fifth floor andinclude the Pediatric RehabilitationCenter on the ninth floor. The 34,000square feet of renovated and newly-constructed space will address thespecialized needs of families withsmall children and adolescents alike,in a warm, cheerful environmentdesigned specially for young patients.

• A Center for Hip Pain andPreservation was established thisyear with the goal of treating young,active adults suffering from hip pain.The Center includes surgeons andnon-operating physicians all focusedon preserving the hip joint over thelong term.

• The Sports Medicine and ShoulderService is also undergoing a renova-tion which will create a more sports-centered environment for doctorvisits and therapy appointments, aswell as for those who visit the SportsPerformance Center.

The addition of an eighth MRI in the Hospital is scheduled for laterthis year.

A Next GenerationScientist Looks atBone BiologyInvestigators at Hospital for SpecialSurgery, collaborating with scientists from other institutions, have contributedto the discovery that a gene called interferon regulator factor-8 (IRF-8) isinvolved in the development of diseasessuch as periodontitis (gum disease),rheumatoid arthritis, and osteoporosis.Baohong Zhao, PhD, a research fellow inthe HSS Arthritis and Tissue DegenerationProgram, was the lead author of a studypublished this summer in the journalNature Medicine. The discovery could leadto novel treatments of these diseases inthe future. Dr. Zhao initiated the studywhile working in the laboratory at ShowaUniversity, Tokyo, where much of thework was performed. Dr. Zhao completedthe study and extended the work tohuman cells during the past year at HSSworking with Dr. Lionel Ivashkiv.

MAKING UP FOR LOST TIME

Anne WellesWith pain so severe that she was

unable to braid her daughter’s

hair or tie her shoes, Anne Welles

refused to allow systemic lupus

erythematosus (SLE) to hold her

back. This young mother of two
