penalty of $1,000 and had a reprimand placed on his permanent disciplinary record with the Board

penalty of $400, an assessment for the costs of investigation of $833.92, and had a reprimand placed on
his permanent disciplinary record with the Board, because he operated a crane without being properly

Blair County Bethany Cann, license no. DS036656, of Altoona, Blair County, was levied a civil penalty of $3,000,
costs of investigation of $1,491, and must complete continuing education. (03/15/19) Lehigh County Nikki Marie Szemenyei-Guidon, license no. DH071433, DF003042, and DHA001626, of Bethlehem,
Lehigh County, was assessed a Civil Penalty of $750, costs of investigation of $320; and received a
reprimand on her disciplinary record with the Board because she practiced as a dental hygienist, expanded
function Dental Assistant, and administered local anesthesia while her licenses were lapsed (03/15/19)
Philadelphia County Lashanda Rorie, unlicensed, of Philadelphia, Philadelphia County, was ordered to cease and desist from
presenting herself as a licensed practitioner entitled or authorized to practice any profession requiring
licenses, registrations, certificates, approvals, authorizations, or permits issued by the Dental Board and
she will refrain from performing any activity which requires an authorization to practice the profession
issued by the Dental Board unless and until the Dental Board issues her the appropriate authorization to
practice the profession and cannot apply to the Board for any authorization to practice the profession for a
period of no less than five years from the date of the acceptance of the Consent Agreement. (03/15/19)

Out of State David Anthony Uliana, PA license and registration no. PE080388, of Centerville, Maryland, had a
reprimand placed on his permanent disciplinary record with the Board and was assessed a civil penalty of
$1,000 because he was guilty of misconduct in the practice of engineering in that he practiced the

engaged in unprofessional or immoral conduct in the practice of massage therapy. (10/29/18)
Lehigh County Shannon Lee Hardner, of Trexlertown, Lehigh County, was denied a massage therapist license because
she failed to prove both her good moral character and her ability to practice with reasonable skill and

PA Board of Medicine Chester County Laura Elizabeth Diana, license no. MD447027, of Pottstown, Chester County, voluntarily surrendered her
license, because she is unable to practice with reasonable skill and safety to patients by reason of illness;
addiction to drugs, or alcohol.
Christopher Henri Wendel, license no. MD023365E, of Landenberg, Chester County, was revoked based

Delaware County Lee Rachelle Wolfer, license no. MD444733, of Drexel Hill, Delaware County, voluntary surrendered her
license and a reprimand was placed on her permanent disciplinary record with the Board, because she had
a license or other authorization to practice the profession suspended, revoked, refused or otherwise

Franklin County Paul Douglas Orange, license no. MD039208L, of Fayetteville, Franklin County, had his license

Indiana County Michael S. Cash, license no. OS009606L, of Indiana, Indiana County, was automatically suspended,
based on being convicted of a crime that would be a felony under the Drug Act. (10/29/18)
Montgomery County Sheriee Wilson, PA license no. BH002170, of Wynnewood, Montgomery County, was indefinitely
suspended based on conviction of three felonies and conduct which was unprofessional and immoral.

Angie Sung Park and Sarah Laser Center, Inc., of Montgomery County, were assessed a civil penalty of
$1,000 and to cease and desist from performing any activity requiring a current and active license to
practice as a medical physician and surgeon in the Commonwealth of Pennsylvania unless those activities
are performed under the supervision and delegation of a medical physician and surgeon in accordance
with the Board’s regulations, because Angie Sung Park practiced medicine and surgery without otherwise
being authorized or exempted under of the act, and Sarah Laser Center Inc. has a corporation, partnership,
institution, association or sole proprietorship which aided or abetted the unlicensed practice of medicine.
Philadelphia County Brian Cable, of Philadelphia, Philadelphia County, was granted a license to practice medicine and surgery
and was immediately placed on probation for no less than three years based upon prior criminal

Westmoreland County Ashley G. Ponterio, license no. YM013844, of Greensburg, Westmoreland County, was indefinitely
suspended for no less than three years retroactive to February 6, 2018, for violating a previous Board

Out of State Vamsimadhav C. Korrapati, PA license no. MD435507, of Basking Ridge, New Jersey, was indefinitely

Fayette County Bonnie K. Bryner, notary commission no. 1019310, of South Connellsville, Fayette County, accepted the
suspension of her notary commission for three months, such suspension immediately stayed in favor of
probation, must complete a six- hour notary practice and procedures course, was levied a civil penalty in
the amount of $250 and probation assessment fees of $150, because she failed to maintain an accurate
notary register, failed to be familiar with the duties and responsibilities of a notary public by failing to
properly process BMV title transfer paperwork, and failed to be of good character, integrity and ability.
(03/25/19) Philadelphia County Sumeje Duka, notary commission no. 1291074, of Philadelphia, Philadelphia County, is suspended for no
less than 3 months, stayed in favor of probation, and required to attend six hours of approved notary
education and submit copies of her journal, based on findings that she failed to maintain a bound journal
with numbered pages and to record all the information required by law in the journal entries. (03/13/19)
Francis Van Zandt, notary commission no. 1170767, of Philadelphia, Philadelphia County, permanently
and voluntarily surrendered his notary commission because he failed to be of good character, integrity,
and ability, and to be familiar with the duties and responsibilities of a Notary Public by completing a
notarial transaction without requiring the personal appearance of all parties, and failed to keep and
maintain an accurate chronological notary register. (03/19/19) York County Alexis S. Vakoutis, notary commission no. 1258117, of York, York County, voluntarily surrendered her
notary commission because she failed to know through personal knowledge or satisfactory evidence that
the person appearing before her was the person described in and who was executing the instrument; failed
to be familiar with the duties and responsibilities of a notary public; and failed to be of good character,
integrity, and ability.
PA Board of Nursing Allegheny County Charles Edgar Cherkes, license no. RN677817, of Pittsburgh, Allegheny County, voluntarily surrendered
his license, because he is unable to practice professional nursing with reasonable skill and safety to
patients by reason of physical or psychological dependence upon alcohol.
Darlene L Williams, license no. PN084857L, of N. Braddock, Allegheny County, had her license
suspended for no less than three years, with such suspension stayed in favor of no less than three years of
probation, because of addiction to alcohol, or hallucinogenic, narcotic drugs, or other drugs which tend to
impair judgment or coordination; or has become mentally incompetent.

Jennifer Lynn Bayer, license no. PC005309, of Allison Park, Allegheny County, paid a civil penalty of
$2,000, an assessment for the costs of investigation of $4,579.97, received a reprimand on her disciplinary
record with the Board, was suspended for one year with such suspension to be immediately stayed in
favor of probation, and must complete 20 hours of remedial education; because she violated client
confidentiality, failed to protect the confidential information of clients, failed to take reasonable steps to
ensure that documentation in records is accurate, legible, and reflects the services provided, failed to
ensure that documentation accurately reflected client progress and services provided, and failed to
conform to the standards of acceptable and prevailing practice.
Maria Gonda Caiaccia, license no. RN264094L, of Sewickley, Allegheny County, had her professional
nursing license suspended indefinitely retroactive to September 7, 2017, and was ordered to pay costs of
investigation in the amount of $700.00, because she is unable to practice with reasonable skill and safety
to patients by reason of mental or physical illness or condition or physiological or psychological
dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs which tend to impair judgment
or coordination, so long as such dependence shall continue. (03/15/18)
Jade Adawn Young, license no. PN299663, of Tarentum, Allegheny County, was indefinitely suspended
retroactive to January 26, 2018, and assessed $700 in costs of investigation, based on being addicted to
alcohol, hallucinogenic or narcotic drugs or other drugs which tend to impair judgment or coordination.

Berks County Tammy Jo Hernandez, license no. RN600489, of Laureldale, Berks County, was indefinitely suspended
retroactive to April 30, 2017, and ordered to pay $1,000 in costs of investigation, based on being unable to
practice professional nursing with reasonable skill and safety to patients by reason of mental or physical
illness or condition or physiological or psychological dependence upon alcohol, hallucinogenic or narcotic
drugs or other drugs which tend to impair judgment or coordination, so long as such dependence shall

Blair County Lisa Marie Hunter, license no. PN286116, of Duncansville, Blair County, had her practical nursing
license suspended indefinitely retroactive to October 23, 2017, and was ordered to pay the costs of

skill and safety to patients by reason of addiction to alcohol or addiction to hallucinogenic or narcotic

Bradford County Danielle Marie Romano, license no. PN281591, of New Albany, Bradford County, had her practical nurse
license automatically suspended, based on being convicted of a misdemeanor under the Controlled
Substance, Drug, Device and Cosmetic Act. (05/25/18)
Bucks County Nadia E. Lapinski, license no. RN621986, of Holland, Bucks County, had her registered nurse license
suspended for no less than three years, retroactive to March 5, 2018, based on having violated the terms of
a previous board order. (11/29/18)
Sharon E Durivage, license no. RN307426L, of Yardley, Bucks County, had her license suspended for no
less than three years, during which time she may petition the Board for the suspension to be stayed in
favor of no less than three years of probation because she violated a lawful disciplinary order of the Board
and is addicted to alcohol or hallucinogenic or narcotic drugs or other drugs which tend to impair
judgment or coordination or has become mentally incompetent.
Daniel Michael McGee, license no. RN584382, of Levittown, Bucks County, had his registered nurse
license indefinitely suspended for a no less than four years, retroactive to June 6, 2018, the first year to be
actively served and the remainder stayed in favor of no less than three years of probation, for violating
terms of a previous Board order. (11/05/18)
Jennifer Moffa Ventresca, license no. RN239727L, of Chalfont, Bucks County, had her license to practice
professional nursing in the Commonwealth of Pennsylvania suspended for a period of two years with such
suspension immediately stayed in favor of two years of probation; had a reprimand placed on her
permanent disciplinary record with the Board; was levied a civil penalty of $1,000; and was ordered to
successfully complete the American Heart Association course titled: Advanced Cardiac Life Support,
because she had been guilty of unprofessional conduct. (03/01/19)
Carbon County Gail Blazejewski Hein, license no. PN082849L, of Lehighton, Carbon County, must take four hours of
continuing education on falls and four hours of continuing education on nursing ethics and received a
reprimand on her disciplinary record with the Board because she was guilty of unprofessional conduct.
Chester County Mary Monzo Aleszczyk, license no. RN513465L, of West Chester, Chester County, had her registered
nursing license indefinitely suspended and placed on probation for an indefinite period which shall run
concurrent with her term of criminal probation in Philadelphia County, and was ordered to pay a civil
penalty in the amount of $3,700.00 for having received Accelerated Rehabilitative Disposition in the
disposition of felony charges. (03/29/18)
Crawford County Joel Lincoln Chapman, license no. PN273918, of Conneautville, Crawford County, was indefinitely
suspended, based on being unable to practice practical nursing with reasonable skill and safety to patients

judgment or coordination so long as such dependence shall continue or has become mentally

Cumberland County Susan Spangler Reeves, license no. RN255548L, of Boiling Springs, Cumberland County, had her license
indefinitely suspended, because she is unable to practice nursing with reasonable skill and safety to
patients by reason of physical, mental illness, or condition, or physiological, or psychological dependence
upon drugs or alcohol and has violated a lawful disciplinary order of the Board. (03/01/19)
Denise Wilkins Starner, license no. RN291836L, of Mechanicsburg, Cumberland County, had her license
to practice nursing in the Commonwealth of Pennsylvania indefinitely suspended for a minimum of three
years, with such suspension immediately stayed in favor of probation, because she is unable to practice
nursing with reasonable skill and safety to patients by reason of physical or mental illness, condition,
physiological or psychological dependence upon drugs or alcohol; and violated a lawful disciplinary order

Delaware County Donte’ Donnard-William Thomas, license no. PN300959, of Chester, Delaware County, had his license
suspended for no less than three years with such suspension to be immediately stayed in favor of
probation and a reprimand was placed on his disciplinary record with the Board, because of addiction to
alcohol, or hallucinogenic, narcotic drugs, or other drugs which tend to impair judgment or coordination,
or has become mentally incompetent and violated a lawful disciplinary order of the Board.
Lisa M. Elia, license no. RN631366, of Broomall, Delaware County, had her registered nursing license
indefinitely suspended until she can prove that she is fit to safely practice with reasonable skill and safety
to patients, by reason of mental or physical illness or condition or physiological or psychological
dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs which tend to impair judgment

Billie Jo Pastuszok, license no. PN276870, of Clifton Heights, Delaware County, had her practical nurse
license indefinitely suspended, was assessed $125.06 in costs of investigation, and required to complete
education based on her pleading guilty to a felony. (06/12/18)
Elk County Lynn Marie Delullo, license no. PN278327, of Wilcox, Elk County, had her practical nursing license
suspended indefinitely retroactive to November 28, 2016, and was ordered to pay the costs of
investigation in the amount of $700.00, because she is unable to practice practical nursing with reasonable
skill and safety to patients by reason of addiction to alcohol or addiction to hallucinogenic or narcotic
drugs or other drugs which tend to impair judgment or coordination, so long as such dependence shall
continue, or she has become mentally incompetent. (01/29/18)
Erie County Ashley Ann Raucci, license no. PN287076, of Erie, Erie County, was assessed a civil penalty of $500,
because she was convicted, pleaded guilty, entered a plea of nolo contendere, or was found guilty by a
judge or jury; of a felony, or a crime of moral turpitude in the courts of this Commonwealth, the United
States, any other state, territory, or country, or received probation without verdict, disposition in lieu of
trial or an Accelerated Rehabilitative Disposition in the disposition of felony charges, or has been

penalty of $500 and costs of investigation of $1,000 because she performed surgery on a feline prior to
being licensed by the Board to practice veterinary medicine in the Commonwealth. (03/15/19)
Delaware County Steven Alan Cantner, license no. BV004305L, of Bryn Mawr, Delaware County, voluntarily permanently
surrendered his license to practice as a veterinarian, because he violated a lawful disciplinary order of the

McKean County Molly Rissmiller, of Bradford, McKean County, was ordered to cease and desist from holding herself out
as a veterinary technician and/or providing veterinary medical care requiring a license, registration or
certification from the Pennsylvania State Board of Veterinary Medicine; was levied a civil penalty of
$5,000, and costs of investigation of $942, because she held herself out as a Veterinary Technician and
engaged in the practice of veterinary medicine without being properly licensed or certified to do so.
