Each time you turn on your meter, the unit of measure will appear onthe display with the R symbol. The unit of measure will also appearunder every test result.

You may misunderstand your blood glucose results if your meter is set tothe incorrect unit of measure. Your meter’s unit of measure should be setto mg/dL.

If your meter is set to the correct unit ofmeasure, “mg/dL” will appear on the display.Your test result will not include a decimalpoint (Fig. 1). Do Not change this setting –this is the correct unit of measure for theUnited States.

If your meter is set to the other unit of meas-ure, “mmol/L” will appear on the display. Thetest result will include a decimal point (Fig.2). Unless your doctor has told you to usethis unit of measure, Do Not test your bloodglucose. Call LifeScan Customer Serviceat 1 800 227-8862 immediately forassistance.

Always confirm that the unit of measure is inmg/dL every time you test your blood glucose level. Use of the wrong unit ofmeasure may cause you to misinterpret yourblood glucose level, and may lead to incorrect treatment.

About Your New System

The OneTouch® Ultra® System uses the latest blood glucosemonitoring technology. It measures the glucose content of ablood sample by means of an electrical current produced in the teststrip and measured by the meter. Test results are “plasma-calibrated.” This makes it easier for youand your diabetes-care team to compare yourmeter results with laboratory tests. Before using any product totest your blood glucose, read

all instructions and practice the test. Do allquality control checks as directed and consultwith a diabetes healthcare professional. Theserecommendations apply to all blood glucosemonitoring systems and are supported by theAmerican Association of Diabetes Educators,the American Diabetes Association, the U.S.Food and Drug Administration, and theAdvanced Medical Technology Association.

CAUTION:

The OneTouch® Ultra® System is intended foruse outside the body (in vitro diagnosticuse). It should be used only for testing freshcapillary whole blood samples for glucose(sugar). It should not be used for the diag-nosis of diabetes or for testing newborns.

◗ Dehydration:Severe dehydration resulting from excessive water loss may cause false lowresults. If you believe you are suffering from severe dehydration, consult ahealthcare professional immediately.

◗ Low glucose results:If your test result is lower than 70 mg/dL or is shown as LO, it may meanhypoglycemia (low blood glucose). This may require immediate treat-ment according to your healthcare professional's recommendations.Although this result could be due to a test error, it is safer to treat first, andthen repeat the test.

◗ High glucose results:If your test result is greater than 180 mg/dL or is shown as HI, it may meanhyperglycemia (high blood glucose). If you do not have symptoms, firstrepeat the test. Your healthcare professional can work with you to decidewhat actions, if any, you should take if you continue to get results higher than180 mg/dL or if you have symptoms.

◗ Repeated unexpected results:If you continue to get unexpected results, check your system with controlsolution. See Checking the System with Control Solution on pages 13–19. If you are experiencing symptoms that are not consistent with your blood glu-cose results and you have followed all instructions in this booklet, call yourhealthcare professional. Never ignore symptoms or make significant changesto your diabetes control program without speaking to your healthcare profes-sional.

◗ Hematocrit:A hematocrit (percentage of your blood that is red blood cells) that is eithervery high (above 55%) or very low (below 30%) can cause false results.

WARNING: Keep the meter and testing supplies away from smallchildren. The battery door, test strips, lancets, protective disks, andcontrol solution cap are choking hazards.

◗ Store test strip vials in a cool, dry place below 86˚F (30˚C). Keep away fromdirect sunlight and heat. Do Not refrigerate.

◗ Store test strips in their original vial only. To avoid damage or contamina-tion, Do Not transfer test strips to any other place.

◗ After removing a test strip from the vial, replace the vial cap immediately andclose it tightly.

◗ Use each test strip immediately after removing it from the vial.◗ Do Not use test strips from any vial that is damaged or left open to air.◗ Write the discard date (3 months after first opening the vial) on the vial label

when you first open it. Discard remaining OneTouch® Ultra® Test Strips afterthe discard date.

◗ Do Not use test strips beyond the expiration (printed on package) or discarddate, whichever comes first, because they may cause inaccurate results.

◗ Avoid getting dirt, food or liquids on the test strip. With clean, dry hands, youmay touch the test strip anywhere on its surface.

◗ Do Not bend, cut, or alter a OneTouch® Ultra® Test Strip in any way. ◗ OneTouch® Ultra® Test Strips are for single use only. Never reuse a test

strip that has had either blood or control solution applied to it.◗ Apply only OneTouch® Ultra® Control Solution or a blood sample to the test

strip.◗ Refer to additional information in the OneTouch® Ultra® Test Strip package.

Each time you turn on the OneTouch® Ultra®

Meter either by inserting a test strip or pressing the button, all segments of thedisplay will appear briefly. This tells you that the system is performing several self-checks toconfirm that the meter is working properly.

To check that all display segments are working, with the meter off, press thebutton and immediately hold the button down.

CAUTION: Matching the code on the meter and the code on the teststrip vial is essential to obtain accurate results. Each time you test,check to make sure the code numbers match.

Coding the Meter

Code numbers are used to calibrate the OneTouch® Ultra® Meter with OneTouch® Ultra® Test Strips for accurate results. You must code the meterbefore using it for the first time and then every time you change to anothervial of test strips. Each time you test, check to be sure that the codenumber on the meter display matches the code number on thetest strip vial.

STEP 1Enter theCode Mode.Insert a test stripto turn on themeter. Push it allthe way in until it will go no fur-ther. Avoidbending the test strip. The display check willappear. Then the code number is displayed forthree seconds. The first time you use the meter, threedashes Q will flash, meaning that there is nocode stored in the memory. Follow Step 3 tocode the meter. If three dashes appear anyother time, see page 50 of Display Messagesand Problem-Solving Guide.

STEP 2Match the Code Numbers.Compare the code number on the meterdisplay with the code number on the test strip vial. If the two code numbers match, you may begin testing. If they do not match,follow Step 3.CAUTION: Matching the code on the meter and the code on the test strip vial is essential to obtain accurate results. Each time you test,check to make sure the code numbers match.

STEP 3Code the Meter.Immediately press the button, while thecode number is displayed on meter to select thecorrect code. Each time you press and releasethe button, the number will increase byone. To move more quickly, press and hold the

button.

Note: If the apply blood symbol is displayedbefore the meter is coded correctly, remove thetest strip and repeat steps 1 through 3.Note: If you scrolled past the desired codenumber, continue to press the button untilthe correct code number appears. After selecting the correct code number, it willflash for three seconds and then appear solidfor three seconds. CAUTION: Matching the code on the meterand the code on the test strip vial is essential toobtain accurate results. Each time you test,check to make sure the code numbers match. Then the symbol will appear with mg/dL,indicating that the OneTouch® Ultra® System isready for testing. WARNING: If mg/dL does not appear withthe symbol, call LifeScan CustomerService at 1 800 227-8862.

When you should do a control solution test:

◗ To practice the test process instead of using blood.

◗ When you open a new vial of test strips.

◗ Once a week.

◗ Whenever you suspect that the meter or test strips are not working properly.

◗ If you have had repeated unexpected blood glucose test results (as described on page 3).

◗ After dropping the meter.

Why you should do a control solution test:

◗ OneTouch® Ultra® Control Solution is used to check that the meter and the test strips are working together as a system and that you are performing the test correctly.

◗ It is very important that you do this simple check routinely to make sure you get an accurate result.

Before you begin:

◗ Use only OneTouch® Ultra® Control Solution.

◗ Check the expiration date on the control solution vial. Record the discard date (date opened plus three months) on the vial label. Do Not use after expiration or discard date, whichever comes first.

◗ Control solution, meter, and test strips should be at room temperature(68–77˚F/20–25˚C) before testing with control solution (seeSpecifications section on page 60 for blood testing temperature ranges).

◗ Shake the vial, discard the first drop of control solution, and wipe offthe tip to ensure a good sample and an accurate result.

◗ Store control solution tightly closed at temperatures below 86˚F(30˚C). Do Not refrigerate.

WARNING:◗ Do Not swallow control solution; it is not for human

consumption.◗ Do Not apply control solution to the skin or eyes as it may

cause irritation.

Then the code number will appear,followed by the R symbol andmg/dL. Be sure the meter and teststrip codes match. If they do not,code the meter correctly. (Seepages 10–12.)

To ensure an accurate control solution test, shake the vial, discard thefirst drop and wipe the dispenser tip before you begin.

The control solution test result will appear on the display. Ë will appear above your result.

Touch and hold a drop of control solution where the narrowchannel meets the TOP EDGE of the test strip.

The control solution will be drawn into the narrow channel. When theconfirmation window is full, themeter will count down from 5 to 1.

If the results are not within the control range printed on the test stripvial, the meter and strips may not be working properly. Repeat the control solution test.

The OneTouch® Ultra® System requires a very small blood drop to per-form a test. You may obtain it from a fingertip or forearm. (Seepage 25 for information on obtaining a blood sample from the forearm.)Choose a different puncture site each time you test. Repeated punctures inthe same spot may cause soreness and calluses.

Proceed with your blood glucose test.

Forearm Blood Sampling

The forearm has fewer nerve endings than the fingertip so you may findthat obtaining a blood sample from the forearm may be much lesspainful than using the fingertip. The technique for forearm sampling isdifferent from fingertip sampling. Also there are differences betweenforearm samples and fingertip samples that you should understand.Please read the important information on page 27 carefully.

STEP 1Install the OneTouch® UltraClear®

Cap.To aid in obtaining a blood sample fromthe forearm, replace the OneTouch®

UltraSoft® Cap on the Sampler with theOneTouch® UltraClear® Cap. If necessary,set the Sampler for a deeper puncture.

STEP 2Choose the Puncture Site.Select a soft, fleshy area on your forearm thatis clean and dry, away from bone, and free ofvisible veins and hair.

STEP 5Allow Blood Drop to Form.Continue holding the Sampler against theskin for a few seconds until the blood dropforms. Allow enough blood to form under theOneTouch® UltraClear® Cap until you have ablood sample ( actual size) that is sufficientto fill the confirmation window of the test strip.If you must massage the area to obtain more blood, Do Not squeeze thesite excessively.

Important information about forearm testing◗ Under certain conditions, blood glucose test results obtained using

samples taken from your forearm may differ significantly from fingertip samples.

◗ The conditions in which these differences are more likely to occur are whenyour blood glucose is changing rapidly such as following a meal, an insulin dose,or physical exercise.

◗ When blood glucose is changing rapidly, fingertip samples show these changesmore quickly than forearm samples.

◗ When your blood glucose is falling, testing with a fingertip sample may identify ahypoglycemic (low blood sugar) level sooner than a test with a forearm sample.

◗ Use forearm samples only for testing prior to, or more than two hours aftermeals, insulin doses, or physical exercise.

◗ Testing performed within two hours after meals, insulin doses, or physical exer-cise, or whenever you feel that your glucose levels may be changing rapidly,should be done with a fingertip sample.

◗ You should also use fingertip testing whenever you have a concern about hypo-glycemia (insulin reactions) such as when driving a car, particularly if you sufferfrom hypoglycemic unawareness (lack of symptoms to indicate an insulin reac-tion), as forearm testing may fail to detect hypoglycemia.

Step-by-Step Test Procedure

STEP 1Insert Test Strip.Insert a test strip, contact bars end firstand facing up, into the test port. Push itin until it will go no further. The meterwill turn on and the display check willappear briefly. Then the code number willappear, followed by the symbol withmg/dL. Check to make sure the codenumber on the meter display matches thecode number on the test strip vial.Always confirm that the unit ofmeasure is mg/dL when you testyour blood glucose level.

WARNING: If mg/dL does not appearwith the symbol, call LifeScanCustomer Service at 1 800 227-8862. Use of the wrong unit of measure may cause you to misinterpret your blood glucose level, and may lead to incorrect treatment.Be sure the meter and test strip codes match. STEP 3Accurate Results in Just 5 Seconds.Your blood glucose test result will appearafter the meter counts down from 5 to 1.Blood glucose test results are automaticallystored in the meter memory. You may alsorecord them in the logbook provided withyour system. Turn the meter off by removingthe test strip. Dispose of used test strip in asealed container.

WARNING: If mg/dL does not appear with the test result, callLifeScan Customer Service at 1 800 227-8862.

CAUTION: If you tested at the low end of the operating range (43°F/ 6°C)and your glucose is high (over 180 mg/dL), the reading on your meter maybe lower than your actual glucose. In this situation, repeat the test in awarmer environment with a new test strip as soon as possible.

Discarding used test strips and lancets

It is important to discard the used lancet carefully after each use to avoidunintended lancet stick injuries.

Used test strips and lancets may be considered biohazardous waste inyour area. Be sure to follow your local regulations for proper disposal.

This may mean severehypoglycemia (low blood glucose) thatrequires immediate treatment according toyour healthcare professional’s recommen-dations. Although this message could be due to atest error, it is safer to treat first, and then repeatthe test.

If your blood glucose test result isabove 600 mg/dL, h1 will appearon the meter display. This maymean severe hyperglycemia(high blood glucose). Youshould recheck your bloodglucose level. If h1 againappears on the display, callyour healthcare professionalimmediately.

When your blood glucose test result isabove 240 mg/dL or reads h1,“Ç” will appear on the meterdisplay. This message does not meanthat the system detected ketones butthat testing with a ketone test stripmay be advisable. Consult yourhealthcare professional about whenand how to test for ketones. Using the Meter Memory

Your OneTouch® Ultra® Meter stores the 150 most recent blood glucose andcontrol solution test results with date and time in its memory. It also providesyou with 14- and 30-day averages of your blood glucose test results. If your14-day and 30-day test result averages are not what you expect, you shouldnot rely on them. Review the test results in memory with these easy steps.

STEP 1Enter the Memory Mode.You may enter the memory mode either withthe meter turned off or immediately aftercompleting a test. Press the button. ˘and the 14-day average will appear with thenumber of blood glucose tests performed inthis period (e.g. 64n means that 64 tests wereperformed within the period). After three seconds, the 30-day average will appear. (If you are using the meter for the first time, Q indicatesthere are no test results in memory.)

These averages are calculated from the bloodglucose results obtained during the last 14 and30 calendar days. A h1 result will be included inyour averages as 600 mg/dL and a lo result as 20mg/dL. Results marked as control solution testswill not be included in your averages. If your 14-day and 30-day test result averages are notwhat you expect, you should not rely on them.

Comparing Meter andLaboratory Results

OneTouch® Ultra® Meter test results and laboratory test results are bothexpressed in plasma-equivalent units. However, your meter result may differsomewhat from your laboratory result due to normal variation. Meterresults can be affected by factors and conditions that do not affect laboratoryresults in the same way. Your OneTouch® Ultra® Meter glucose valueshould agree with a laboratory measurement within ±20% most of thetime under normal conditions. A result within that range is consideredaccurate when testing with the OneTouch® Ultra® Meter. However, resultscan differ by more than ±20% in some situations. See the OneTouch®

Ultra® Test Strip package insert for typical accuracy and precision data andfor important information on Limitations of Procedure.

To make an accurate comparison between meter and laboratory results,follow these guidelines:

Before going to the lab:

◗ Perform a control solution test to make sure the meter is workingproperly.

◗ It is best to fast for at least eight hours before doing comparison tests.

◗ Take your meter with you to the lab.

While at the lab:

◗ Make sure that the meter test and the lab test are performed within 15 minutes of each other.

◗ Wash your hands before obtaining a blood sample.

◗ Use only fresh capillary blood obtained from the fingertip.

You may still have a variation from the laboratory result because bloodglucose levels can change significantly over short periods, especially if youhave recently eaten, exercised, taken medication, or experienced stress.1

In addition, if you have eaten recently, the blood glucose level from a fingerstick can be up to 70 mg/dL higher than blood drawn from avein (venous sample) used for a lab test.2 Therefore, it is best to fastfor eight hours before doing comparison tests. Factors such as the per-centage of red blood cells in the blood (a high or low hematocrit) orthe loss of body fluid (severe dehydration) may also cause a meter resultto be different from a laboratory result.

Caring for Your OneTouch® Ultra®

System

Meter

Your OneTouch® Ultra® Meter does not require special maintenance orcleaning. Take care to avoid getting dirt, dust, blood, control solution,or liquids inside the meter through the test port or data port. LifeScan recommends that you store the meter in its carrying case after each use.

A cloth dampened with water and mild detergent can be used to wipedown the outside of the meter. Your OneTouch® Ultra® Meter is a precision instrument. Please handle it with care.

Do Not use alcohol or any other solvent to clean your meter.

OneTouch® UltraSoft® Adjustable Blood Sampler

Clean the Sampler and Caps (OneTouch® UltraSoft® and OneTouch®

UltraClear®) with soap and water. To disinfect the Sampler, prepare a dis-infectant solution of one part household bleach to 10 parts water. Dampena cloth with this solution and wipe the Sampler thoroughly. Soak onlythe cap or caps for at least 30 minutes in the disinfectant solution. DoNot soak the Sampler in liquid. Rinse the Sampler and caps with waterand dry thoroughly.

Battery

Your OneTouch® Ultra® Meter comes with a pre-installed 3.0 V (#2032 orequivalent) lithium battery. The battery will provide you with enoughpower to perform about 1,000 tests. When replacing the battery, use only a 3.0 V (#2032 or equivalent) lithium battery.

The meter will alert you when the power is getting low by displaying twodifferent messages:

The symbol appears on the displaywith mg/dL when the meter is turned onand all other display messages are func-tional. From the time the symbolfirst appears, there is enough power leftfor about 50 tests. The test results willbe accurate, but it is time to change thebattery.

STEP 2Remove the Old Battery.Pull upward on the clear plastic tab. Insertone 3.0 V (#2032 or equivalent) lithium bat-tery, making sure the positive “+” side of thebattery is facing up.

STEP 3Place the Cover.Place the cover over the battery compartment.Push down until you hear the tab click intoplace.

STEP 4Check the Time, Date, and Unit ofMeasure.After replacing the battery, turn the meter on by inserting a test strip orpressing the button. If the meter is in setting mode, confirm that thetime, time format and date are set correctly. If they are not, use the and buttons to reset the meter before testing. See Meter Set-up, pages40–45. Also, be sure to confirm the unit of measure is mg/dL.
