    DEFINITION

    Number of open on-line corrective maintenance work orders (WOs) as defined by INPO AP-928, WorkManagement Process Description is the repair and restoration of power production related equipment,components, or structures that have failed or are malfunctioning and are not performing their Intendedfunction.

    During the week ending 2122104, 7 new corrective orders were initiated and 2 completed. Theorganizational focus remains on emergent work and preventive maintenance activities. The BacklogReduction Plan Is being implemented starting January 2004 to support achieving Corrective MaintenanceBacklog goals within the Operational Improvement Plan.

    DEFINITIONThis Indicator tracks the closeout of all open Temporary Modifications identified before Mode 2.

    GOAL AT STARTUPZero Temporary Modifications

    ANALYSIS! SUMMARYSignificant progress has been made in eliminating Temporary Modifications. Since July 2003, 20 TemporaryModifications have been eliminated leaving the I described below, which will remain open until shortly afterrestart.

    Temporary Modification 03-0028 will remain installed until after restart and will be removed shorty after plantstartup during the next scheduled Auxiliary Boller Outage. This Temporary Modification is for Auxiliary BoilerFeedwater Control Valve AS1678 which was reassembled without a pusher plate and handwheel. AnAuxiliary Boiler outage is required to repair the valve and remove this Temporary Modification. This work cannot be completed prior to restart due to the need for station heating and establishing a vacuum. Once theStation is on line, the auxiliary boiler can be removed from service to repair AS1678 and remove TM 03-0028.

    DEFINMONThis Indicator measures the number of Category / Priority I & 2 Procedure Change Requests (PCRs) whichare awaiting Incorporation Into Davis-Besse procedures. The term PCR Includes change requests Initiated asCondition Reports. The above count does not include category / priority 3 PCRs as these PCRs areenhancements not affecting procedure adequacy or quality, and categorylpriority 4 PCRs as these PCRs arerestrained by plant modifications, license amendment requests, etc. and cannot be closed until theserestraints are completed. A PCR Is considered completed when It Is Incorporated Into an effective procedureor when the PCR Is cancelled.

    DEFINITIONThis indicator measures the percent of Condition Report (CR) Senior Reactor Operator (SRO) reviewscompleted within one day after the review by the originators supervision to ensure timely review of plantconfiguration and operability concerns. The SRO reviews requested by the Management Review Board(MRB) are excluded from this indicator.

    GOAL AT STARTUPRestart goal Is for 95% or more of the SRO reviews to be completed within one day.

    ANALYSIS/ SUMMARYNinety-four percent (50/53) of the SRO reviews were completed within one day after the review by theoriginator's supervision with the remaining three SRO reviews having a duration of up to three days.The decrease can be contributed to resources focusing on plant start-up activities, training activitiesand operating crews benchmarking at Perry and Beaver Valley. Major plant activities have beencompleted and this trend Is expected to improve. The 12-week average (1211/03 - 2/22104) 1s 87percent of the SRO reviews completed within one day.

    This performance Indicator has been adjusted through week ending 218/04 to remove the timeOperations staff were performing peer reviews of the Initial SRO Condition Report review. Thepurpose of the indicator Is to measure the timeliness of SRO reviews of potential plant concerns. ThisIs accomplished on the initial SRO review. The performance Indicator was adjusted back to weekending 11/2/03.

    The restart goal is a decreasing trend of open CR corrective actions.

    ANALYSIS/ SUMMARYThe trend of open CR corrective actions has Increased three percent over the last nine weeks. The 8,597 opencorrective actions consist of 3,199 (37.2%) - NCAQ (Conditions Not Adverse to Quality), 5,062 (68.9%) - CAQ(Conditions Adverse to Quality), and 336 (3.9%) - SCAQ (Significant Conditions Adverse to Quality). Theorganizations with the highest number of open CR corrective actions are:

    Plant Engineering - 2,017 open (decreased by three percent)Design Engineering - 1,562 open (increased by less than one percent)Outage Management & Work Control - 1,247 open (increased by two percent)Maintenance - 753 open (no change)Operations - 713 open (decreased by one percent)Training - 626 open (decreased by two percent)

    DEFINITIONThis Indicator reviews the population of Condition Reports (CRs) originated and calculates thepercent self-Identified compared to the total. CRs originated as a result of QA/INPO/NRCInput/feedback or CRs, originated as a result of a self-revealed Issue are not considered self-Identified. The CR Senior Reactor Operator (SRO) review date Is used as the starting point fordetermining the self-Identified rate.

    This Indicator assesses the Davis-Besse organization's ability to Internally identify conditions adverseto quality. A high value is representative of an organization that Is engaged and committed to theCorrective Action Process.

    GOAL AT STARTUPRestart goal Is for a twelve week rolling average of 85% or more of the CRs originated to beself-Identified.

    DEFINmONThe top line of this Indicator measures the percentage of Root Cause evaluations and other designateddocuments accepted (approved and approved with comments) by the Corrective Action Review Board (CARB).Root Cause evaluations are counted once regardless of subsequent changes or CARB acceptance after Initialrejection. The differential between the 12 week average lines for Root Cause evaluations accepted (approvedand approved with comments), and Root Cause evaluations approved Is an Indication of CARB participationand their added value to the Root Cause evaluations.

    GOAL AT STARTUPRestart goal Is for an Improving trend of the twelve week average, then a long-term goal of 9Wh or moreRoot Cause evaluations accepted by the CARB.

    ANALYSIS/ SUMMARY

    One CARB meeting was held during the week ending 2122104. No Root Cause reports were reviewed by theCARB. There are no Root Cause evaluations currently ready for CARB review. The 12-week average(1211/03 - 2122104) Is 78 percent of the Root Cause evaluations accepted by the CARB.

    The reduction to the 12-week average this week Is not a result of current performance, but the dropping off ofan approved Evaluation from the 12-week ave-rage. The two rejects In December 2003, caused the 12-weekrolling average to dip.

    Restart goal Is to maintain a Twelve Week Rolling Average level of 90% or better.

    ANALYSISI SUMMARYThe MRB changed the supervisory recommended category to a higher level on four percent (7/177) ofthe CRs. One category change resulted hi an increase from a condition adverse to quality (CAQ) to asignificant condition adverse to quality (SCAQ). The 12-week average (12/1103 - 2/22/04) of CRcategory recommendations not escalated by the MRB Is 94 percent.

Strengths and weaknesses result from the coaching opportunities.

    ANALYSIS/ SUMMARYTwelve Week Rolling Average Is 93%. 85% (11113) of the scheduled management observations werecompleted during the week ending February 22, 2004.

    The focus for management observations for the week ending February 22. 2004 was on use of EventFree Tools during various activities.

    Activities observed Included Just-In-Time training, housekeeping Inspections, Regulatory Affairs activities.backshift tours, MOV preventative maintenance, removal of scaffolding, and various pre-job briefs.

    A pre-job brief was conducted for Individuals who have never toured a nuclear plant, therefore, they werenot familiar with the process. The pre-Job brief included questioning attitudes of Individuals. Discussionsof the protected train were Included. A thorough brief was conducted which resulted In zero mRem for all.

    MOV activities Included a pre-Job brief and field activity observations. Strengths Included good anddetailed brief, In particular, emphasizing Tech Spec requirements and methods of control andcommunications. A weakness was identified In three part communication for directives, equipmentstaging, document verification, and coordination with support groups.

    Regulatory Affairs noted excellent use of self check I peer check and the application of correspondencecontrol process Including validation.
