CENTRAL LUZON STATE UNIVERSITY Student Union Building, Science City of Muñoz, UNIVERSITY SUPREME STUDENT CONCEPT PAPER TITLE OF ACTIVITY: Enrollment Assistance Program for Incoming Freshman Students DATE: May 18 - 29, 2015 VENUE: Central Luzon State University RATIONALE The starting Academic Year 2013-2015 the University Supreme Student Council is dedicated to assist the Enrollment Period through providing different services that will extend aid to the students. One of the programs being provided by the University Supreme Student Council was providing food for the volunteer and encoders at the Office of Admission and Information System Institute, until we decided to provide additional services such as providing free Viewing of Movies, Candies, Cold and Hot Water for Coffee, Choco and other drinks. The council also provides free Printing of Assessment Forms and OAd Form 6 (Official Enrollment Form) to the students together with the installation of Action Desk. OBJECTIVE To provide food for the volunteer and encoders at the Office of Admission and Information System Institute. To provide free Viewing of Movies, Candies, Cold and Hot Water for Coffee, Choco and other drinks.
