1. The Entry form opens and prompts for Name of Patient and Mobile of Patient (Self/Family) for OTP based authentication. Skip OTP option is available (configurable)

2. System checks for repeated use of same patient mobile number3. Location details of patient are captured and Form can be saved at this stage for data entry in Lab later, if desired3. Complete form can also be filled.

1. Complete A2 section and move to A3 Section using Next button. Aadhaar and Pin code are optional. If entered, must be correct. Address field accepts alpha-numeric content

2. Cancel will take you to home screenand save record, which may be completed later

1. Complete A.3.1 (Community) or A.3.2 (Hospital) section.

2. Move to B1 section for filling it up.

1. B2 and B3 sections may be completed. In case of hospitalization, hospital details will be required.

2. Test Type to be Selected from RT-PCR or Rapid Antigen

3. In case of Rapid Antigen Test, Kit used and Test result to be entered

1. Select Lab

2. Your State/District Labs will come on top. But you may search by typing name of laboratory also. Will get Refresh Lab button in case of new labs are added.

3. A pop-up box will give all details of the SRF form with option to Edit or Continue

1. If you tap continue, you are shown the Preview of the SRF form with filled data.

2. You may Submit the record, Edit it, Delete it or Save it on local machine for future editing or submitting to server.

1. Saved/ Incomplete forms are shown in your App.

2. You need to complete (after reviewing, editing etc) and Finally submit these to server by Taping on appropriate button.

1. For repeat test, either enter Patient ID available with your or enter 1 or more mobile numbers of patient

2. Tap the patient for who repeat test is being done

3. Only some fields can be entered in this case.

1. SRF Form PDF file can also be seen by Tapping on appropriate option in the Pop-up of selected Form

2. You may download multiple SRF forms in Excel/ PDF format from Portal too
