December 28, 2016 Project No. C1192 Bridger Smith [email protected] Onsite Wastewater Treatment System (OWTS) Installation Observations 4110 CR 243 Garfield County, Colorado Permit Number SEPT-07-16-4345 Bridger, ALL SERVICE septic, LLC observed the installation of the onsite wastewater treatment system (OWTS) on October 3, 2016 for the subject property. Teagle Excavating installed the system. ALL SERVICE septic, LLC also confirmed the soils on September 15, 2016. A soil profile test pit was excavated and materials encountered in the Test Pit consisted of light to medium brown loam to silt loam to a maximum depth explored of 8-feet. No bedrock or groundwater was encountered. A sample from approximately 3-feet below grade formed a ball and a ribbon less than 1-inch in length before breaking. The texture was a little more smooth than gritty. The soil structure shape was blocky, soil structure grade was moderate, and consistence was friable. STA sizing is based on Soil Type 2, Loam to Silt Loam with moderate structure grade. A long term acceptance rate (LTAR) of 0.6 gallons per square foot will be used to design the OWTS, in accordance with Table 10-1 Soil Type 1 presented in the Garfield County On-Site Wastewater Treatment System Regulations, adopted April 14, 2014. Test pit The verification of the soils allowed a slight reduction in the required square footage from 420 square feet to 350 square feet. A ditch that ran to the west of the soil treatment area (STA) was re-aligned and is located greater than 50-feet from the STA. The system installation included a new 1000-gallon, two-compartment Valley Precast concrete septic tank with an effluent filter installed on the outlet tee of the septic tank. This filter must be cleaned annually, or as needed. Effluent gravity flows to a distribution box, which is accessible from grade. The STA consists of three trenches, each with 10 ‘Quick 4’ Infiltrator® chambers for a total of 30 ‘Quick 4’ Infiltrator® chambers and 360 square feet of infiltrative area. Trenches have a minimum of 6-feet of undisturbed soil between them. Inspection ports were installed at both the beginning and end of each trench. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. Do: • Consult a septic service professional to

advise you of the proper distance for planting trees and shrubs, depending on your septic tank location.

Do: • Stagger the use of water-generating

appliances. This can be helpful especially if your system has not been pumped in a long time.

• Become more water efficient by fixing plumbing leaks and consider installing

bathroom and kitchen faucet aerators and water-efficient products.
