TAMPA GENERAL HOSPITAL PRESENTS The National Diabetes Prevention

& Lifestyle Change Program

This program is for people with prediabetes. This program is NOT for those already diagnosed with diabetes.

The Centers for Disease Control and Prevention (CDC) estimates that more than 79 million Americans age 20 and older, have prediabetes. The vast majority of people living with prediabetes do not know they have it. A person with prediabetes has a blood sugar level higher than normal, but not high enough for a diagnosis of diabetes. Prediabetes is a serious health condition that increases the risk of developing type 2 diabetes, heart disease and stroke.

Am I at risk?Many factors can increase your risk for prediabetes and type 2 diabetes. Risk factors can include:

• being 45 years of age or older• being overweight• having a family history of type 2 diabetes• being physically active fewer than three times per week• ever having diabetes while pregnant (gestational diabetes) or giving birth to a baby that weighed

more than nine pounds

The CDC-led National Diabetes Prevention Program, can help participants adopt the healthy habits needed to prevent type 2 diabetes. Participants will meet with trained lifestyle coaches from Tampa General Hospital over the course of one year to work towards the goal of losing five to seven percent of their starting body weight in order to reduce their risk for type 2 diabetes. The program emphasizes improving food choices, being physically active for at least 150 minutes each week, and developing skills to identify and overcome barriers to making lifestyle changes. Groups meet for 16 weekly sessions, followed by eight monthly follow-up sessions with a trained lifestyle coach.

This program requires a 16-week commitment and is followed by the opportunity for ongoing support.

Who Can Participate in the Program? If you have been referred by your doctor, have prediabetes, have been told by a doctor that you are at risk for developing type 2 diabetes, or have a body mass index (BMI) of 24 or higher, then you are eligible to participate in the National Diabetes Prevention & Lifestyle Change Program.

Tools to Quit is a two-hour seminar where you’ll receive information about how to select and prepare for your quit date. You’ll learn about what triggers you to smoke, dip, or chew, and how to handle withdrawal symptoms.
