CONGENITAL GINGIVAL GRANULAR CELL TUMOR

A pedunculated growth on the anterior maxilla or mandible of newborns that is composed of granular cells.

The congenital gingival granular cell tumor is also known as:1. Congenital epulis of the newborn2. Congenital granular cell epulis

An infant with a pedunculated, multilobular lesion of the anterior alveolar ridge.Tissue of origin is still controversial.

CLINICAL FEATURES1. Lesions are found only in newborn2. Lesions mostly occur in anterior maxilla and

occasionally lesions are found in the anterior mandible

3. Occurrence : Females > Males4. Lesions are pedunculated, originating from

crest of alveolar ridge5. Texture: Smooth surface6. Size: several cm in diameter(1-9)7. Colour: Pink

Hematoxylin and Eosin (H and E) stained section show uniform pattern of granular cells with outer surface of atrophic stratified squamous epithelium

HISTOPATHOLOGY

Page 7: Congenital gingival granular cell tumor

• Large, rounded cells

• Granular cytoplasm• Rounded nuclei

High-power microscopy show

Page 8: Congenital gingival granular cell tumor

• Lesion is composed of sheets of large cells with a granular cytoplasm and vascular, noncollagenous stroma.

• Lesions contain autophagic vacuoles• Lesions are negative for S-100 protein

Page 9: Congenital gingival granular cell tumor

TREATMENT• Surgical Excision that includes the base of stalk• Recurrence is rare

Page 10: Congenital gingival granular cell tumor

SUMMARY• Newborns• Composition: Granular cells• Lesion is multilobular &

pedunculated• F>M• Colour: Pink• Smooth surface• Size: 1-9 cm• Treatment: Surgically excised• Reoccurrence: Rare
