Policies sold after 1992 are stan-dardized. This means a Plan F
from on ompany will hav thsame benets as a Plan F fromanother company. Policies issuedprior to 1992 may differ somewhatfrom ompany to ompany.
Effective June 2010, the Medi-care Improvements for Patientsand Providers Act (MIPPA) madehangs to Mdiar supplmnt

Mdigap plan or th amount paid by th onsumr.
Formulary - This is a list of rtain kinds of prsription drugs that a Mdiar drug
plan will ovr, subjt to limits and onditions.
Medicare - This is a federal insurance program for people age 65 and older, certain
disabled people under 65, and certain people with permanent kidney failure.Medicare Advantage - Ths plans riv a prpaid monthly sum from th fdralgovernment to provide care. The benets include services available under Medicare. The insurance company or HMO will provide all your benets instead of Medicare.Medicare Advantage typically offers an HMO, PPO or FFS plan.Medicare Health Maintenance Organization (HMO) - In a Medicare HMO plan, you receive benets from an insurance company, not from Medicare. These plans donot provid standardizd Mdigap ovrag. enrolls must us a rstritd pro-
vider network within the health plan. Many of these plans provide additional benetssuh as prsription drugs, vision, dntal and haring ar. Ths plans ar rgu-
latd by th cntrs for Mdiar and Mdiaid Srvis, not th Kntuky Dpart-
mnt of Insuran.
Medicare Preferred Provider Organization (PPO) - Mdiar Advantag plan
choices have expanded to include regional PPO plans. Regional PPOs offer morechoices for Medicare health coverage. PPOs can help you save money by choosingfrom dotors and providrs on a plans prfrrd list, but usually dont rquir you
to gt a rfrral.
Medicare Fee-for-Service (FFS)/Private Fee-for-Service (PFFS) -Ths ar
halth ar plans offrd by privat insuran ompanis. Th insuran ompany,
rathr than th Mdiar program, dids how muh it pays and how muh you pay
for srvis. Ths plans typially do not rstrit you to a providr ntwork.
Medigap Insurance - This is also known as Mdiar supplmnt insuran. This
insuran pays for ovrag not providd by Mdiar. Mdigap insuran is not a
dupliation of Mdiar and may pay part of th ddutibls, o-insuran, srvis
and xpnss not ovrd by Mdiar.
Medicare SELECT - Ths ar standardizd Mdigap plans with rstritd providr
ntworks. You must go to a partiipating providr ntwork, xpt in mrgny situ-
ations.
Open Enrollment - This is a six-month period following enrollment in Medicare PartB. If you are covered by your employers health coverage and turn 65, your enroll-ment in Medicare Part B can be delayed until you retire. Keep in mind that if you areovrd by a small mployr group of lss than 20 mploys, Mdiar is primary
and would pay benets before your employer plan would. If you have a small groupmployr plan b sur that it will ovr what Mdiar would hav ovrd if you do
not enroll in Part B.
