REGISTRATION INFORMATION• All summer programs require CYS registration. Initial CYS

registration must be completed at Parent Central Services, building 1-4157 on Knox Street. Walk-ins welcome (Tuesdays and Thursdays 12–2 pm) or to lessen your wait time please schedule an appointment by calling 910-396-8110 or go online at webtrac.mwr.army.mil.

• Registered patrons can pay and enroll in SKIESUnlimited programs/academies/classes, enroll in youth sports, pay for monthly child care fees and make hourly care reservations online at webtrac.mwr.army.mil. Call 910-907-1824 to obtain your user ID and password.

• All payments are due at time of enrollment. All cancellations, transfers and refunds must be requested in person a minimum of three business days prior to the activity start date.

• All programs are subject to change.

REGISTRATION REQUIREMENTS• Physical/Health Assessment within 30 days of initial registration.• Family Care Plan for single/dual military within 30 days of initial

registration.• Parent’s work/home address and phone numbers.• Information for 2 local emergency contacts.• Proof of Eligibility (Active Duty Military/DoD sponsor or

dependent ID card).• Child’s Proof of Birthdate (Birth Certi�cate, DEERS enrollment form

or passport).• O�cial up-to-date Immunization Record (Children birth -

preschool).• Proof of Total Family Income (LES/pay stubs for spouse and
