Impacto gráfico
Todo lo que vemos y sentimos a nuestro alrededor tiene sentido gracias al
fenómeno de la percepción, el cual abarca desde el momento en que recibimos un
estímulo (visual, auditivo, olfativo, táctil o gustativo) hasta el momento en que nuestro
cerebro da una respuesta comprensible de lo que hemos recibido. De esta forma, la
percepción influye directamente en el proceso de comprensión y memorización.
Mediante el proceso de creación mental de la comprensión, partimos de
cualquier información que nos transmite un emisor, y creamos una imagen del
mensaje que éste pretende transmitirnos. De forma básica, se puede decir que el
proceso de comprensión consiste en aislar, identificar y unir de forma coherente unos
datos externos con los datos de que disponemos. El proceso de comprensión en sí,
es el mismo en todos los casos aunque variarán los medios y los datos que
tendremos que utilizar para llevarlo a cabo.
La memoria es parte de un amplio proceso mediante el cual tiene lugar el
conocimiento, el proceso cognitivo. Entre los distintos tipos de memoria, la memoria
a corto plazo es la memoria operativa o de trabajo, donde podemos visualizar las
imágenes mentales y trabajar con ellas. La memoria a largo plazo almacena una
especie de modelo del mundo exterior muy esquemático con imágenes, experiencias,
ideas, procedimientos, etc.
La importancia de esta revista radica en su expresión gráfica, la cual facilita
ambos procesos, el de comprensión y el de memorización, ya sea por un pictograma
que resuma un contenido difícil de asimilar mediante un complejo y tedioso texto, o
por una imagen que facilite la asociación mental con el mismo.
Es por ello que lo que pretendemos crear en las retinas del lector es un
verdadero impacto gráfico que le sugestione y motive en su importante labor diaria
dentro del Laboratorio Clínico.

Fibrosis quística.
ECM de aminoácidos (Jarabe de Arce,
Tirosinemia tipo I).
ECM de ácidos grasos (Deficiencia de:
carnitina, acil-CoA deshidrogenasa de
cadena media, 3-hidroxi-acil-CoA
deshidrogenasa de cadena larga, acil-CoA
deshidrogenasa de cadena muy larga).
ECM de ácidos orgánicos (Deficiencia de
β-cetotiolasa, Aciduria 3-hidroxi-3-metil
glutárica y acidemias glutárica tipo I,
isovalérica, metilmalónicas y propiónica).
Uno de estos errores del metabolismo de
aminoácidos es la hipermetioninemia, que cursa con
una elevación de los niveles en sangre y orina de este
aminoácido azufrado. Nuestro estudio incluye 513
pacientes a los que se les solicitó un perfil de
aminoácidos en el Hospital Universitario 12 de
Octubre, que cubre el área sur y sureste de Madrid.
Estableciendo como punto de corte 400 µmol/L la
frecuencia de hipermetioninemia fue de 3/513.
Presentamos el caso de un varón de 11 meses
de edad en seguimiento por hipermetioninemia
diagnosticada en el cribado neonatal.
El estudio metabólico se lleva a cabo mediante
cromatografía líquida de alta resolución (HPLC) con
columna de intercambio catiónico y marcaje de
aminoácidos con ninhidrina, utilizando como muestra
el plasma de sangre venosa periférica en tubo con
EDTA. El espectrode los aminoácidos analizados se
muestra en la figura 1. La concentración en plasma de
metionina fue de 421 µmol/L (valores de referencia:
[15-45]).
Se ha descrito que la causa genética más
común de hipermetioninemia aislada en recién
nacidos se debe a un déficit de la enzima MAT1A
(metionin adenosil transferasa) implicada en el ciclo

DHasa and long chain fatty acid 3-hidroxi-
acil-CoA DHasa).
Organic acids IMED (β-cetotiolase
deficiency, 3-hydroxy-3-methyl glutaric
aciduria and isovaleric, methylmalonic and
propionic acidemia).
One of these errors of the amino acid
metabolism is hypermethioninemia, characterized by
high levels of methionine in blood and in urine. Our
study includes 513 patients whose amino acid analysis
were performed at Hospital Universitario 12 de
Octubre, which covers the south and southeast of
Madrid. The frequency of the event is 3/513 (cutoff:
400µmol/L).
An eleven-month-old boy diagnosed of
hypermethioninemia by neonatal screening.
The metabolic study was performed by cation-
exchange HPLC with ninhydrin post-column
derivatisation, using plasma (EDTA) as sample. The
list of amino acids is shown in Fig1. Plasma
methionine concentration is 421 µmol/L [15-45].
It is known that the most common genetic cause
of persistently elevated methionine in newborns is the
methionine adenosyltransferase (MAT1A) deficiency,
involved in the methionine cycle1. Its function is to
catalyze the synthesis of S-adenosilmethionine
(AdoMet or SAM), which is the major donor of methyl
groups for cell processes (Fig2). Furthermore, it is
essential to regulate sulfur amino acid, vitamin B12
and folate metabolism, and liver function2.

de la metionina1. Su función es catalizar la síntesis de
S-adenosil metionina (AdoMet ó SAM), principal
donante de grupos metilo en procesos celulares
(figura 2), indispensable en la regulación del
metabolismo de los aminoácidos azufrados, las
vitaminas B12 y folato y en la función hepática2.

La estructura de la enzima MAT está altamente
conservada a lo largo de la evolución. Se conocen 3
isoformas codificadas por los genes MAT1A (codifica
MAT I y MAT III) y MAT2A (Codifica a MAT II). MAT1A
(Chr 10:80,271,819-80, 290,002 GRch38 Assembly)
se expresa fundamentalmente en el hígado humano,
si bien se han descrito en páncreas y en pequeñas
cantidades en otros tejidos como cerebro. MAT II sólo
se expresa en hígado fetal4.
The structure of the enzyme has been highly
conserved during evolution. There are three known
isoforms, which are encoded by MAT1A gene
(encoding MAT I and MAT III) and MAT2A gene
(encoding MAT II). MAT1A (Chr 10:80,271,819-80,
290,002 GRch38 Assembly) is mainly expressed in
human liver, although its expression has been
reported in pancreas and in small amounts in other
tissues such as brain. MATII is only expressed in fetal
liver4.

Se realizó estudio genético al paciente
mediante secuenciación masiva, y posterior
confirmación por secuenciación capilar (Sanger). Se
encontró una mutación en heterocigosis en el gen
MAT1A (c.791G>A; p.Arg264His), sin descartarse
posibles mutaciones en regiones no estudiadas del
gen ni grandes deleciones. Se confirmó, con el estudio
genético de los padres, la presencia de esta mutación
en el alelo materno. Esta variante está asociada a un
patrón de herencia dominante y es clínicamente
benigna. El paciente se encuentra asintomático, con
buen estado general, y un correcto grado de
crecimiento y desarrollo. No precisa medidas
farmacológicas ni restricciones dietéticas, y se
alimenta vía lactancia materna.
La hipermetioninemia constituye una alteración
del metabolismo de los aminoácidos, de escasa
prevalencia. Su etiología es variada, incluyendo
causas genéticas como Tirosinemia I por déficit de
fumarilacetoacetato hidrolasa (FAH), homocistinuria por
déficit de cistationina beta sintasa (CBS), deficiencia
de glicina N-metiltranferasa (GMNT) y deficiencia de
S-Adenosil-homocisteina hidrolasa (AHCY); y causas
no genéticas como enfermedad hepática, prema-
turidad o elevada ingesta del propio aminoácido.
Existen dos patrones de herencia: autosómica
dominante y autosómica recesiva1. La forma
mayoritaria es la primera, clínicamente benigna y
caracterizada principalmente por la mutación
p.Arg264His en el gen MAT1A. La forma recesiva es
minoritaria y sintomática, en algunos casos se
produce una desmielinización cerebral progresiva que
induce retraso psicomotor, mental, distonías, cefaleas,
nistagmos y dificultades para el aprendizaje y
adquisición del lenguaje. Cuando la hipermetioni-
nemia es mayor de 600 µmol/L, puede aparecer olor
corporal y aliento desagradable debido a la presencia
abundante de dimetilsulfuros.
Genetic analysis were performed using massive
sequencing and afterwards they were confirmed by
Sanger sequencing. MAT1A heterozygous mutation
c.791G>A (p.Arg264His) was identified. However,
neither the possibility of mutations in other regions of
the gene nor the possibility of great rearrangements
can be dismissed. The family study confirmed the
mutation of the maternal alelle. This variant is
associated with dominant inheritance and is clinicaly
benign. The patient remains asymptomatic and grows
and develops appropriately. No treatment is required
and he continues to be breastfed.
Hypermethionine is a low prevalence inborn
disorder of amino acid metabolism. It has a wide range
of etiologies, including: genetic conditions, such as
fumarylacetoacetate hydrolase (FAH),
Homocystinuria due to cystathionine β-synthase
deficiency (CBS), Glycine N-methyltrasferase
deficiency (GNMT) and S-
adenosylhomocysteinehydrolase deficiency (AHCY);
and non-genetic conditions, such as liver disease,
prematurity or high intake of the amino acid itself.
There are two inheritance patterns: an
autosomal dominant manner and an autosomal
recessive manner1. The first one, more frequent and
clinically benign, is characterized by mutation
p.Arg264His in the MAT1A gene. The recessive form
is less common and occasionally simptomatic: it is
characterized by progressive brain demyelization
leading to psychomotor and mental retardation,
dystonia, cephalalgia, nystagmus and learning and
language acquisition difficulties. Methionine
concentrations beyond 600 µmol/L may cause bad
body odor due to high levels of dimethyl sulfide.
Given most individuals are asymptomatics, the
disorder requires no treatment4. Only severe cases
with neurological features justify a dietary methionine

Dado que la mayoría de casos de deficiencia de
MAT son asintomáticos, no se requiere tratamiento4.
Únicamente ante hipermetioninemias importantes con
clínica neurológica se lleva a cabo una restricción del
aminoácido en la dieta. En el caso de problemas
durante la restricción también se valora la
suplementación con SAM, que podría ser útil para
normalizar los valores de metionina.
El laboratorio desempeña un papel fundamental
en el abordaje de las hipermetioninemias. Se puede
detectar de forma precoz a los pacientes afectos
gracias a los programas de cribado neonatal y es
posible realizar un correcto seguimiento temprano de
los mismos para evitar o minimizar posibles daños
futuros.
La deficiencia de MAT1A es una alteración
genética generalmente asintomática, en la que una
correcta monitorización clínica y bioquímica de los
casos detectados es importante para evaluar la
conveniencia de una posible suplementación con
SAM o algún grado de restricción proteica. También
es de interés realizar un estudio de metioninemia en
los padres, ya que podría ser de ayuda para orientar
el diagnóstico y el tratamiento.

Laboratory plays a critical role in the medical
approach to hypermethionine disorders. It allows their
early detection by neonatal screening programs and a
suitable monitoring of the affected, in order to reduce
future symptoms.
Although MAT1A deficiency is a generally
asymptomatic genetic disorder, clinical and
biochemical monitoring of the affected cases seem
important to evaluate the suitability of a potential SAM
supplementation or some degree of protein restriction.
Also, family studies can be of interest, orientating the
diagnostic and treatment.

monoclonal basado en las recomendaciones del
Grupo Internacional de Trabajo del Mieloma (IMWG).
Este protocolo incluye en un primer lugar la realización
del proteinograma en suero (SPE) y la cuantificación
de las cadenas ligeras libres (CLL) séricas para
detectar la presencia del componente monoclonal.
Una vez identificada la presencia del componente
monoclonal se procede a su tipificación mediante la
realización de un inmunotipado (ITE) o inmunofijación
en suero (IFE). Un aspecto importante de este
algoritmo es que el estudio de la proteinuria de Bence
Jones (PBJ) en la orina de 24 horas no es necesario
en esta primera etapa del cribado. Una vez detectada
y tipificada la proteína monoclonal es cuando se
procede a la determinación de la PBJ en orina de 24
horas. No obstante, en pacientes con sospecha de
amiloidosis primaria (AL), es imprescindible la
realización de la PBJ aunque todas las pruebas en
suero sean negativas1,2.
Este algoritmo es muy útil en el estudio de
aquellos pacientes que presentan un componente
monoclonal muy pequeño y no detectable en el SPE
donde la cuantificación de las CLL en suero es
fundamental. En estos pacientes, la sospecha
diagnóstica se puede orientar hacia un mieloma de
cadenas ligeras, un mieloma de isotipo IgD o IgE o
incluso un mieloma no secretor. En este último caso,
la identificación en suero de la CLL monoclonal
reclasificaría a esta entidad como mieloma
oligosecretor3. Igualmente, otras entidades clínicas a
valorar en estas situaciones son la AL y la enfermedad
por depósito de cadenas ligeras según los datos
clínicos del paciente.
La cuantificación de las CLL en suero aporta
valor pronóstico en la gammapatía monoclonal de
significado incierto (GMSI), mieloma múltiple
quiescente (MMQ), mieloma múltiple activo (MM) y
AL. Además, la normalización del ratio entre las CLL
gammopathy based on the recommendations of the
International Myeloma Working Group (IMWG). This
protocol includes the determination of serum protein
electrophoresis (SPE) and the quantification of serum
free light chains (FLC) to identify the presence of a
monoclonal protein. Once a monoclonal protein in
identified, it is characterized by serum immunofixation
(IFE) or immunotyping (ITE). Interesting, the study of
Bence Jones proteinuria (BJP) in 24-hour urine is not
necessary in the first step of this algorithm. Once the
monoclonal protein has been detected and
characterized, the BJP is determined in a 24-hour
urine. However, in patients with suspected primary
amyloidosis (AL), it is essential to determine the BJP
although the serum tests were negative1,2.
This algorithm is very useful in the study of
patients with a small or non detectable monoclonal
component in the SPE where the quantification of
serum FLC is essential. In these patients, the
diagnostic study could be focused on light chain
multiple myeloma, IgD or IgE isotype or even non-
secretory multiple myeloma. In the latest kind of
myeloma, the identification of serum FLC would
classify this entity as oligosecretor multiple myeloma3.
Likewise, other clinical entities to assess in these
situations are AL and light chain deposition disease
according to the patient´s clinical data.
The quantification of serum FLC levels provides
prognostic value in monoclonal gammopathy of
uncertain significance (MGUS), quiescent multiple
myeloma (SMM), active multiple myeloma (MM) and
AL. In addition, the normalization of the serum FLC
ratio after treatment of MM is a fundamental parameter
in the definition of the stringent Complete Response
(sCR), a higher response rate than the complete
response (CR) and it has prognostic value on patients
in remission of the disease1,4. Recently, a serum
involved/uninvolved FLC ratio of 100 or greater,

en suero tras el tratamiento del MM constituye un
parámetro fundamental en la definición de la
respuesta completa estricta (RCe), un grado de
respuesta superior a la respuesta completa (RC) y que
tiene a su vez valor pronóstico en los pacientes en
remisión de la enfermedad1,4. Recientemente, una
relación entre las CLL involucrada y no involucrada en
suero igual o superior a 100 se considera un evento
definitivo para el diagnóstico de MM activo, siempre
que el nivel absoluto de la CLL involucrada sea al
menos de 100 mg/L (la CLL involucrada es la cadena
ligera libre monoclonal que está por encima del
intervalo normal de referencia mientras que la CLL no
involucrada es la que está dentro o por debajo del
rango normal)5.
En conclusión, el algoritmo diagnóstico
expuesto en la figura permite detectar de forma rápida
y sencilla la presencia de un componente monoclonal
mediante el uso combinado de “SPE+CLL”. Además,
con la cuantificación de las CLL en suero se pueden
identificar a pacientes con componente oligosecretor
que no se detecta o es débilmente detectado por las
técnicas tradicionales (SPE e IFE).

In conclusion, the protocol exposed in figure
allows a quickly and easy way to detect the presence
of a monoclonal component by the combined use of
"SPE+FLC". In addition, quantification of serum FLC
allows us to identify patients with oligosecretory
component that can not be detected or is weakly
detected by traditional techniques (SPE and IFE).

El cuadro clínico del paciente se clasificó
inicialmente como dermatitis inespecífica y se inició
tratamiento con ciclopirox y prednicarbato. A los 3
meses, y debido a la persistencia y aumento del
número de lesiones se realizó el estudio
microbiológico. En la imagen se pueden observar
abundantes hifas, pequeñas, ligeramente curvadas y
con disposición en cadenas cortas; y levaduras,
formando densos racimos de células ovales o
redondeadas. Estas células presentan en un extremo
un prominente collar a modo de tabique plano y
asemejando una cicatriz resultado de la formación
continuada de las células hijas en ese extremo de la
célula, (gemación monopolar). Este hallazgo confirmó
el diagnóstico de pitiriasis versicolor. Al paciente se le
retiró el corticoide y se continuó el tratamiento con
ciclopirox tópico en champú1 y en crema durante 2
meses, con buena evolución hasta la fecha.
El diagnóstico microbiológico se realizó
mediante microscopía de fluorescencia (400x), con
tratamiento de la muestra con KOH y tinción con
blanco de calcoflúor (Remel Bactidrop Calcofluor
White, Thermo Scientific; filtro Exc:330-380/
EspDic:400/Bar:420; lámpara LED UV, pE300 Light
Source). Las células de las distintas especies del
complejo presentan un tamaño y formas variables
(entre 1,5 a 8 µm de longitud, y formas redondeadas,
ovaladas y cilíndricas). El examen microscópico es
suficiente para establecer el diagnóstico. Aunque en
nuestro contexto clínico no era necesario, también se
puede realizar el cultivo en medios sólidos. Debido a
los especiales requerimientos lipofílicos de
Malassezia (a excepción de M. pachydermatis2), no es
fácil su aislamiento a partir del cultivo de muestras
clínicas. La identificación de las distintas especies del
complejo se basa en la morfología, utilización
diferencial de lípidos, pruebas bioquímicas y tipo de
crecimiento a 37ºC3,4. Para las especies con
The patient had a clinical picture that was first
classified as an unspecific dermatitis. He was treated
with ciclopirox and prednicarbate. Three months later,
as lesions persisted and increased, microbiological
study was carried out. Small, slightly curved, arranged
as short chains hyphae and clustering oval to round
yeast cells can be observed. These cells show at one
end a prominent bud scar or collarette due to the
continued formation of daughter cells at that end of the
cell (monopolar budding). This picture confirmed the
diagnosis of pitiriasis versicolor. Corticoids were
retired and topical ciclopirox (shampoo1 and cream)
was given to the patient for two months, remaining a
symptomatic so far.
The infection was diagnosed by using
fluorescence microscopy (400x), previous sample
processing by KOH and calcofluor white stain (Remel
Bactidrop Calcofluor White, Thermo Scientific; filters
Exc:330-380/ MirDic:400/Bar:420; LED UV lamp,
pE300 Light Source). The cells of the different species
of the complex vary in shape and size (1,5-8 µm of
length and spherical, oval to elongated forms).
Microscopic examination is enough to diagnose the
infection. Although not mandatory is this clinical
setting, fungal culture in solid media can be made. The
special lipophilic requirements of Malassezia
(excluding M. pachydermatis2), make difficult the
recovery from culture of clinical samples. Identification
of the individual species is based on morphology,
utilization of certain lipids, biochemical tests and rate
of growth at 37ºC3,4. The more fastidious lipophilic
species require more enriched media (Leeming,
Dixon3,4). In this case, we employed the most popular
media, Sabouraud agar, with olive oil overlaying agar
(7 days, 30ºC). We only could identify the strain to the
species complex level, as our resources only allow us
to support on morphological microscopy characters
and the olive oil requirement for growth.

requerimientos lipófilos más exigentes son necesarios
medios más enriquecidos (Leeming, Dixon3,4). En
nuestro caso, el medio que empleamos es el más
comúnmente utilizado, agar Sabouraud, recubierto
con aceite de oliva (7 días, 30ºC). La identificación se
realizó a nivel de complejo de especies, debido a que
nuestros recursos sólo nos permiten basarnos en los
caracteres morfológicos microscópicos y el
requerimiento de aceite de oliva para su crecimiento.
Malassezia es una levadura lipófila colonizante
de la epidermis de forma epiparasítica, produciendo
una mínima respuesta inflamatoria del huésped5. La
pitiriasis versicolor presenta una distribución mundial,
si bien su incidencia aumenta en los climas húmedos
y cálidos. Cursa con máculas serpiginosas separadas,
hipo o hiperpigmentadas, en la piel de tórax, brazos y
parte superior de la espalda, no dolorosas y sin
prurito6. La piel afectada no se broncea bien, lo que
explica que las consultas sean más frecuentes en
verano, que es cuando las lesiones se hacen más
evidentes. Existe predisposición individual y su
aparición se relaciona con la presencia de ciertos
aminoácidos y compuestos hidrófobos en la piel, así
como con la disminución del recambio epitelial en el
estrato córneo.
El tratamiento consiste en la administración
tópica de azoles (clotrimazol, miconazol...), o de
terbinafina, ciclopirox o sulfuro de selenio; en formas
extensas se emplea itraconazol o fluconazol orales7.
Malassezia is a lipophilic yeast that colonizes
the epidermis in a epiparasitic way, producing a
minimal inflamatory host response5. Pitiriasis
versicolor has a worldwide distribution, although its
incidence increases in warm and humid climates. The
signs are serpiginous isolated maculae, hypo or
hyperpigmented, in the torax, arms or upper back skin,
with no pain or itching6. Skin does not tan properly, so
people search more medical counselling in summer,
when lesions are more evident. There exists individual
predisposition to the disease. Its appearance is related
with the presence of certain aminoacids and
hidrophobical compounds from the skin, as well as a
decreasing epitelial turnover of the stratum corneum.
Treatment consists in topical administration of
azoles (clotrimazole, miconazole…) as well as
terbinafine, ciclopirox or selenium sulphide; in
extensive forms orally fluconazole or itraconazole is
used7.

La actividad de los laboratorios se ha medido
como número de determinaciones. Y sigue
midiéndose así…
Los enfoques de “productividad” pretenden
disminuir costes (del laboratorio) aumentando la
producción y disminuyendo coste unitario. Los
resultados ignoran el impacto del mal uso del
laboratorio en el coste total del sistema3. Surgen los
modelos de “no hacer” o “elecciones sabias” (choose
wisely) que en nuestro país ha liderado el Ministerio
de Sanidad4,5.
Los costes de pruebas diagnósticas son un
elemento recurrente en los discursos sobre
sostenibilidad. Los estudios de evaluación económica
publicados son discutibles, se suelen usar costes
directos (recursos materiales o humanos) en lugar del
efecto que puede tener la actividad del laboratorio en
el proceso del paciente.
Costes y ahorro en relación al laboratorio deben
identificarse y evaluarse dentro del proceso, tanto
antes (validación previa de la petición y su gestión) de
su intervención, como después, (downstream cost)
midiendo su impacto sobre:
- Acciones sobre el paciente
- Falsos positivos y negativos de la técnica y/o
de la interpretación de los resultados
- Disponibilidad de los resultados
- Tiempos de respuesta (brain to brain)
El modelo de laboratorio es esencial en los
resultados en salud que proporciona. Está definido
más que por sus tecnologías, por la inteligencia y
desempeño profesional en la gestión del proceso,
entendido como un trabajo realizado en equipo donde
los servicios de diagnóstico son un actor activo6.
La bibliografía nos enseña que una parte de la
actividad que realizamos no sirve para nada o incluso

Productivity approaches aim to reduce costs
(laboratory costs), increase production and decrease
unitary costs. The results ignore the impact of the
laboratory misuse on the system’s overall costs3.
Therefore, new laboratory models have emerged
based on a premise to “choose wisely”. In Spain, the
design of this kind of models to proceed efficiently
inside laboratories has been led by the Ministry of
Health4,5.
When discussing sustainability, the costs of
diagnosis tests are always a key issue. However, the
economic studies published up until now can be
debated since they are based on direct costs (material
and human resources) and they do not take into
consideration the effect that the laboratory activity may
have on the patient’s process.
Both laboratory related costs and savings need
to be identified and evaluated within the process:
before (pre-validation of the request and its
management) and after (downstream cost). And this
has to be carried out measuring its impact on:
- Action taken on the patient
- False positives and negatives of the technique
and / or interpretation of the results
- Availability of results
- Turnaround time(brain to brain)
The laboratory model is essential when it comes
to the health results it provides. It is not defined by the
technologies used in said model but by the intelligence
and professional performance present in the
management of the process, meaning the team work
where the diagnostic service is an active player6.

proceso asignando los porcentajes publicados de
ausencia de retorno de la inversión en salud.
La producción de un laboratorio puede
clasificarse como:
Información coste-efectiva: genera cambios
positivos para el paciente o disminuye la
incertidumbre sobre su patología.
Actividad: no genera cambios en el paciente y
no mejora la incertidumbre. Hablamos de despilfarro
(eficiencia inefectiva). Este tipo de actividad
representa entre el 20 y el 30% del total7,8. En estos
casos, el laboratorio no realiza adecuación de la
demanda; se limita a realizar todo lo que se le solicita.

The laboratory production can be classified as:
Cost-effective information: The laboratory
generates positive changes for the patient or reduces
the uncertainty about his pathology.
Activity that does not generate any changes in
the patient and does not reduce the uncertainty. In
other words, it is a waste (ineffective efficiency). This
happens when the laboratory does not perform the
advisable tests for each case but everything
requested. This type of activity represents 20 to 30%
of the total7,8.
Iatrogenic results: the laboratory generates
negative changes for the patient because
- Data is not available when needed (15-20%)9
due to:
Turnaround time issues, insufficient,
wrong or unclaimed samples.
The laboratory does not perform further
tests (unsolicited) that turn out to be
necessary once the results that appear
during the analysis process are evaluated.
- The clinicians provide a misinterpretation (false
positives and false negatives) in 30% of the
cases10. This errors are made because the
laboratory does not provide an suitable
interpretation or it is confusing.
The sum is not algebraic. The results that do not add
value overlap in some cases. Useless data can be
both unavailable and misinterpreted.
