When you see an in‐network (Delta Dental PPO™/Delta Dental Premier®) dentist:

• Network savings on covered services

• Your annual benefit dollars go further when you see an in‐network dentist

• Dentists submit claims directly to Delta Dental

• Delta Dental sends plan payment directly to dentist

Out‐of‐Network Coverage

If you see an out‐of‐network dentist:

• If the dentist’s fees are higher than Delta Dental’s allowable charge, you may be responsible to pay the difference • Your dentist may assist you in submitting a claim form to Delta Dental • Benefit payments go directly to you

Transition of Care What happens if I (or a dependent) currently have work‐in‐progress?

• Work on major restorative services (crowns, bridges, dentures) started prior to your effective date will be paid based on the service completion date.• Example‐You are in the process of getting a crown and have an appointment in June (CIGNA coverage).  The permanent crown is placed in July (Delta Dental coverage).    All steps of the procedure will be considered a claim under Delta Dental, even the appointment in June.

What happens if a dependent currently has orthodontic work‐in‐progress?

• The orthodontist will need to file a new claim with Delta Dental.• Claim will be prorated depending upon where member is in treatment plan.

• Example‐Your child is 12 months into their 24 month treatment and payment plan.  There is a number of factors to determine the coverage under Delta Dental.  Please call Delta Dental Customer Service (800‐448‐3815) as they can review your specific situation.

ID Cards

• You will receive a new ID Card in the mail. Please provide your dental office with your new group number at your next appointment.• Group # 100278

–An alternate ID number will be assigned to each employee and will appear on the ID Card–All ID cards will have the employee’s name printed on the card–Dependents are not issued personalized cards; they will use the card with the employee’s name on it
