The Central Coast Council Subsidised Desexing Program will run for the period 1 July 2018 to 30 June

To be eligible for a voucher:

1. Your animal must be identified (by way of a microchip) in accordance with s.8 of the Companion Animals

Act 1998. Information about microchipping your pet is available from the Council's website.

2. You must be the owner of the animal.

3. You must be a resident of the Central Coast local government area.

4. You must be in receipt of a pension or other entitlements from Centrelink and/ or the Department of

Veterans' Affairs. Concession cards are required to be sighted on receipt of application.

Making an application:

5. All applications must be submitted at one of Council's Customer Service Centres located at 49 Mann

Street Gosford or 2 Hely Street Wyong.

6. Applications can be made during 1 July 2018 to 30 June 2019. No applications made outside of this time

will be accepted.

7. Council has absolute discretion and reserves all rights to verify the validity of applications.

8. You may apply for vouchers for up to two (2) animals for which you are the registered owner. Separate

applications have to be submitted for each animal.

9. Approval of applications is at the absolute discretion of Council. Successful applicants will be notified via

email or in writing by the Council.

How to use the voucher:

10. If your application is successful, and you receive a voucher, the voucher can only be used for desexing

services at an approved veterinary practice during the period 1 July 2018 to 30 June 2019. Details of

approved veterinary practices are available on the back of this form, or the Council's website.

11. A voucher is not redeemable for cash, and is not transferrable. Desexing vouchers are for the following

monetary values:

- Desexing voucher for a female dog, $180;

- Desexing voucher for a male dog, $150; and

- Desexing voucher for a cat, $70.

12. The veterinary practice you go to will discount the cost of the desexing service you obtain, by the

monetary value of the voucher. The voucher does not cover the full cost of the desexing services you

obtain.

13. The Council has no obligations or liabilities in relation to your use of the voucher, or the provision of any

desexing services to you by an approved veterinary practice. You agree that the Council is not liable for

any claims, losses, damages, penalties or demands for any loss resulting from the use of the voucher, or

any desexing services you obtain.

14. The Council reserves its rights to terminate the program at any time, and without notice. Any vouchers

not used during the term of the program are forfeited. The term of the program is from 1 July 2018 to

30 June 2019, unless terminated by the Council prior to that time.
