His/her primary responsibilities are to provide clinical evidence based services and interventions; program development; Veteran and staff education on homeless issues; community outreach; resource development; continuous quality improvement activities; and evaluation/consultation. In addition, the incumbent must be a skilled communicator capable of developing effective partnerships with local, state, and federal agencies as well as non-governmental organizations on behalf of primary and homeless care. The incumbent is administratively responsible for the clinical programming and daily operations of the homeless program. S/he may develop policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use. The incumbent is a clinical social worker providing direct outpatient psychosocial health services to patients and families, consultation to other services and/or agencies and to the community. The clinician provides education of patients, families, clinical, and administrative staff on a wide range of psycho-social interventions, disorders, treatments, and programs. The incumbent manages and controls the use of data related to the H-PACT program regarding day-to-day functioning and program evaluation. In conjunction with the Service Chief, the incumbent plans, organizes, and directs administrative, operational support for Veteran and personnel activities for the service. The Homeless PACT (H-PACT) Coordinator has experience that demonstrates the advanced practice skill level in social work.
