I. Signature SPONSOR: $5,000The Signature level sponsorship provides the opportunity for your company to experience these benefits offered at an individual CHA leadership education program as well as visibility on its website:

• Corporate name and logo with hyperlink on the CHA Education Services website section for six months.• Corporate logo on all promotional materials sent to over 500 healthcare professionals.• Opportunity to introduce one of the speakers.• Corporate logo on a full-screen display during meals and breaks.• Draped display table for product/service promotion.• Two Program Sponsor representatives to attend program at no charge.• Verbal acknowledgement of sponsorship during the program.• Advertising space in the program materials.• List of attendee names, titles, organizations, and addresses in Excel format.

II. Speaker SPONSOR: $2,500The Speaker level sponsorship provides the opportunity for your company to experience these benefits offered at an individual CHA leadership education program:

• Company recognition as the sponsor of one of the presentations made at a CHA leadership education program.• Opportunity to introduce one of the speakers.• A vendor display table.• One Speaker Sponsor representative to attend program at no charge.• Verbal acknowledgement of sponsorship during the program.• Advertising space in the program materials.• Attendee list.

III. Contributing SPONSOR: $1,500The Contributing level sponsorship provides the opportunity for your company to experience these benefits offered at an individual CHA leadership education program:

• Your company will be recognized as the sponsor of breakfast, lunch, or a refreshment break.• A vendor display table.• Verbal acknowledgement of sponsorship during the program.• Advertising space in the program materials.• Attendee list.
