Reflective Statement: During my first faculty position at St. Jude Children’s Research Hospital, my clinical research interests have included allogeneic hematopoietic stem cell transplantation for pediatric disorders including children with non-malignant diseases, such as osteopetrosis, and immunodeficiencies, such as Wiskott-Aldrich syndrome. Through treating children with malignant infantile osteopetrosis, I have been able to gain knowledge regarding this rare disease that has led to a treatment protocol using a reduced intensity conditioning regimen, an invitation to the First International Symposium on Osteopetrosis, published manuscripts, and most recently, the opportunity to present my research findings at the 2009 BMT Tandem meeting in Tampa, Florida. I also believe that supportive care plays a major role in outcomes after transplantation. Therefore, I have been the primary investigator for a clinical research study using palifermin in the pediatric setting. This protocol is continuing to enroll patients and will hopefully soon finish accrual. As a clinical investigator, I try to learn from my patients while providing the best medical care possible. I have been involved in several clinical research protocols involving partially matched family member transplant, for which I have been the primary investigator (as noted above). I believe that we should continue to strive to learn from our patients and encourage clinical research whenever possible, whether it be for therapeutic, supportive, or data collection purposes and using resources, internal and external, as available to participate in studies in order for the field of pediatric hematopoietic stem cell transplantation to advance

safely. Part of my role as a physician is to be an educator to my patients, families, and staff. Through this philosophy as noted through the lectures I have provided, I participate as a regular educator in the BMT courses at St. Jude for the nursing staff, I present a lecture regarding the basic concepts of hematopoietic stem cell transplantation to the Pediatric Oncology Education (POE) students each summer, and I have given formal and informal lectures and provided teaching didactics to the pediatric hematology/oncology fellows at St. Jude Children’s Research Hospital. During my clinical visits with my patients, I also provide education, whether it is their first meeting with me or an annual visit. Education provides the tools to make knowledgeable decisions. My vision for the future is to build a strong pediatric bone marrow transplant program at the University of North Carolina with the help of the staff currently present and utilizing the foundation that has been set. I believe that it will be important to also develop a strong working relationship with those involved in the collection and processing facilities. Engaging community physicians and other pediatric hematologist/oncologists will be important as we build, and educating the staff as changes occur to provide the best medical care possible with consistency will be important for all involved in the program. Building can begin as I learn the foundation and the team works together to provide excellent outcomes for pediatric patients undergoing hematopoietic stem cell transplantation at UNC.
