NAICAM

What is Primary Health Care?

Primary Health Care is a framework for providing everyday health care services. Health care professionals work proactively with intersectoral partners and community groups to address broader community needs.

Primary Health Care works towards:

• preventing people from becoming ill or injured

• managing chronic conditions

• making the most effective use of health care provider expertise

• treating acute and episodic illness

• efficiency and co-ordination of services

• accessing the right service at the right time

• individuals playing an active role in their own health care

• understanding that factors outside the health system influence individual and community health (Determinants of Health)

Introduction

A Primary Health Care Provider Team has been in existence in Naicam since 2003. The Team consists of health professionals and community representatives who meet on a regular basis to address wellness needs within the community.

One way to identify wellness needs is through the use of a Community Wellness Assessment. The first assessment was conducted in May 2007 utilizing a community forum to identify areas of concern with 12 community members attending. The Team took the information gathered and developed action plans to address some of the identified needs. Any needs that were beyond the team’s ability to resolve were forwarded to the appropriate agency.

In 2009, a different approach was taken to conduct the assessment to connect with a wider cross-section of the community. The team went to the community to gather information versus expecting the community to attend a public meeting. A booth was set up at the two local grocery stores and shoppers were encouraged to voice their opinions on survey questions. In total 51 community members completed the survey questionnaire. The team also shared the information gathered with the local Mayor. The team then developed action plans to address some of the identified needs. Needs that were beyond the Primary Health Care Team’s ability to resolve were forwarded to the appropriate agency.

Primary Health Care Team and partners have accomplished the following since the last assessment:

• April 2010 partnered with community agencies to offer PARTY Program (Preventing Alcohol Related Trauma in Youth) in the school

• July 2010 Clinic staff hosted “Kiddy Kollege” tours of the clinic

• October 2010 offered Hypertension presentation

• September 2010 developed and circulated within the community, the Naicam Community services Directory pamphlet

• “Faces of Primary Health Care” posters posted within the community

• May 2011 presentation to the Seniors group regarding the upcoming Community Wellness Assessment

• May 2011 held a Diabetes Meter Clinic and Information Day

• May 2011 Noon Walking program

• June 2011 applied for an SGI grant and held a Bike Rodeo in partnership with the school and every child from grades 1 – 4 participated.

In 2011, members of the Primary Health Care Team were present at Naicam Shop Easy in the morning and Naicam Co-op in the afternoon to conduct the Community Wellness Assessment. Community members shopping at the stores were invited to complete the survey questionnaire. In total 119 surveys were completed.

The Team will be reviewing the survey results, develop action plans to address some of the needs and those beyond the scope of the Team will be forwarded to the appropriate agency/group. The final assessment report will once again be shared with the Mayor of Naicam.

New to the survey this year was the addition of questions 5. (e) and (f) – refer to attached survey questionnaire. The information gathered from these two specific questions will also be shared with Primary Health Care Program Awareness Team who are now investigating how community members are currently finding out about health information as well as how community members would prefer to find out about this information. This team will be making recommendations to health care teams on how best to create awareness of health information, programs and services within our KTHR communities.

• Mental Well-Being/Substance Use and Abuse

• and an additional category entitled “Other” (ideas beyond the scope of the Team)

*In May the Primary Health Care Clinic staff had asked clients accessing services at the clinic to complete the survey and team members had also taken surveys to various businesses and groups within the community of Naicam to complete.

Other comments: Fortunately I’m a healthy person so far. I would hate to lose any of the services we currently have. I think we are one of the more fortunate rural communities but I believe to maintain our current health services we need to support ALL our local services/businesses.
