When the office is closed after regular office hours you may call our office phone number at 585–546–1272 and select option #1 to be directed to our answering service for the on-call physician. You may also directly access our answering service by calling 585.258.4990. The on-call provider will contact the patient to assess and respond to the patient’s needs. Upon assessment, the provider may provide telephonic advice or prescribe as appropriate. For urgent problems the patient will be offered a next day appointment or directed to an urgent care facility. For emergent situations, the patient will be directed to the closest emergency room. Our Patient Portal Our Patient Portal provides you with secure, online access to portions of your medical records, as well as an easy and convenient way to communicate with the practice. What is the Patient Portal? Think of it as an online account to your doctor’s office, similar to online banking or credit card accounts you may have. Once you are logged into your portal account, the information you view pulls from your own personal medical record. Only you will have access to your health records, including any messages or information sent to you from the practice. Why is the patient portal important for me? With the patient portal you will be able to: Send and receive non-urgent messages and information from your doctor’s office, including test results and educational material. Easily notify the practice of changes to your personal information, including phone number, address, and insurance changes. • Request a refill for a medication prescribed by a provider at the practice. View upcoming and previous appointments, including the doctor and location of those appointments. View and/or print your current medications and allergies list. We take great care to make sure your health information is kept private and secure. Only you will have access to the login information needed to view your account. Your physician leads your care team which may include nurses, medical assistants, receptionists, specialists and others, but, most importantly, with you as the center of your team. Your Care Team at Alexander Medical GroupAlexander Medical Group is totally committed to your health.• Our office hours are listed on the back of this brochure but you can get clinical advice 24 hours a day/ seven days a week during and after office hours.• By learning about you and your health history we can design treatment options specifically for your good health and focus on preventive care to keep you as healthy as possible.• It’s important to us that you have the information you need to understand your conditions and how to take care of yourself so you can make informed decisions about your care.• We help you coordinate your care across the health care system, making appointments for you when appropriate and making sure that specialists have the information they need to care for you.

• Our staff uses technology like our patient portal to share records with you and your other health care professionals. This ensures that your health records are maintained as much as possible in one place and helps avoid duplicate testing.• Physicians, care manager, medical assistants and others meet to review charts of our patients scheduled for appointments and to identify unique patient needs.• We are alerted when our patients are discharged from the hospital and will contact within 72 hours to ensure that discharge instructions are understood and patients have appropriate transportation for follow up appointments. • We closely review our records to identify patients who are at high risk or have chronic diseases such as diabetes or heart failure and we make sure our patients have the information and services needed.• Automated courtesy calls remind patients of appointments and the importance of scheduling preventive care services such as mammograms and colonoscopies.

Gary Glazer, M.D., F.A.C.P.Dr. Glazer holds his M.D. from the University of Rochester School of Medicine and Dentistry and completed his residency at Highland Hospital. He is a respected physician, published researcher, and dedicated teacher. His passion is working with his patients towards personal weight loss and fitness goals

Shyla Krishnan, M.D.Dr. Shyla Krishnan completed her medical education in India, and her medical residency program at University of Rochester School of Medicine and Dentistry where she serves as a Clinical Instructor in Medicine.

Sylvia Park, M.D. Dr. Park is a graduate of Johns Hopkins University, received her M.D. from the University of Rochester School of Medicine and Dentistry where she currently serves as a Clinical Senior Instructor in Medicine. She also completed her residency at Strong Memorial Hospital.

David Stornelli, M.D., F.A.C.P. Dr. Stornelli received his M.D. from the University of Rochester School of Medicine and Dentistry and completed his residency in Internal Medicine at a Johns Hopkins Medical Center. With a longstanding passion for

health care reform, Dr. Stornelli has served as President of the local chapter of Physicians for National Health Program, and is the President of the Highland Hospital Medical Staff.

Learn about how your lifestyle choices affect your health and make sure you fully understand the benefits offered to you through your health care insurance.• Participate in your care – Follow the plan that you and your medical team agree on for your best health. Share with us any barriers that may arise during your treatment plan. Take medications as prescribed and keep scheduled appointments or reschedule them when necessary.• Communicate with your care team – Ask questions that you have about your care and make sure you get the answers you need and fully understand them. Bring your list of medications, herbal supplements and vitamins to your appointments. Let us know if you are cared for by other health professionals. Be open and honest with us.
