HeartSafe Moore County Saving more lives with a community-wide AED network

There are hundreds of thousands of out-of-hospital deaths that occur each year in the United States from sudden cardiac arrest (SCA), so there is a great em-phasis on innovative programs that reduce time to car-diopulmonary resuscitation (CPR) and defi brillation. HeartSafe Moore County is a group of dedicated individuals who are making it their mission to improve the health and safety of their county’s residents and visitors. The group is implementing and expanding an effective automated external defi brillator (AED) public access defi brillation program (PAD) in Moore County, North Carolina. SCA can strike persons of any age, gender, race, and even those who seem in good health. It is one of the leading causes of death in the U.S. claiming nearly 320,000 lives each year – about 1,000 people a day – or one every two minutes. SCA is extremely deadly, with a mortality rate of approximately 95 percent. SCA occurs without warning. The heart suddenly stops beating abruptly. When this occurs, the heart is no longer able to pump blood to the rest of the body. Blood fl ow to the brain stops and the person loses consciousness in seconds. Without help death occurs in minutes. SCA is usually caused when the electrical impulses in the affected heart become rapid (ventricular tachy-cardia, or “VT”) or chaotic (ventricular fi brillation, or “VF”). The general public and media often mistak-enly refer to SCA as a “massive heart attack.” When an individual collapses from SCA, immediate

cardiopulmonary resuscitation (CPR) and the use of an automated external defi brillator (AED) are optimal for recovery. AED’s are failsafe, user-friendly, and will not cause injury to either the user or the victim. The AED will not deliver a shock unless it is needed. For patients in VF, if defi brillation is provided within the fi rst minute, the odds are 90 percent the victim’s life can be saved. After that, the rate of survival drops 10 percent for every minute. After 10 minutes, the chance of survival is only fi ve percent. As many as 30-50 percent of SCA victims would survive if CPR and an AED were used within fi ve minutes of collapse. According to the American Heart Association, 50,000 deaths from SCA could be pre-vented each year. The national average emergency medical service (EMS) response time is 8-12 minutes, which is too long for most victims to recover. That is why it is critical to have an AED immediately available for fast rescue. Since two-thirds of SCA deaths occur at home, at work, or during recreation – outside a hospital set-ting – it is critical that communities develop a Heart-Safe emergency plan. As advocates for public access defi brillation pro-grams, HeartSafe Moore County has one goal – to save lives through early defi brillation. The HeartSafe PAD program serves as the foundation for the Moore County AED network. Their approach includes AED access, medical over-sight, AED training and basic life support, EMS coor-dination, AED maintenance, and program monitoring to ensure training. In addition, the group provides policy, procedures, and emergency plan templates for effective use of AEDs. The group worked to identify high-value targets – those buildings and locations that represent the greatest potential for an SCA to occur because of the volume of human traffi c. Some of the primary targets include: schools, workplaces, churches, golf courses, hotel/motels, restaurants, athletic venues, and any other location where people gather. The organization ensures that each location has the required physician AED prescription, medical direc-tion, and assurance that each location is in compliance with state and federal regulations.

HeartSafe Moore County also partnered with the local chapter of the American Red Cross as well as other entities to provide AED and basic life support training. Organizations pay for their training and then report the certifi cation to the county’s public safety department for tracking. The county’s EMS support is vital to the program’s ongo-ing success. EMS personnel have access to information regarding the location of AEDs. When a dispatcher receives a 911 call about a potential heart attack, he or she can direct the caller to the nearest AED in an effort to maximize treatment during the fi rst critical minutes following an attack. The public safety department is charged with the annual inspection of all registered AEDs. The depart-ment notifi es the business or organization if the AED battery or pads need replaced. In addition, they have details on each location’s training activity and monitor the training certifi cations. Collaboration between the medical community, pub-lic safety, businesses, civic groups, non-profi t organi-zations, and citizens, makes it a success story to repeat in other communities. HeartSafe Moore County also has an agreement with Cardiac Science, a leading distributor of AEDs, to ensure the community has access to a quality, cost-effective product. HeartSafe Moore County is working to make AEDs widely- and readily-available throughout Moore County with special emphasis on installations for pub-lic schools, public recreation areas, churches, busi-nesses, and in general, in areas of high human traffi c. A community-wide fundraising effort began in late 2007 and businesses, civic organizations, and individ-uals stepped up to make gifts of AEDs to the schools, the primary target for installations. In addition, HeartSafe Moore County teamed up with the local

hospital’s foundation for grant-writing assistance and received funding through the Moore County Commu-nity Foundation and the Foundation of FirstHealth for AED installations at the schools. In just over one year HeartSafe Moore County met its primary goal of installing at least one AED in each of the county’s public schools. The group anticipates that by the end of 2009 all public schools will have the additional AEDs needed to meet the optimum response time (within fi ve minutes) in the event of an SCA. In addition, the busy Southern Pines downtown busi-ness and shopping district completed the installation of AEDs on several street corners. These AEDs provide easy public access in the event of an emergency and earned Southern Pines the designation of a HeartSafe Community, one of the fi rst in the state. “We hope to duplicate this effort in other areas in Moore County, such as Pinehurst,” explains Buddy Spong, executive director of the Moore County chap-ter of the American Red Cross and vice-president of HeartSafe Moore County. “It protects not only our citi-zens, but those visitors who come to shop, play golf and to enjoy all the other things that Moore County has to offer.” Community awareness of SCA & the need for AEDs is growing – rapidly. Churches, private schools, busi-nesses, restaurants, hotels, & golf courses have been among more than 150 installations in the county since

(continued...)the group launched its effort in the winter of 2007. “People understand there is a real need for an AED – that the EMS truck will most likely not get to the SCA victim in time,” says Spong. “AEDs need to be as ubiquitous as fi re extinguishers – and within anyone’s reach in a fi ve-minute response turnaround so that there is a real chance of saving a life.” HeartSafe Moore County’s AEDs are a cost-effective tool in treating SCA. By working to share information about the value of the technology, HeartSafe Moore County hopes to encourage many businesses and com-munity organizations to place AEDs in their public buildings and community areas. It really is a simple and effective way to keep more hearts beating.

Alka-Seltzer • Clean a toilet. Drop in two Alka-Seltzer tab lets, wait 20 minutes, brush and fl ush • Remove burned food from cookware. Fill your cookware with warm water, then dissolve six tab lets and allow to sit for an hour or so. • Sooth Inset bites. Dissolve two tablets in water and saturate a cloth or cotton ball with the solu- tion, and place on the bite for 30 minutes. Don’t do this if you are allergic to aspirin, which is a key ingredient in Alka-Seltzer

Banana Peel • Polish leather shoes with a de-stringed banana peel. Finish by buffi ng with a soft cloth or paper towel. • Fend off aphids by burying a dried or cut-up banana peels a few inches deep around the base of rosebushes. • Treat scraped skin. Rub minor scrapes with the inside of a banana peel to promote healing. Shine your houseplant leaves. Rub them down with the inside of a banana peel to remove dirt and to make them shine.

Aspirin • Remove perspiration stains from white fabric by dissolving two aspirin in half a cup of warm water and applying to the stained fabric. Leave for a couple of hours before washing. • Eliminate chlorine discoloration from hair by dis solving eight tablets in a glass of water and rub- bing into your hair. Leave fro about 10 minutes and then rinse and shampoo. • Reduce pimples by making a paste of water and crushed aspirin. Cover each pimple with this paste: rinse after a few minutes.

Un-diagnosed, diabetes can result in vision impairment, a frequent complication of both Type-1 and Type-2 diabetes, and blindness. Diabetic retinopathy, a diabetes-related eye disease, is the leading cause of new cases of blindness among adults 20-74 years old. Other vision problems caused by diabetes include: vision changes, glaucoma, and cataracts. bleaching) is a simple, non-invasive dental treatment used to change the color of natural tooth enamel and is an ideal way to enhance the beauty of your smile. Because having whiter teeth has now become the number one aesthetic concern of most patients, there are a number of ways to whiten teeth. The most popular method is using a home tooth whitening system that will whiten teeth dramati-cally. Since tooth whiten-ing only works on natural tooth enamel, it is impor-tant to evaluate replace-ment of any old fi llings, crowns, etc. Replacement of any restorations will be done after bleaching so they will match the newly bleached teeth. Tooth whitening is not permanent. A touch-up maybe needed every several years, and more often if you smoke, drink coffee, tea, or wine. The trays are worn with special whitening solution either twice a day for 30 min-utes or overnight for a couple of weeks depend-ing on the degree of stain-ing and desired level of whitening. If you had a 5% chance of developing a potentially fa-tal illness, and you could signifi cantly reduce this risk, wouldn’t you? Colorectal cancer (cancer of the colon or rectum) is the second leading cause of cancer related death in our country. It takes about 50,000 lives annually, and some-one dies from colon cancer in the United States about every ten minutes. About as many people die from co-lon cancer in the United States each year as they do from breast cancer and AIDS combined. Despite these statistics, there is so much reluctance to talk about colon cancer screening. Just the thought of getting a colon examination usually makes people squeamish. “You want to put that tube where?” is a common response. This March is the tenth annual National Colorectal Cancer Awareness Month. So really there isn’t a bet-ter time to talk about the importance of colon cancer screening. If there is anything good about colon cancer, it is that IT CAN BE PREVENTED. Most colon cancers begin

as polyps, small growths on the inside surface of the co-lon. Polyps come in different shapes and sizes, and they have different microscopic characteristics too. Certain polyps, called “adenomatous” polyps, have the poten-tial to turn into cancer. Unfortunately, colon polyps usually do not cause any symptoms. So even when you may feel well and appear to be in good health, colon polyps might be forming and indolently progressing into a life threatening ma-lignancy. If, however, you get screened for colon cancer with a colonoscopy, your chance of getting colon cancer could be dramatically reduced. Those individuals who have colon cancer caught early have over a 90% chance of survival fi ve years after the disease is diagnosed. This can be accomplished with a 20 minute examination. No one can argue that getting a colonoscopy is fun or exciting. Most people hate going to the doctor’s offi ce, but after it’s over, most people who get a colonoscopy marvel about how easy it was and how they should have never worried about it in the fi rst place.

The day before the procedure is spend drinking a clear liquid diet, culminating with drinking a laxative in the evening in preparation for the exam. While this is usu-ally the most disagreeable part of the process, it is also the most important. The cleaner your bowel preparation is, the better your colon examination will be. While the standard preparation comes in a four liter jug, there are other options that you can talk about with your doctor. For years con-sidered a “mild-mannered”, “unassuming”, simple headache and pain reliever, aspirin recently has meta-morphosed into a multiple-purpose super pill that not only reduces the pain of arthritis, fever from infection and infl ammation due to illness or injury but is also be-lieved to help prevent heart disease and cancer. The bark of the willow tree was the original “aspi-rin”, used for centuries by people in many countries to relieve pain and fever. The willow contains a chemi-cal that is converted by the body to one of a family of compounds called salicyates. But it wasn’t until the last century that scientists reproduced the pain-relieving chemical in a laboratory. Since then, the white, odor-less, powder manufactured in the shape of a pill has be-

Several studies in recent years have shown that aspirin reduces the risk of heart attack and stroke among people with heart disease. More recently, it has been shown to prevent heart attack and stroke among low-risk groups as well. A study of 22,000 healthy male doctors showed that those who took one aspirin every other day for fi ve years had 44% fewer heart attacks than the group that didn’t take aspirin. Even more recently it has been shown to reduce stroke in women. Evidence is also building that aspirin may help cut the risk of colon cancer, although scientists aren’t sure exactly why. In a study a few years ago of over 662,000 people, scientists found that the risk of colon

cancer death was about 50 percent lower among those who took aspirin 16 or more times a month than among those who didn’t take it at all. The scientists suspect that aspirin slows the growth of pre-cancerous growths in the colon. It may also fi ght colon cancer by helping to boost the body’s immune system and block the body’s production of a hormone-like chemical that can promote cancer growth. But with all of aspirin’s powers of prevention, it has its negative side. It can irritate the lining of your stomach, causing it to bleed. More important, it can increase the risk of death in certain kinds of strokes – those caused by bleeding in the brain as opposed to those stemming from clots. This is because aspirin’s power to prevent heart disease is due to its ability to thin the blood. Thinner blood makes it less likely that your blood will clot. But in strokes caused by bleeding aspirin’s blood-thinning powers are counterproductive; they enhance bleeding, thereby increasing risk. Fortunately, there’s evidence that you can reduce these negative aspects of aspirin by taking considerably less, yet still benefi t fully from its protective powers against heart disease and stroke. Research indicates that a dose of 81 mg/day still offers the same amount of protection,

with less risk of bleeding, (one baby aspirin is roughly 80mg.). The bottom line is this; if you are a male over 45 and less than 79 or a female over 55 and less than 79 you may benefi t from aspirin’s preventative effects on heart attack and stroke respectively. Treat aspirin’s new-found powers of prevention with respect, use it spar-ingly, and check with your doctor before trying aspirin therapy on a daily basis. Of course there’s another even more fundamental way to protect your self against heart disease, and that’s by eating properly. Here’s another heart-friendly recipe that is also great-tasting.

Ingredients:Salt to taste ¼ cup canola oil1 ½ tbsp. pure olive oil ¼ lb. whole bay scallops4 C defatted chicken broth1 med-sized shallot, fi nely chopped 1 large onion, cut into large chunks 3 stalks celery, sliced thick on a diagonal5 small red potatoes cut in large chunks2 medium carrots cut into large chunks½ cup unbleached white fl our or whole wheat1 lb. white meat fi sh fi llets (cut into large chunks)6 med-sized shrimp, peeled, deveined & fi nely chopped

Preparation - Serves 6 to 8:1. In a large soup pot, heat the olive oil. 2. Add the chopped shal-lot to the hot oil & sauté until golden, about 3 or 4 minutes. 3. Add chunked onions, sliced celery, carrot chunks, fi sh chunks & salt & sauté for 5 more minutes. 4. Add defatted chicken broth & bring to a light boil over medium heat. Reduce heat to low & simmer for 15 minutes. 5. Add chopped shrimp, whole bay scallops & red potato chunks. Simmer covered, over low heat for 40 minutes. 6. Season to taste with salt & ground pepper & simmer for 5 more minutes. 7. Make a roux by mixing the canola oil & fl our in a small skillet. Using a wire whisk, stir constantly over medium heat until four begins to turn golden & become fragrant, about 8 minutes. 8. Slowly add the roux to the bisque, stirring constantly. 9. Cook over medium-low heat, stirring often, until bisque be-comes thick & creamy, about 5 minutes. 10. Serve immediately.

Moore Free Care Clinic first opened in April of 2004 in the Health Department in Carthage and in May 2009 moved into a newly renovated 3000 sq. ft. facility in Southern Pines. The clinic is a private, volunteer-based, nonprofit organization that provides primary health care services at no cost to limited-income peo-ple who lack health care insurance. Many of Moore County’s residents live at or below the federal pov-erty level and struggle with poor health. Many are employed but unable to afford health care or health insurance. Public assistance programs like Medicare and Medicaid, while valuable to the community, cov-er only specific segments of the population. Those who fall through the cracks of the public health sys-tem often seek care only in emergency situations as opposed to receiving routine follow-up appointments and prescription medication regimens to treat disease. The clinic serves residents of Moore County whose income falls at or below 150% of the federal poverty guidelines. The clinic interviews each individual to determine their eligibility before an appointment is scheduled. U.S. Citizenship is not a criterion for care. Comprehensive care is provided for acute illness and continuing care is provided for asthma, hypertension, diabetes, and other chronic illnesses. Numerous spe-cialty care clinics are held each month and the clinic can arrange necessary off-site specialty care referrals

after a preliminary exam and the recommendation of the clinic physician. Resource information, health edu-cation and limited laboratory and X-rays are also pro-vided. Patients needing costly medications are enrolled in the Patient Assistance Programs offered by various pharmaceutical companies to cover prescribed medica-tions. Seeking to expand the network of care, the clinic is in partnership with Sandhills Community Care Network that works with local health departments, hospitals, and social service agencies to better integrate the care for low income residents. They furnish the clinic with case managers to assist patients with a variety of health care-related issues. Our partnership with Sandhills Mental Health and First Health of the Carolinas allows us to provide mental health services for our patients and has been an important step in furthering comprehensive and holistic care of our patients. How can you help? The clinic relies on dedicated med-ical professionals and lay volunteers who are willing to make a quality difference in the health of the Moore County residents. We are looking for doctors, nurses, lab technicians, nurse practitioners, and physician as-sistants. In order for our bodies to function properly we need to intake between one and seven liters of wa-ter per day. Post Rehabilitative training is an important aspect of the healing process. With proper training and stretch-ing, in most cases you can regain full use of the muscle and joint. Examples of stretching exer-cises for the shoulder and rotator cuff would be, place a towel against the wall and make small circles each way and also include up and down movements (just like a windshield wiper). As the warm, summer months approach us, we want to remind everyone to protect your skin from the sun’s harmful ultra-violet rays. You should protect your skin all year round! Sunlight contains two ultraviolet radiation wave lengths that are harmful to our skin. Ultraviolet

B rays (UVB) penetrate and affect the epidermis layer of the skin. This is the outermost layer where our melanin production takes place. The fi rst de-fense for our skin against the ultraviolet radiation is to produce this melanin. This may often cause our skin to burn if we have been exposed to sunlight for a period of time. Ultraviolet A rays (UVA) penetrate the dermis layer of the skin. This layer is also known as our true skin. The dermis layer contains collagen and elastin fi bers and allows our skin to be pliable and supple. Eighty percent of fi ne

lines, wrinkles and sag-ging are caused as a result of too much sun expo-sure. The sun’s harmful rays penetrate into the skin, causing damaged collagen and elastin fi bers, photo-aging and precancerous actinic kera-toses. It has been found that by applying topical Vitamin C & E, you can repair some of the dam-age that has been done. This is known as antioxi-dant treatment. The best way to protect your skin from sun expo-sure is to wear a Broad Spectrum sunscreen that blocks both UVB and UVA rays. Also, be sure the sun protection factor (SPF) is at least a 15 or

higher. Protective cloth-ing such as hats, shirts, pants and sunglasses can also help shield the sun’s ultraviolet rays. Wear a cover-up, double-up on sunscreen and stay in the shade when it’s pos-sible. It is also benefi cial to avoid sun exposure during the mid-day times of 10am and 4pm. The sun’s rays are the stron-gest and do more damage during this time period. While skin cancer is on the rise, it is our goal at the Pinehurst Surgical Facial Plastic Surgery Center to educate the community and surround-ing areas on skin cancer awareness and sun pro-tection.

6TO REDUCE FALL RISKSIX TIPS...

The risk of falling each year after age 65 is about one in three. Falls are the leading cause of injury and injury-related death among older adults. However many falls and fall-related injuries are preventable. Here are six (6) tips (modified from the MayoClinic.com – “Tools for Healthier Lives”) to help you to reduce your risk of falling and injury.

1. Make an appointment with your doctorYou and your doctor should take a comprehensive look at your health, your medications, and your level of activity to identify situations that put you at risk for falling. Bring a complete list of all medications you are taking, or simply bring the bottles of medicine with you to discuss with your doctor. Many medica-tions have side effects such as dizziness that may con-tribute to a loss of balance. If you have fallen before, write down the details and share this information with your doctor. Keep your doctor informed if you experi-ence any dizziness, joint pain, numbness, shortness of breath, and/or changes in bowel or bladder patterns.

2. Keep movingIt is important to get regular physical activity. Walk-ing as much as you are able is one of the best forms of regular exercise.

3. Wear sensible shoesBuy properly fi tting, sturdy shoes with non-skid soles (no slip-ons). Avoid shoes with extra thick soles that may catch on carpeting and contribute to a fall. If you wear lace-up shoes remember to keep them tied. If you have trouble tying your shoes select footwear with Velcro fasteners.

4. Remove home hazardsBe sure there are clear paths especially to and from your bed to the bathroom. Move furniture, plant stands, etc from high traffi c areas. Use nonskid mats in your bathtub or shower.

5. Light up your living spaceKeep your home brightly lit (with energy saving bulbs) to avoid tipping on objects that are hard to see.Place a lamp or nightlight near your bed to assist you when you get up at night. Keep fl ashlights in easy fi nd places in case of a power outage.

6. Use assistive devicesYour doctor or physical therapist may recommend the use of a walker or cane to improve your balance. Ad-ditional grab bars in your bathroom may provide ad-ditional security. A raised toilet seat or shower bench can increase safety while bathing.

Meditation serves as a simple way to counteract the negative ef-fects that stress has on the mind and body. Although there are several variations to this ancient practice, the ultimate goal of meditation is to reach a

It seems to me that people of all ages are having to “poison” themselves just to stay alive. It sounds as-tonishing, yet from early morning people are loading up with harmful stimulants and toxic foods in order to step out into the fast-moving world, then fi nding themselves in need of ways to relax and slow down in the evening when they get home. This is, unfortu-nately, being seen as early as grade school, and as late as post-retirees who often times still need their health and vitality to produce an income. How do these amazingly harmful habits happen? Days begin with high sugar- containing doughnuts, cereals, juice, and sodas. A quick read of the newspa-per or TV news increases morning stress hormones, as does a quick glance at the computer, as one dashes out the door to go to a stressful job. Before 9 am, both the mind and body have been poisoned with artifi cial stimulants, yet one thinks they are ‘energized’ enough to proceed through the day. There may be no rest for the weary during work hours, but another sugar “pick-me-up” in the form of a candy bar or soda around mid-morning and afternoon makes what seems to them, a good energy bridge. Arriving home from work or school, one is still buzzing from the artifi cial daytime stimulants, or dragging from over-stress and mental exertions. It is now time to settle down for what many people never quite get enough of, sleep. But before it is time to turn in, there is more TV and computer visual stimulants mixed often with an alcohol beverage, and more di-etary sugar from dessert or evening snacks. It doesn’t have to be this way, but people forget that all it takes is to know (and remember to do) what our bodies need to maintain its own natural energy, to manage easily the day-to-day stress, and to have what it takes to prevent this kind of lifestyle from causing a chronic illness.
