c. Population Management Curriculum, UTSW Family Medicine Residency Program. This

curriculum was developed for Delivery System Reform Incentive Payment (DSRIP)

program. The curriculum is a longitudinal experience designed to train Family Medicine

(FM) residents in applying the principles of the patient-centered medical home (PCMH),

quality improvement (QI) and mentoring with the ultimate goal of training FM physicians to

provide care for Medicaid and low income populations.
