What is a CommunityStroke Reviewer?

Community Stroke Reviewers providepersonalised information, advice & support forpeople affected by a recent diagnosis ofstroke.

Stroke Reviewers are qualified health or socialcare practitioners who can monitor patients’health and social care needs after their stroke– guiding them and their carers.

How do I arrange to talkto a Community StrokeReviewer?

When you are discharged home from hospital,the medical team will discuss the ReviewService with you and with your agreement,refer you to your local Reviewer. If you haveany concerns or wish to discuss somethingbefore your review is due, you can contact theservice - see details on the back of thisleaflet.

If you were not admitted to hospital at thetime of your stroke or you were treated orresiding outside Oxfordshire, please ask yourGP to refer you.

What can theCommunity StrokeReviewer do for me?

Your Community Stroke Reviewer will:

Review your progress six months afteryour discharge home. In somecircumstances review your progress sixweeks after your discharge home.

Link you with any services needed tofurther develop independence andchoice.

Provide advice and support onpsychological and social needs.

Provide a range of information to enableyou to understand the different aspectsof stroke and some of the associateddifficulties.

Provide information on local andnational support services.

Provide information and support forrelatives and carers.

Help you to find out about benefits andother financial matters.
