Thai same day BSEE granted verbal approval (enclosed) to Arena to make the emergency repairs.

Riser Repair Pipeline Segment No. 18543/ROW No. G29120

October 7. 2016: Arrived on location. Operators isolated Pipeline SN 17054 at EI 217 B Platform and SN 18543 at EI 215 B-PROD by locking close two block valves and installing a blind flange at EI 215 B Platform. Bled off pressure and verified pipelines were at 0 psig. Connected one end. of hose to tie-in point upstream of isolation point on the pipeline at EI 215 B and routed the other end to a stock tank.

October 8, 2016: Continued draining pipeline and removing the piping at/below the Cellar Deck.

October 9, 2016: Once pipeline linished draining, inserted hose into pipeline and pumped down approximately 336 gallons of liquid and allowed to stabilize. Removed piping above the flange and disconnected the flange at +15' elevation.

October 10, 2016: Secured top of riser from Main Deck and cold cut riser approximately 10' above waterline. Removed damaged section of riser. Inserted smart vent plug into riser approximately 8' above waterline, engaged smart vent plug, added two gallons of water on top of plug and checked riser for hydrocarbon presence. Piped smart vent piping/hose to safe location away from all hot work. Replaced damaged riser section with new riser and welded out. Note: new riser section was hydrotested onshore for 8 hrs at 2220 psig and 100% of welds were x-rayed onshore and offshore. Disengaged smart vent plug and removed from riser piping. Torqued upper flange connection.

October 11, 2016: Installed piping from +10 elevation to the top ofthe Cellar Deck.

October 12, 2016: Continued installing piping from +10 elevation to the top of the Cellar Deck.

October 13, 2016: Opened valves, at both ends of Pipeline SNs 18543/17054 and performed visual leak test for two hours with product above operating pressure (500 psig operating pressure, 587 psig actual test pressure); used calibrated crystal gauge with report. Placed Segment No. 18543 back in service.

Arena noticed seeping on a riser at El 215 when at operating pressure, No sheen or actual leak of hydrocarbons occurred and the pipeline has been shut in. Attached is the repair procedure. My understanding is that we can start these emergency repairs now and submit the formal application after the repairs are complete. The pipeline wi I not be brought back on line until the required testing requirements (addressed in the procedure) are performed.

Arena plans on starting these operations at first light tomorrow morning (10/02/2016).

Per my conversation with Beau Boudreaux this afternoon, he will take a look at this procedure and pass it along to the Production Operations Supervisor for any comments.

*This is sort of confusing because the 6" for repair comes from El 2178 via a 4" departing line at El 217B.

6) PROPOSED PROCEDURE

a Brief description of the method to be used Pipeline Segment 18543 / Bulk Oil Pipeline from El 217 " B " / 4" and 6" MAOP 1100 PSIG / KAQ-4000 Pipeline Segment 18543 normal operating pressure is 500 psig.

1) Operators to isolate pipeline segment No. 17054 at El 217 " B " and segment No. 18543 at

El 215 "B-Prod" by locking closed two block valves and/or installing skillets.

2) Operators to bledd of pressure and verify the pipelines are at zero psig.

3) Operators connect one end of hose to tie-in point upstream of isolation point on the pipeline, and route the other end to a stock tank.

4) Open up tle-ln valve and allow pipeline to drain to stock tank.

5) Once pipeline h.>s finished draining, insert hose into pipeline and pump down £Pi!roxlmately 100 gals of liquid (80ft.) and allow to stabliize, Monitor fluid level after pumping and repeat pump off if necessary until the level stabilizes.

7) Insert smart vert plug into riser to an elevation below the cut point (approximately ten feet above waterline), engage smart vent plug, add two gals of water on top of plug, and check riser for hydrocarbon presence. Repeat flush as necessary until hydrocarbon free.

8) Pipe smart vent piping/hose to a safe location away from all hot work.

9) Secure top of rUer from main deck, and cold cut riser approximately ten feet above

waterline and above smart plug.

10) Remove damaged section of riser.

11) Replace dama jed riser section with new riser and weld out. 'New riser sectior is 6.625"OD x .432" wt sch 80 Gr. X42 pipe and will be hydrotested for

4 hours (1.25xMAOP) and welds X-Ray tested prior to, and folllowing installation.

12) Disengage thc smart vent plug and remove from riser piping.

13) Torque upper flange connection to recommended Ib-ft.

14) Construction to open valves at both ends of pipeline segment No. 18543/17054 and perform visual lesk test for two hours with product at operating pressure (500pslg). A

calibrated digital feadweight gauge and chart recorder shall also be used during thc leak test­

i s ) Upon completion of a successful leak test recorded on chart recorder, pipeline segment No. 18543 shall be placed back into service.

I emailed Aimee Deady earlier today hadn't heard back, so I was hoping maybe you could help with this. I'm processing the completion report for the repair that was done on PSN 18543, and I just want to verify the elevations of the replaced pipe. The plan submitted on 10/1/16 indicated the riser would be replaced from the +10" to the +20' elevation. In the actual repair procedure it states that pipe was replaced from the +10" to the top of the cellar deck. Is the top of the cellar deck at +20'? There is also mention of disconnecting a flange at the +15', so I just want to make sure we get it straight.
