November 30, 2017 4030 Lennane Drive Sacramento, CA 95834 CPhA Awards Committee, It is with great enthusiasm that we write this recommendation in support of Mr. Rajan Vaidya for the California Pharmacists Association Student Pharmacist of the Year award. Rajan is a student in the inaugural class at Keck Graduate Institute (KGI). He is currently an active member of CPhA-ASP and has been one since the inception of the student chapter at KGI. In fact, he was not only a founding member but was elected as the inaugural chapter president and guided the leadership team through the process of establishing the chapter. In his leadership role with CPhA-ASP, Rajan worked with the student leadership team and faculty advisors on creating the first student-lead health fair with the professional staff of the Claremont University Consortium (CUC). The event was open to graduate and undergraduate students and focused on preventative care, included free HIV screenings and information booths on mental health awareness. It was a collaborative effort with many KGI student professional associations and the CPhA-ASP was the lead organization. From this experience KGI also started a state-wide initiative that was a collaboration among all CPhA-ASP chapters. The CPhA-ASP Outreach Week was created to bring together all California Student Pharmacists as one voice to serve the community at large. Senator Jeff Stone introduced the resolution to the California State Senate and now February 12th through the 21st is California Pharmacists Outreach Week and KGI was listed as one of the participating schools in this resolution. The CUC health fair is now an annual and collaborative event and includes flu shot clinics in addition to health education. Rajan’s vision to work with other CPhA-ASP chapters was unexpected and rather impressive when he disclosed the launch of the CPhA-ASP Outreach Week and KGI being a part of the resolution. Rajan has not only been highly involved with CPhA-ASP, but also as the Inaugural Class President for the Class of 2018, as a member of the American Pharmacists Association Academy of Student Pharmacists (APhA-ASP) and Indian Pharmacists Association (IPA). As class president he worked with administration and his classmates to become involved in an event known as Eat & Be Well in Fontana. The event is held the day before Thanksgiving and assists the underserved population of San Bernardino County nourishment, social support services and health screenings. The inaugural class participated by providing free blood pressure screenings and diabetes education. Because of the contribution from the KGI students a relationship was created and this event has

Page 13: CPhA Student Pharmacist of the Year Nomination Form · CPhA Student Pharmacist of the Year Nomination Form ... and parenteral nutrition therapies Compounding TPN using EXACTAMIX Automated

Page 2

become an annual tradition for students to volunteer and give-back to the local community. Rajan led two of the most successful fundraising events as an APhA-ASP member to benefit the KGI Relay for Life Team, an event sponsored by the American Cancer Society. He coordinated a poker tournament and a casino night that raised several thousand dollars to benefit the KGI team. He worked with the KGI Student Government to make the Casino Night an annual event to benefit the KGI Relay for Life team and without this fundraising event the team would not make their annual goals. The KGI team has been in the top five fundraising teams since participate began three years ago. Rajan has also been involved with IPhA as part of the Political Action Committee, Program Co-Coordinator for the Mentor-Mentee Program, as master of ceremonies for events, and as a KGI student liaison. He often represents KGI at IPhA events by providing information about what KGI has to offer, but also by demonstrating the level of professionalism expected at the KGI School of Pharmacy. Because of these and many other achievements, Rajan has received recognition from his classmates and from some of the professional organizations for his continued dedication to leadership and service to pharmacy. Rajan was the first recipient of the KGI Distinguished Service Award, for the School of Pharmacy. This award recognizes an outstanding student leader who has distinguished him or herself by contributing to the Institute and its reputation of excellence. Any student from KGI is eligible for the award and the recipient must be endorsed by a faculty member and the student body votes for the recipient. Rajan has also been the recipient of a scholarship from IPhA not once, but twice, in 2016 and 2017. The scholarship is awarded to five students annually, for their involvement and service to pharmacy. It is rare for a student to win this award more than once. Rajan continues a leadership role with KGI as part of the Class of 2018 leadership team. Even while on rotations, Rajan remains engaged and in contact. He remains as a student voice and advocate for his class in addition to continuing to be a resource and mentor for other student leaders. Rajan was on the inaugural KGI California Pharmacy Student Leadership (CAPSLEAD) team; he has mentored students on subsequent KGI CAPSLEAD teams. He is a founding member of Phi Delta Chi and continues to remain involved with the chapter as an active brother. It is not just because of Rajan’s previous activity and achievements as a student leader that we write this letter of recommendation. Rajan continues his commitment as a student leader, to KGI School of Pharmacy and to the profession while completing his experiential education. Many student pharmacists become less or completely disengaged with their institutions while on rotations. Rajan has proven to be different and taken the lesson of, “Remember to look back and put a hand out to help a student in a class behind you” to heart and action. Because of these and

Page 14: CPhA Student Pharmacist of the Year Nomination Form · CPhA Student Pharmacist of the Year Nomination Form ... and parenteral nutrition therapies Compounding TPN using EXACTAMIX Automated

Page 3

many additional reasons, it is without hesitation that we recommend Mr. Rajan Vaidya for the California Pharmacists Association Student Pharmacist of the Year award. Please feel free to contact us if you have any questions. Sincerely,

Kathy D Webster Founding Dean and Professor,

Kristen D. Felthousen Associate Director of Student Affairs, School of Pharmacy

December 2017

Dear CPhA Awards Committee member,

Rajan not only ‘demonstrates a willingness to become involved in the future of our profession’, he is already involved in influencing the future of pharmacy through his leadership of CPhA, APhA, and various other organizations. He’s not just ‘involved in community projects’, he is creating and leading significant community projects across the state to ‘bring credit to our profession’ and impact public health. Finally, as an inaugural leader at KGI, Rajan is leaving a lasting legacy at the entire university, pathing the way for future leaders and ‘advancing his school’ outside what is expected of any student leader.

In the past four years and in my staff role for CPhA, I have had the privilege of working with Rajan on various initiatives. I have observed him contribute a seemingly impossible measure of time and leadership to various organizations, campuses, and students. Rather than focus on his many, many achievements, I want to paint a picture of how Rajan accomplishes so much in each of the criteria mentioned above as this will shed light to the values and characteristics that make him so unique.

Rajan, similar to other remarkable student leaders, has a passion and enthusiasm for serving others. However, Rajan is unique in that he possesses wisdom beyond his years. He is able to think strategically and globally, and then switch gears, to execute and coordinate with smooth professionalism. He is polished and able to communicate at all levels, to all persons, connecting with diverse audiences; often reminding me of leaders like CEO Jon Roth and ‘Pharmacist of the Year’ awardee Dr. Ryan Gates. These are leaders who can communicate with high level executives while also connecting with the public and patients. I observed these strengths in Rajan’s execution of Rx Boot Camp, where he coordinated and hosted student leaders across the state; in his co-founding of ‘California Pharmacists Outreach Week’ where he collaborated with legislators and students from all CA schools; and in his numerous speaking engagements across CPhA, IPA, and KGI.

Despite being so refined in his abilities to communicate, Rajan also has the ability to build and foster relationships, often reminding me of mentors and leaders like Ralph Saroyan. It is not enough for Rajan to be out front, leading others. He adapts and makes time to meet people where they are; demonstrating a rare trait among leaders. This was best revealed to me in his role as CPhA Student Trustee. Besides leading his inaugural CPhA-ASP chapter toward a ‘Chapter of Excellence’ recognition in their 1st year and despite being relatively new to our CA student board, Rajan became Student Trustee in his 1st eligible year. In this role, Rajan far surpassed my expectations. For example, he visited neighboring universities, in my place as staff, to support the development of new chapters. He connected very well with new students, personally mentoring several student leaders. During my busiest days as a new father, Rajan actually made the time to visit in my hometown of Visalia, driving about 4 hours each way; this showing his dedication to CPhA and his colleagues, and his support of a slightly stressed out friend. He has visited on numerous occasions to discuss association activities, always bearing gifts for my little ones and willing to babysit them as his own. He did all of this as a full-time student pharmacist and leader! This is the type of friend and leader Rajan is, and I love him for this. He makes the time and effort to foster and care for relationships. I have been blessed to have his influence, and

Page 16: CPhA Student Pharmacist of the Year Nomination Form · CPhA Student Pharmacist of the Year Nomination Form ... and parenteral nutrition therapies Compounding TPN using EXACTAMIX Automated

the same can be said for CPhA. During my transition out of CPhA, Rajan ran for a 2nd term as Student Trustee. He did not do this for any selfish reason; he did this because Jon Roth and I (and many others) had asked him to. My replacement, Katie Dahl, was new to pharmacy. Fortunately, Rajan again stepped up to go beyond his job duties and help Katie transition in, ensuring the continued growth of our student chapters and membership.

At his core, Rajan is a service oriented leader. Like one of his nominator’s for this award, Dr. Nancy Alvarez, he has a heart for serving others and is the type of leader who will continue to empower others around him, taking the criteria of this award and leadership to the next level.

Thank you for your time in reviewing this letter and for your service on our awards committee. I have no doubt that Rajan will bring honor and further credibility to this award.

Sincerely,

Michael Shawn Conner, PharmD, MBA

December 4, 2017 Dear CPhA Awards Committee,

I highly recommend Rajan K. Vaidya for CPhA’s “Student Pharmacist of the Year”. Rajan has excelled in propelling the pharmacy profession forward and leading by example for his fellow student pharmacists, such as myself. I first met Rajan as a fellow Board of Directors representative in our 2nd year of pharmacy school. Since then, I have had the great pleasure of collaborating with him on various projects for many professional events. CPhA-ASP has served as a great foundation for his growth as a student pharmacist and healthcare leader.

Rajan continues to be a great advocate in encouraging student pharmacists to further grasp how advocacy and community service influence patient care. Rajan has always had a passion for pharmacy leadership in order to inspire others to realize their own potential. With some of his notable positions, including CPhA-ASP Board of Directors (BOD) representative and Student Trustee on CPhA Board of Trustees, he has been able to effectively utilize his highly personable character to connect and unite student pharmacists throughout California. As BODs, Rajan and I co-founded “California Pharmacists Outreach Week”, the first statewide initiative where California pharmacy schools collaboratively create events that directly serve the community. Understanding each school’s circumstances to participate, he sparked unique ideas in other BODs to ensure all chapters were included in the event’s success. During the following year as Student Trustee, Rajan utilized his BOD experiences such as this to establish this pilot event as an annual initiative. He has constantly encouraged student pharmacists, like myself, to recognize that we are all leaders of our chapters and are pioneers of our profession.

His vision in nationally advancing the profession heightened when he recently collaborated with Rear Admiral Pamela Schweitzer during his rotation at Centers for Medicare & Medicaid Services. The passion in his voice as he spoke in high regards about government efforts exuded his dedication to expand patient care services in order to improve public health. He was inspired by how pharmacists are also public health activists, a role that goes beyond the traditional job description. In addition, his open-mindedness led him to network with pharmacists from different organizations, including American Pharmacists Association (APhA) and Food and Drug Administration, who have helped shape his view of the profession. Rajan sees the benefits in utilizing these established organizations as platforms to better the community, which very few student pharmacists can envision. For instance, he is interested in learning how APhA intertwines patient advocacy efforts through interprofessional collaboration, in hopes to continue to bring that momentum of change back to CPhA.

Rajan strongly values the successes involving the collective efforts of his colleagues. Because of attributes evidenced above, he is the most forward thinking and well-spoken student pharmacist I know. He is an irreplaceable friend and leader in the pharmacy profession. I look forward to witnessing his future growth and successes in his career in patient advocacy.

CPhA Awards Selection Committee California Pharmacists Association 4030 Lennane Drive Sacramento, CA 95834

Dear CPhA Award Selection Committee Members,

I write in support of the nomination of Rajan Vaidya for CPhA Student Pharmacists of the Year.  I am pleased to have the opportunity to share my thoughts regarding this young man.  I have come to know this individual in the course of the last couple of years through the Association and to a greater depth in the last 14 months.  Thus, I feel comfortable sharing my observations pertaining to Rajan’s character and conduct.

It would be easy for the committee to simply see that this student has served on the CPhA Board of Trustees for 2 years and make its allocation solely and superficially on this fact.  However, it would be a complete disservice to this student pharmacist who demonstrates that he is much more than his student title might suggest.

I observe Rajan to be a caring and humble individual.  As part of his CPhA role, he traveled early to visit students at Chapman University School of Pharmacy (CUSP) and was supportive of their endeavors to establish an American Pharmacists Association Academy of Students of Pharmacy Chapter that would incorporate CPhA activities under its umbrella.  What struck me was his ability to connect to the CUSP student pharmacists without appearing to be “above” them.  He was generous in sharing information to help the chapter establish a fundraising footing.  Soon after, I learned that a number of students were in frequent communication with Rajan and spoke highly of his manner of interaction and his effort to see that these student pharmacists’ needs were met.

I find Rajan to be an effective communicator and representative of his constituents.  During CPhA’s strategic planning efforts during the summer of 2016, Rajan did not hesitate to speak up and contribute thoughtful input into the process.  He spoke about the Association’s need to create experiences for student pharmacists that would capture their attention and retain their membership because they would look forward to subsequent experiences.   His perspective resonated with more senior individuals around the table resulting in robust discussion on the matter.  I recall feeling happy about how well Rajan was representing himself and thought about the positive deposits he was making towards his professional reputation.  Furthermore, he happened to be on rotation in the Washington DC area and readily accepted an invitation to accompany me on a visit to Capitol Hill as the CPhA CEO and I engaged in hill

visits to our California elected leaders.  Rajan offered his perspective to the various legislative aides with whom we visited in a measured and confident manner and was a wonderful complement to our team.

I find Rajan to be generous towards his peers and interested in their growth and development.  He was the single driving force for the establishment of a Phi Delta Chi Professional Fraternity on his campus.  He brought together campus administration with leaders within the fraternity so that they could learn about the vision and mission of the organization and how it might be a fit for its students, particularly as it related to leader development.  Then he stepped back and encouraged others to work to organize – in part because he knew that he would be going on rotations and he wanted to be sure that others would stand on their own.   He did not insist on grabbing all of the glory and notoriety that comes with being the “Alpha Class” chapter president.  As the chapter experienced expected bumps in the road as they formed the colony, students reached out to him to “solve the problem”.  Instead of taking over and solving the problem directly, he coached them (and sought to be coached himself by fraternity elders so that he could be effective) through situations between them and the chapter advisor.

These are a few examples to illustrate the character and behavioral attributes that I have observed.  I truly enjoy being around Rajan and appreciate his authenticity, particularly towards me.  Thus, writing this letter was easy and a pleasure.

Kind regards,

Nancy A. Alvarez, PharmD, BCPS, FAPhA Assistant Dean, Professional, External Relations, and Strategy Office of the Dean Assistant Professor of Pharmacy Practice  President 2017‐2018, American Pharmacists Association

December 1, 2017 Awards Committee California Pharmacists Association 4030 Lennane Drive Sacramento, CA 95834 Re: CPhA Student Pharmacist of the Year Dear Colleagues: I am very pleased to support the nomination of Rajan K. Vaidya for the CPhA Student Pharmacist of the Year award. This award recognizes a graduating student who is an active member of the ASP/CPhA student chapter and demonstrated a willingness to become involved in the future of the profession as evidenced by involvement in CPhA and APhA professional activities. The student pharmacist should be involved in community projects or other programs designed to bring credit and public acceptance to the profession. Additionally, the student pharmacist should be involved in advancing his/her school academically and professionally. In each of these areas Mr. Vaidya has performed exceptionally and I am hard pressed to think of someone more deserving of this award. I first met Mr. Vaidya as a first-year student pharmacist from the Keck Graduate Institute (KGI) School of Pharmacy as he took on multiple leadership roles for his inaugural class, including Class President and Founding Chapter President for the KGI CPhA-ASP Chapter. Rajan stood out as articulate and personable with a passion for becoming a leader within our profession. In addition to his leadership activities at KGI, Rajan has been an active student member of the Indian Pharmacists Association (IPA) for three years and has served as the Student Trustee on the CPhA Board of Trustees, an elected position voted on by his peers, for two years, which traditionally has only been a one-year term. He has also served on the CPhA Strategic Planning Taskforce, Legislative Committee, Policy Committee and this year, served as the Rx Boot Camp Co-Chair. Rajan has represented himself and the CPhA-ASP delegation professionally and with high quality work. In addition to these leadership activities, Mr. Vaidya has represented the pharmacy profession and KGI School of Pharmacy in the community. He has coordinated and participated in numerous health fairs, providing health screenings for the underserved population. The future of our profession is dependent on committed individuals who are willing to become involved, can lead others in the pursuit of excellence both professionally and personally, and who understand the need to encourage his student peers to become engaged in the profession and potentially become leaders in their own way. Rajan Vaidya exhibits all of these qualities and CPhA will be well served now and in future by student pharmacists like Rajan. I offer my strongest recommendation for your consideration of this outstanding individual and young professional for the CPhA Student Pharmacist of the Year Award. Sincerely yours,

Vinson C. Lee, PharmD, MS, FCPhA, FAMCP President, California Pharmacists Association
