I don't know who invented the pillbox. Or why they

thought the combination of cramped spaces and

elderly fingers made for better medication

management.

It seems like we demand the most from the patients

who are least able to manage on their own — and

who most need to stay adherent. The answer? Using a

familiar solution in a new way. One that quickly

and accurately creates customized, easy-open,

multi-dose packaging – and provides an easy way to

confirm our patients took their medications.

I’ve always heard necessity is the mother of invention.

But I like to think that common sense is the brainchild

behind the best ideas I bring to my patients.

Lately there has been a lot of press and federal-level attention to the abuse of prescription drugs. Underscoring the extent of the problem is that more

deaths now occur from the abuse of prescription drugs than from auto accidents.

All states but one now have prescription-monitoring programs (PMPs) that are designed to detect doctor shoppers. While 30-some states have the authority to alert prescribers and pharmacies when a person meets a trigger threshold, not all the states send out such alerts. One reason is that the PMPs operate with very lim-ited budgets, making it difficult to initiate a proactive alert program.

What this means is that the information being reported to the states by pharma-cies on the controlled substances dispensed is being underutilized.

Steps are being taken to address this problem. One is the National Association of Boards of Pharmacy’s InterConnect program. One purpose of the program is to facilitate the sharing of data among states. Because doctor shoppers will have pre-scriptions written in one state and filled in another, InterConnect is set up to help detect this. Another step is to encourage prescribers and pharmacists to increase their use of the data prior to writing or filling a prescription.

Where states are sending out alerts, these are sent via email, fax, or regular mail. When a prescriber or pharmacist wants to query the database, this means stepping out of the workflow and using a Web portal. There needs to be a more convenient way for the query to happen.

The American Society for Automation in Pharmacy (ASAP) developed the stan-dard that every state requires pharmacies to use in reporting controlled substances dispensed. If a standard could be developed to access a PMP’s database right from the pharmacy management system, this convenience could very well increase us-age when a pharmacist suspects a particular person of doctor shopping. Building on its expertise with PMPs, ASAP convened a workgroup of stakeholders, which included a number of PMPs, to develop an XML Web service request/response standard that can be readily incorporated into a pharmacy management system.

ASAP has also developed a standard that will improve the timeliness of proactive alerts that PMPs send out when a person of interest is identified.

There is no question that ASAP is making an important contribution in address-ing a serious national problem. I wanted to make sure the organization gets the recognition it deserves for this. CT

Modular ArchitectureThe modular architecture of our systems enables us to create cost-effective, scalable solutions that grow along with your pharmacy. Unlike “one size fits all” systems, an OPUS-ISM system is tailored to your specific needs. Our account managers will help configure the system that’s right for you.

Built for outpatient pharmacies, these systems offer all of the functionality you expect from an industry leader, including centralized hosting and support for multiple locations.Central profiling, third-party management, cost control and reporting, and HL7 interfaces are just some of the many available features. We also offer innovative options such as RxVue and Announce Rx to improve the way your pharmacy interacts with physicians and patients.RxVue provides a secure portal that is accessible from a computer or tablet, allowing physicians to review patient medication histories and write or discontinue orders, which are transmitted directly to the pharmacy system. Announce Rx can increase patient satisfaction and reduce waiting-area crowds by alerting your patients via LCD displays as soon as their prescriptions are ready.

Key Features The touch-screen interface of ECRS Catapult POS is user-friendly and easy to learn. Catapult’s enterprise capability makes it a great choice for phar-macies with multiple locations that need the ability to produce centralized reports. Catapult also includes the ability to load an OTC catalog to ensure accurate pricing throughout locations. Equally important as the ease of use and back office capabilities of Catapult is the customer support ECRS provides. During our re-cent pharmacy management sys-tem upgrade, the staff of ECRS worked very hard to develop a fully functional interface prior to implementation. Following implementation, any unforeseen issues that arose were quickly resolved by the support team.

User-friendly system makes it easy to train new employ-ees. The workflow system adapts to the number of staff, which increases efficiency. Pill imaging for each refill is helpful for counseling pa-tients when there is an NDC change. The auto-refill process enhances patient compliance. All the pharmacists utilize the Clinical Pharmacology link for provider and patient medication questions. The technicians find the All Rx Status key a quick, easy way to see where all the prescrip-tions for one patient are in the workflow.

The InterRx program allows us to access patients with either a badge number or name. This was a customization that we request-ed. The ability to access all patient information from the prescription input screen is very user-friendly.

Management reports and finan-cial reports help us to know the status of business in a concise, easy-to-read format.

Our system integrates efficiently with our IVR and also with the Surescripts module, making the workflow efficient.

Most important is the personal attention given by the OPUS-ISM staff whenever the need arises concerning a question or prob-lem. I have not experienced an outsourced situation for customer service, which I appreciate greatly. They are very knowledgeable about their product and commu-nicate that clearly.

We automated with Parata Max to handle increasing volume without adding labor or expanding our pharmacy footprint. Max allows us to dispense with high accu-racy, speed, and efficiency, and to provide prompt, high-quality service. Since we automated, our volume has more than doubled to 1,500 prescriptions a day, and we have not added production labor. We use one Max unit, and it reli-ably meets our high production demands. It also frees up our time to counsel patients, perform medi-cation therapy management, and implement social programs to ad-dress health disparities in the local population. GLFHC serves com-munity health centers and provides outpatient and ambulatory care.

Over the years, we have incorporated nearly every QS/1 module into our pharmacies. Out-of-the-ordinary use includes running multisite manage-ment, IVR, Web portal refill, an inter-face to ScriptPro robotics/workflow, an interface to the Asteres ScriptCenter automated pickup point, an inter-face to the MTS On-Demand 400 for adherence packing, and an interface to our AmerisourceBergen central-fill pharmacy. QS/1 works well in our complex network, and they are happy to work with us as we explore new frontiers in pharmacy practice.

StarLite and StarPlus (POS software at store level) has an easy-to-implement interface with several pharmacy systems. Features I like include signature capture for Rx sales, HIPAA, controlled sub-stance, credit/debit card, nonsafety caps, and refunds using the Verifone MX880 signature pad. Combined signature for Rx sales and credit card. Credit/debit card and electronic check processing integrated into the application. Configu-rable user-security settings. Biometrics log-in. Ability to manage discounts for different customer types (senior, em-ployee). Module to manage and enforce pseudoephedrine sale restrictions. Touch-screen and ability to run POS and pharmacy system on same computer with no impact to performance.

StarLink (multilocation headquarters package) allows head office report-ing, product and pricing management, controlled-substance management and reporting, and, coming soon, central em-ployee management. Utility to automati-cally update product pricing by store, region, or chain. Sales tax management for city, county, state, and other special taxes.

RxMedic TCGRx Beacon

ScriptPro Pharmacy Management

Years System Used 1 Year N/A 7 Years

Key Features We were a QS/1 customer, and the addition of RxMedic robotics allowed us to provide total care with one seam-less integrated system that services both our retail and home delivery customers. The RxMedic technology provides photo verification, which is an important part of our QA process. The RxMedic ADS provides a finished prescription for our retail service side, and the RxMedic ACS provided us with a cost-effective alternative to the ADS and addressed our home deliv-ery needs.

Inventory tracking down to the pill level, including lot number and expiration date, has really allowed us to tighten inventory controls and increase turns. One feature our pharmacy teams really like is that all of the processes of the Beacon control system use pick-to-light to drive the user to the right product, including the product that is going to outdate the soonest, which has reduced our outdates.

The tight footprint also made our stores’ teams more efficient and freed up valuable space that we used to incorporate comfortable patient waiting areas, counseling rooms, wellness rooms, and com-munity rooms.

Ultimately, we were looking for a system that would free up valu-able space and minimize the time that our pharmacy team spends on dispensing activities so it can spend more time with patients to improve health outcomes through such activities as immunizations, medication therapy management, and health screenings. The TCGRx Beacon system has provided that and more.

Interfaced with Cerner Hospital system for patient data transfer (address, phone, weight, aller-gies, insurance); full-scanning capability of paper prescrip-tions; e-prescriptions populate matching fields to decrease time entering information; color pictures of drugs/products and line drawings of tablet mark-ings available for drug checking; automated inventory control based on usage, with electronic tracking for 340B program; barcode-driven for safety; multiple people can work on an order with each transaction logged as to who performed the task; batching of entire prescrip-tion order with multiple places of storage — hanging bags, refrigerator, large bags; closed-loop point-of-sale system; good customer support with any issue; able to do weight-based dosing, a huge advantage in a pediatric hospital setting; used in multiple locations within the hospital — outpatient phar-macy, emergency department pharmacy, pharmacy business office, and two off-site phar-macy locations.
